(Critical) Facility utilizes bleach as a sanitizer. During the inspection observed no bleach available for use as a sanitizer. Pic corrected during inspection by going to the store and purchasing approved bleach.

In the kitchen on the main cook line, observed wiping cloths stored in a bucket of liquid. The liquid contains bleach but not at an approved concentration. Pic corrected during inspection by obtaining an approved bleach and replenishing the wiping cloth storage fluid.

Observed many items that are old and no longer needed for the operation including: 1. Old equipment including a scale, deep fryer, cooking pans 2. Containers of spices 3. Cardboard recycle and/or discard items that are no longer needed/wanted to free up additional space and make it easier to clean the facility. This violation was observed during the previous inspection. Some progress has been made. Please continue discarding items

In the kitchen near the door to the walk in cooler observed a ceiling light fixture with unprotected fluorescent light bulbs. Correct by providing approved shielding

At the ventilation hood above the main cook line observed the filters with a heavy accumulation of grease. Correct by cleaning now and on a more frequent basis to prevent the accumulation of grease. This violation was observed during the last routine inspection. The filters must be cleaned more frequently

A wet mop head was observed to not be hung up to dry. Correct by hanging up your wet mops above the mop sink to facilitate faster drying and to allow dripping moisture to be routed down the drain.

Under the center prep line across from the wok table observed a heavy build up of grease and food debris on the floor. This buildup appears to be from several days. Correct by cleaning now and on a more frequent basis to prevent the accumulation of food debris

(Critical) Provide atmospheric vacuum breakers (a. S. S. E. 1011 devices) for the two hose connections in the mop sink along the left wall

(Critical) Observed the operator separate a large piece of cooked broccoli with his bare hands when packaging a meal. Corrected by discussing this issue with him and by not handling food with his hands after cooking

Observed many items that are old and no longer needed for the operation (equipment) and a lot of old containers of spices, etc. Recycle and/or discard items that are no longer needed/wanted to free up additional space and make it easier to clean the facility

Observed a kitchen light fixture to be missing the cover. Provide individual light bulb tubes with end caps or install a properly fitting light fixture cover

In several locations small bowls were observed to be used for scooping foods. Eliminate the use of these bowls and provide handled scoops/spoons and store them so the handle does not come in contact with the food

Several to-go food containers were stored upright on the top of the shelving unit at the cooking line area. Corrected by inverting these containers to prevent debris accumulation

1) the grey bulk container on wheels (used as a portable table) is over-due for a cleaning of the exterior surfaces. 2) the large bowl on the chinese range used to heat water has a lot of built-up residue on it. 3) the mop bucket and wringer are in need of cleaning. 4) the door handle for the mop closet and the underside surfaces of some of the prep refrigerator handles have a build-up on them that needs to be removed

1) observed many foods in the walk in cooler to be uncovered. Either provide container covers or plastic wrap as needed to protect these foods from contamination. 2) a lot of personal food is being stored in a one door upright refrigerator and in the walk in cooler. Employee foods must be stored below your customer's foods. Either relocate these foods to the bottom of one cooler or provide a separate cooler only for the storage of employee food

Two wet mop heads were observed to not be hung up (one was in the bottom of the mop sink and the other on the floor). Hang up your wet mops above the mop sink to facilitate faster drying and to allow dripping moisture to be routed down the drain

Observed evidence of a roof leak in the kitchen. The operator has a set-up of containers with drain outlets and funnels to catch any water dripping from the ceiling. A roofing contractor was contacted when this leak developed. The roofing contractor indicated they could not make needed repairs until warmer weather arrives. Repair asap

The following areas are over-due for cleaning: 1) the mop room floor, 2) the mop sink basin, 3) the floor under the shallow cabinet/counter next to the one door upright refrigerator, 4) the floor drain for the steam table. 5) some oil droplets were noted on some exhaust hood surfaces.

(Critical) A partial sack of flour in the corner of the dry goods storage room was infested with small moths. Corrected by discarding this flour.

Observed a couple wet wiping towels stored on the counters in the kitchen. Corrected by placing them in a sanitizer solution

1) the walk in cooler fan guards are in need of cleaning at this time to remove dust and mold accumulation. Clean these covers now and more frequently in the future. 2) the box fan near the cooking area is overdue for cleaning

Observed multiple food containers stored in the walk in cooler to be uncovered. Cover these foods to prevent accidental contamination

1) the floor under the shelving in the dry storage room is overdue for cleaning. 2) the floor sink under the one compartment sink is in need of cleaning. 3) after removing the kitchen floor drain covers clean and flush the drains to remove debris. 4) spilled rice and other food crumbs were observed at the front of the dining room along the windows and booths

(Critical) Observed a 3/8" copper water line routed to the front edge of the dish washing basin without an air gap. This water line is one of two drain lines coming from the water cooled walk in cooler compressor. Cap off this water line above the compressor at the 1/2" tee fitting. The other existing 1/2" water line does terminate with an air gap to a waste water drain located below the one compartment sink

Again observed wet towels on the kitchen counters. Ensure employees place their wet towels in a solution of chlorine sanitizer (100 ppm) between uses

The solenoid valve on the incoming water line for the water cooled compressor is apparently non-functional. Water runs continuously, even when the compressor is not running. Repair as needed

Again observed oil droplets located on the portion of the exhaust hood near the top of the hood filter sections. Take a towel and wipe this area down frequently to prevent any oil from dripping into foods or on cooking equipment

Observed cardboard being used as a shelf liner in the kitchen. Eliminate this cardboard as it is not easily cleanable and absorbs moisture

Observed water spots on many of the canned food containers that are located directly adjacent to the one compartment sink. Provide a barrier that is smooth and easily cleanable to place between the sink and the can rack to protect the canned goods. Clean off and sanitize these splashed cans prior to opening them

(Critical) Observed minced garlic covered in oil stored at room temperature out on the cook line cart. Corrected by discarding the garlic and by providing plain garlic in a clean pan.

