(Critical) The following items were observed unclean: the blender at the bar (had not been used yet today and clearly was not disassembled when it was last cleaned), a kitchen knife appeared to have raw chicken residue on it, a colander with pasta residue in most of the holes, five to six 1/3 pans, two deep 1/2 pans, and numerous utensils were observed in the drawer below the prep counter that had an unclean liquid in the bottom of it. Corrected by cleaning all of these items. Ensure staff are fully disassembling equipment prior to cleaning and inspecting equipment for cleanliness after air drying items, but before placing them in storage
(Critical) Observed a server handling garnish for a plate of food with her bare hands. Corrected by discussing this issue with the employee and by providing tongs for this task
(Critical) Walk in cooler: raw pork sausage was stored above the beef roasts and steaks. Corrected by relocating the sausage away from the beef roasts and steaks and above the poultry. Two door refrigerator: observed raw gyro slices stored above ready to eat foods. Corrected by relocating to the bottom of this unit
(Critical) Observed au jus in the large walk in cooler to have expired on 9-23. Corrected by discarding
The interior surfaces of the ice making machine are in need of cleaning at this time
Observed multiple foods in the facility to be uncovered: two soups in the soup warmer, multiple food containers in the two door upright refrigerator, etc. Ensure staff maintain covers on food for protection (unless they are on the top shelf of a cooler actively cooling)
The following areas are overdue for a cleaning: 1) the floor sink behind the ice cream chest freezer, 2) inside the base cabinet of the soda dispensing machine, and 3) floor drains in the back kitchen area. Thoroughly clean these areas now and more frequently in the future to eliminate odors and possible fly breeding areas
The drain lines for the ice making machine have slumped down again. Re-secure them to provide the required 2" air gap above the floor sink.
Dish washing machine: the wash compartment has food debris accumulation in all corners and around piping. Also, the food trap screen and waste water drain basis are not being cleaned daily. Maintain these areas clean to prevent odors and possible fly breeding areas

(Critical) Observed three small food pans containing sausage patties, blueberries and grapes that were stored in a hotel pan used to catch dripping water from the cooling system above on the top shelf of the two door upright cooler. This dripping water was entering the three food containers. Corrected by discarding these adulterated foods. In the future, ensure staff are storing foods in a manner that prevents contamination
Observed a moderate gap located between the wall and the back-splash at the server station behind the toaster. Seal this gap to prevent food from falling behind this cabinet. The side-splash at the men's bathroom hand washing sink located under the soap dispenser to be separated and swollen from soaking up moisture. Remove and replace this material and then properly seal all joints to prevent moisture intrusion. Also, provide a large lexan or buss tub to place under the mop storage area to collect dripping water from the mop heads. This tub needs to be drained and cleaned daily
Observed a few area to be in need of cleaning at this time and more frequently in the future: the floor below the mop rack, the floor sink behind the chest ice cream freezer and under/behind the server area cabinets/equipment. Please increase the cleaning frequency of these out of the way areas
Observed a lot of water (condensation) dripping from the top of the two door upright cooler located at the end of the cook line. Repair this unit (drain hose, drip pan/shield, etc. ) to eliminate this water from dripping into the refrigerated food storage compartment
Observed multiple plastic kitchen food containers that were stacked on upper shelves prior to them fully air drying. Ensure your equipment is air dried prior to placing in storage
Observed two brooms and a couple other handled tools stored in a narrow space located between the boxed soda syrup and a wire storage rack for canned foods in the back storage room. Relocate these items to another space away from food/clean equipment
(Critical) Observed two partially full, covered plastic containers in the walk in cooler with chili in them at 48f around 10:30/10:45 am. This chili was made late the previous day. Corrected by discarding. In the future, ensure staff continue the cooling process longer and separate large batches of food into more containers
(Critical) 1) observed an employee not wash his hands after loading dirty dishes into the dish washing machine / prior to putting away clean dishes. Corrected by discussing this issue with the employee and by the employee washing his hands. 2) observed the cook not wash his hands after cracking raw, in-shell eggs or after wiping his hands on a cloth towel. Corrected by discussing this concern with the employee and by washing his hands
Observed the spouts/diffusers for the soda dispensing station and the interior of the ice machine / bin unit to be in need of more frequent cleaning
No paper towel was provided at the dish washing / back prep area hand washing station. Corrected by providing paper towels
Observed the ice machine drain lines to have slumped down over time--tighten the cable tie and/or trim off the drain lines to re-establish the required air gap (approx. 1. 5-2" in this case)
1) observed the interior portion of the ice machine holding bin door to be damaged and portions taped together. Replace this door to provide a smooth and easily surface. 2) observed some of the exhaust hood filter sections to be damaged and/or have missing vertical pieces. Provide replacement hood filter sections as needed
Observed worn/missing tile grout in the area of the ice making machine and ice cream chest freezer. Please remove remaining grout from these worn areas and apply a full thickness layer of new grout to eliminate the depressions that collect moisture and food debris
I found loose papers inside a storage box of raw potatoes. Food must be protected from contamination while in storage. Make sure that the staff is aware of this rule. Store food in a clean, dry location, that is at least six inches off the floor.
(Critical) I found a canister of home style insecticide. As discussed, these are never allowed. You may only use insecticides that are specifically labeled for restaurant use. Educate the entire staff about this rule. Corrected at the time of inspection. The product has been discarded.
Observed cutting board in server area with deep scores and cutting grooves. All cutting boards that are severely scored and can no longer be properly cleaned and sanitized must be resurfaced or replaced. Pic corrected by replacing cutting board at time of inspection.
Observed clean soup spoons all jumbled together in container next to soup hot holding unit in server area. To correct, please organize container so all spoon handles are pointing up and food contact portion of utensils are down to avoid possible hand contamination by employees
