(Critical) No date marking was provided for the following ready to eat, potentially hazardous foods: ribs, rib tips, gyros, hot dogs, portioned meats (chicken, steak, turkey, ham, brisket, lasagna, mostaccioli, ranch dressing, etc. Ensure staff are providing last day of use stickers with the date when the packaging is opened for commercially prepared rte, phfs or when in-house made rte, phfs are made

(Critical) Observed a container of raw shrimp on the top shelf of the reach-in portion of the salad/sandwich make line cooler. Corrected by relocating this shrimp container to the one door upright cooler on the bottom shelf

No lights were functional in the men's bathroom. Corrected by providing new light bulbs.

Observed a couple stacks of single service to-go containers stored upright on a shelf. Corrected by discarding the top containers and inverting the stack of containers

Observed a couple wet mop heads resting in the bottom of the mop sink. Please hang up the mops on the provided hanging device to facilitate faster air drying

Provide trash cans at your kitchen hand washing stations

Observed the left dumpster cover to be open. Please keep these covers closed to prevent pest entry

Observed a pitcher used for measuring oil to be stored in the back kitchen hand washing sink basin. Corrected by removing the pitcher

Health Report - 01/18/2012 (3 critical incident(s))

(Critical) The following items were observed to be unclean: nine metal hotel pans, the can opener blade, the electric food slicer carriage, a metal spatula and four knives. These items were shown to the manager and sent to the dish washing sink to be cleaned. In the future, ensure staff are thoroughly inspecting these items after they have air dried prior to placing them in storage

(Critical) Observed six individual serving containers of pudding in the refrigerator behind the front counter to be expired (dates of 1-12). Corrected by discarding

(Critical) Observed a drill with a spackle/dry-wall mud mixing attachment on it used to make the pizza sauce. This metal mixing tool is painted black and some of the black paint was observed to be missing. Also the way this mixer is designed and welded it is not easily cleanable. Stop using this tool as of today. Either utilize the hand whisk or purchase another mixing tool that is made for use with food (u. L. Or nsf certification).

Observed a chili dispensing ladle stored in a 1/6 pan of water (125f) inside an empty 1/3 pan in the steam table. Corrected by placing the hot water directly in the 1/3 pan in the steam table. After a while the water was observed to be at 157f.

The men's restroom is still non-functional. Repair or replace as needed

Observed a large pitcher (for oil?) placed in the back kitchen hand washing sink basin. Corrected by removing the pitcher from the sink and sending it to the dish washing sink

Observed a wet mop resting in the bottom of the mop sink in the back room. Hang up your wrung out wet mop heads above the mop sink to facilitate faster drying

Observed a few metal pans stacked on a upper shelf by the dish washing area to be still wet. Allow these pans to air dry prior to nesting them together

Observed two stacked cases of gator-aid stored under the waste water line at the front counter hand washing sink. Corrected by relocating to another storage shelf

(Critical) Found freshly sliced tomato at 45 degrees f. Improperly cooling in prep raised rail section of prep refrigerator. To correct tomato prepared from ambient tempertures should be pre-chilled to temperatures of 41 degrees f. Or less within 4 hours prior to being placed into insert pans of raised rail section of the preparation refrigerator. (pic) person in charge corrected this item during this routine inspection by properly cooling the sliced tomato to 41 degrees f

Observed the mens rest room with the exhaust fan not operating. To correct should provide exhaust fan for the mens restroom to vent the room as required

Observed the bulk food bin of sauce without a label to identify contents by common name. To correct provide a label to identify contents by common name as required

Found light bulb in the exhaust vent hood not lighting at end of hood near the soft drink refrigerator, found the soft drink refrigertor with the light not operating. To correct service lighting to operate properly as required

Observed prep area hand sink paper towel dispenser empty. Person in charge corrected this item by providing paper towels at hand sink to encourage proper employee hand washing

Found mop head placed in mop sink. To correct allow mop head to air dry by hanging mop up at available mop rack

(Critical) Employees personal food stored on cook line. Employees shall eat and drink in a designated area where contamination of exposed food, clean equipment, utensils, linens and unwrapped single-service items can not result. Critical corrected: at time of inspection employee removed half eaten banana and placed covered personal food items in a container in a designated area in one of the beverage reach-in-coolers.

(Critical) Potentially hazardous food items in top and bottom of 2 door reach-in-cooler across from pressure cookers not maintaining at proper temperature. Potentially hazardous food shall be maintained at 41°F or below or 135°F or above. Critical corrected: at time of inspection discarded all potentially hazardous food top and bottom of sandwich make unit maintaining at 52°F - 55°F. This unit is ordered out of service for the storage of potentially hazardous food until it is repaired, reinspected and capable of maintaining potentially hazardous food at 41°F or below at all times. Until then facility will use either of the beverage reach-in-coolers near the line for temporary storage of potentially hazardous sandwich items. Both units maintaining food below 41°F at time of inspection

Please store single-service utensils above sandwich prep ric so that all handles are facing employee to prevent potential contamination of food-contact end

Appears the digital thermometers in facility are not functioning properly. Please provide accurate food thermometer

Two door reach-in-cooler across from pressure cookers maintaining at improper temperature. Refrigeration shall be capable of maintaining food at 41°F or below at all times. Per pic unit was having some problems maintaining food at proper temperature, thought unit had been repaired. Please monitor all refrigeration units a minimum twice daily to assure the food is maintaining at 41°F or below

These areas in need of cleaning: floor/wall juncture in some areas of kitchen and ceiling fan cover women's restroom. Please clean now and as often as necessary to maintain in clean condition

Please replace floor tile near 3-comp sink drain to provide smooth, easily cleanable surface

Please invert all single-service containers on shelf across from front counter to prevent potential airborne contamination and to prevent contact with food-contact surface of container

Please store mop and brooms in an organized manner to facilitate cleaning the area used for storage. Provide rack

These areas in need of cleaning: underside of large mixer and mixer base, pizza oven filters, some hood filters above cook line, handle area barbecue sauce storage container, shelf under prep table rear kitchen and can opener stem and surrounding area. Please clean now and place on routine cleaning schedule. Per pic slicer was used this morning; please disassemble slicer for cleaning immediately after use and cover when slicer is clean

Please store staplers in a designated location away from food to prevent potential contamination from staples

Please replace missing ceiling fan cover in men's public restoom for proper function