Observed several wet wiping cloths stored on the counters while not in use. Corrected by providing sanitizer solution to store these towels in while not in use

Observed most of the large kitchen bowls and colanders stacked on the shelf located above the three compartment sink to be still wet from washing them previously. Please allow time for these items to air dry separately prior to stacking them together

Observed a wooden spoon (not durable over time) used for dispensing rice. Corrected by discarding the spoon and only using plastic spoons/scoops

Observed a wet mop head stored on the floor in the kitchen. Corrected by relocating the mop head to the mop room and hanging it up off of the floor

1) observed some oil drops just starting to form on the bottom edge of the cook line exhaust hood. Regularly wipe down this area to eliminate the oil as needed. 2) the walk in shelves and the fan covers for the cooling unit are in need of cleaning to remove mold growth and spilled food. Please clean now and more frequently in the future

(Critical) Observed a medium pan of rice stored above the insert pans on top of the food insert pans of the cook line prep top cooler. The rice was at 55 to 60f. Also, whole, in-shell eggs were observed in the top of this same unit stacked too high and without a pan cover. The eggs were at 50 to 55f. Corrected by discarding the rice and the top layers of eggs that were above 41f. In the future, provide a pan cover for the eggs or store them in the reach in portion of the cooler

(Critical) Observed home-made covers for some food tubs stored in the walk in cooler made of cardboard covered in aluminum foil and then covered in clear tape. These surfaces are not smooth and easily cleanable. Discard and provide commercial plastic food container covers that are smooth and easily cleanable. Also, observed two kitchen knives stored in a gap between the cook line refrigerator and the prep table, and another kitchen knife stored in a home-made knife sheath made of cardboard. The knives were removed from the gap and the cardboard. The owner will install an extra magnetic knife strip he has on the cooking line to store these knives

The following equipment needs cleaning now and more frequently in the future: the hood exhaust filter sections, the cook line equipment. Maintain this equipment clean to prevent the attraction of pests

Observed large quantities of food cooked this morning (breadded chicken, egg rolls, etc. ) stored stacked and covered in a one door upright kitchen cooler. Corrected by relocating these foods to the walk in cooler. Ensure these bulk hot foods are left uncovered on the top shelf in the wic and not layered too deep

(Critical) Observed two of the five foods in the steam table to be below 135f. The gravy was at 125f and the wonton broth was at 119f. Both corrected by reheating to above 165f prior to returning them to the steam table

(Critical) Observed a rice cooker full of brown rice to be at 50f. This rice was cooked the previous night. When the rice was done cooking the cooker/warmer was unplugged and the rice remained in the cooker/warmer overnight. Corrected by discarding the rice. In the future, ensure this rice is properly cooled by removing it from the cooker/warmer and spread thin on sheet trays to rapidly cool

(Critical) Observed a bag of raw ground pork stored in the one door upright cooler on an upper shelf above rte foods and touching an un-packaged head of lettuce. Corrected by relocating the package of ground pork to the bottom of the cooler and by discarding the lettuce

Observed an employee wearing a single use glove on one hand to prevent bare hand contact with ready to eat foods leave the glove on once they were done handling/dispensing food. Please ensure the glove is removed and discarded once the food handling/dispensing is finished

Observed multiple non-food contact surfaces in the kitchen to be unclean, such as: the rice warmer, the stainless steel top for the steam table, the exhaust hood canopy, the hood filter sections, the sides of the range top, the kitchen timer, the bottom shelf of the table to the left of the steam table, etc. Please clean these areas now and more frequently in the future

(Critical) Observed an brand of regular bleach not approved for sanitizing in a food service establishment being used. Bleach used for sanitizing in a restaurant must have proper usage directions on the label and have an epa registration number. Purchase an approved bleach for your food service sanitizing needs. The current bleach may be used for cleaning the bathroom and the floors if you wish

Observed a shallow metal pan of cleaned shrimp to be stored on the walk in cooler floor. Corrected by relocating the shrimp container to the bottom shelf

Observed many wet wiping cloths stored on several counters in the back kitchen. Also, a bucket for wiping cloths was observed to have too little chlorine concentration. Corrected by placing them in buckets of sanitizer mixed to 100 ppm. Please maintain your wet wiping cloths in the sanitizer solution when they are not in use to prevent bacterial growth

Observed the handle of the rice scoop to be resting in the small pan of rice that was stored in the one door upright cooler at the end of the cook line. Corrected by inserting the scoop in the rice so the handle is extended. Observed small plastic to-go food containers being used to scoop out flour and corn starch from the bulk storage containers. Please replace these containers with a handled scoop

Observed a large plastic tub of general's chicken cooked earlier today to be over filled, and a metal pot full of fried rice made earlier today in the walk in cooler. Corrected by placing the chicken into two separate containers and by stirring the rice to allow these foods to cool more quickly.

Observed no non-smoking sign at the entry to the restaurant. Please post the non-smoking sign provided at the inspection
