(Critical) On follow up inspection found the drain lines of the ice machine tied with a metal clamping strap and secured to the bottom of the ice machine providing adequate air gap from the drain lines and rim of the floor drain receptacle. Violation corrected.

(Critical) On follow up inspection used a quat test kit and found the quat sanitizing solution flowing from the quat dispenser in the 3 compartment sink at about 200 ppm, within manufacturer's recommended range of 200 to 400 ppm. Pic states quat dispenser has been adjusted. Violation corrected. On follow up inspection found used a chlorine test kit and found the bar glass washer chlorine sanitzing glasses. Pic states glass washer was repaired. Violation corrected. Sanitzing risk control plan has been reviewed and should work if followed.

(Critical) On follow up inspection found tcs food blue cheese 41°F, sliced beef 41°F, and beef patties 41°F in the 4 drawer under the grill cooler. Pic states cooler was repaired and now working. Violation corrected. On follow up inspection found tcs food portioned cups of facility made creamy siracha sauce 41°F in the bottom portion of the 2 door prep top cooler. Pic states they now store this sauce in cooler. Violation corrected.

(Critical) On follow up inspection management states staff have been trained to use test kits to verify sanitzing solution and record results in log sheet. Violation corrected.

Health Report - 03/25/2015 (4 critical incident(s))

(Critical) Found the drain lines of the ice machine laying in the floor drain receptacle. To correct provide an air gap(s) of at least 1 inch from the drain line and rim of the floor drain receptacle. This is a repeated violation and a risk control plan must be submitted.

(Critical) Found the quat sanitizing solution flowing from the quat dispenser in the 3 compartment sink , that is in sanitizing buckets and in the 3 compartment sink at about 150 ppm, below manufacturer's recommended range of 200 to 400 ppm. Pic states chemical company recently changed chemical dispenser. To correct have chemical company adjust quat dispenser to dispense quat sanitizer at manufacturer's recommended range of 200 to 400 ppm. Use a chlorine test kit and found the bar glass washer not chlorine sanitizing glasses. Pic has been instructed to call for repair and either use 3 compartment sink to wash, rinse, and sanitize glasses or use the kitchen dishwasher. This violation has repeated 3 of 5 routine inspections, an administrative conference notice has been issued and a risk control plan must be submitted.

(Critical) Found time/temperature control for safety (tcs) food, beef patties 49°F, shredded cheeses 55°F, shell eggs 56°F, and sliced beef 58°F in the 4 drawer under the grill cooler. Pic temporarily corrected by discarding tcs food above 41°F, and has been instructed not to use this cooler to hold tcs food until, repaired or replaced to hold tcs food at 41°F. Note: exterior thermometer indicated temperature of cooler at 55°F. Found tcs food, portioned cups of facility made creamy siracha sauce 65°F in an container of ice. Pic temporarily corrected by discarding creamy siracha sauce, and has been instructed to keep creamy siracha sauce at 41°F that is in an ice bath that completely surrounds sauce or in cooler at 41°F.

(Critical) Found the quat sanitizing solution from the 3 compartment sink and the bar glass washer not sanitizing dishes, and employees apparently not using quat or chlorine test kits. To correct retrain all employees to use appropriate test kits to verify proper sanitizing solutions.

Found the floor under equipment, edges and corners in the potato preparation area soiled with accumulation and build up. To correct thoroughly clean floor and clean on a more frequent schedule.

Found the 4 drawer under the grill cooler not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F or lower. Found open spaces/gaps along hood filters in the cooking hood. To correct either install properly sized hood filters or use spacers to fill gaps.

Found no handwashing sign in the bar handsink area. To correct provide sign reminding employees to wash hands.

Found the interior surfaces of all coolers soiled with accumulation of debris. Found the bottom and interior surfaces of turbo chef ovens soiled with spills and splash. To correct clean coolers and ovens and clean on a more frequent schedule.

Found no thermometer in the right 2 door prep top cooler. To correct provide internal hanging thermometer in cooler.

Found the left fryer not completely under the cooking hood. Pic states after cleaning staff not pushing equipment back under hood. To correct after cleaning properly place all cooking equipment under hood.

(Critical) Used a quat test kit and found the sanitizing solution flowing from the quat dispenser in the kitchen 3 compartment sink and mop sink below manufacturer's recommended range of 200 to 400 ppm. To correct call for repair, and you may manually mix sanitizing solution and use quat test to verify proper concentration. This violation has repeated 3 out of 4 routine inspections, and an administrative conference notice was not issued, since previous violations were of a different situation. Management has been reminded that continuous violations need to stay corrected as future violations will result in enforcement.

(Critical) Found the drain lines of the ice machine draining into a floor drain without air gap. To correct provide an air gap of at least 1 inch from the drain lines and the rim of the floor drain receptacle.

Found some of the beer tap spouts on the bar counter top soiled with light accumulation. To correct follow manufacturer's directions and clean beer tap spouts.

Found the interior panel on the lid/door of the 2 door prep top cooler loose with exposed insulation. To correct repair panel. Found 2 gaps along the hood filters in the cooking hood. To correct provide missing spacers to eliminate gap. Found a gap along the walk in cooler door and the floor. To correct replace torn gasket on the bottom of walk in cooler door.

Found no handwashing sign in the bar handsink area. To correct provide sign reminding employees to wash hands.

Found several boxes from facility on the floor besides the plaza's shared dumpster. To correct work with neighbors keep dumpster areas clean and free of debris.

Found several spray nozzles in the dishwashing machine clogged with debris. To correct follow manufacturer's directions and clean spray nozzles.

Found the floor under equipment, edges and corners in the prep area soiled with accumulation of debris. To correct thoroughly clean floor and keep clean.

(Critical) At the handsink adjacent to the shake making area observed no hand soap in the dispenser. The pic corrected during the inspection by installing a new bag of handsoap into the dispenser. Item corrected

(Critical) In the kitchen, at the in use mechanical dishwashing machine observed the residual chlorine concentration of the final rinse at 0 ppm. The sanitizer reservoir was found to be almost empty. The pic corrected during the inspection by installing a new sanitizer reservoir. Item corrected

(Critical) Observed the handsink adjacent to the grill station blocked by a trash can. The trash can is being used as a work table for a new order display screen. Pic corrected by relocating the trash can so the sink is accessible. Item corrected

At the shake making station observed several clean shake making cups stacked wet for storage. Pic corrected during inspection by rewashing the cup and setting them out for drying

(Critical) At the handsink in the bar area observed no hand towels at the sink. The pic indicates the dispenser is not easily serviceable. On the floor of the walk in cooler underneath and behind the keg beer bunk rack observed a heavy accumulation of viscous beer liquid. Correct by cleaning now and on a more frequent basis to prevent the accumulation of this residue. The pic indicates the cooler floor is cleaned at least once a week and is due to be cleaned

(Critical) In the kitchen at the dishwashing area three compartment sink, observed the chemical dispenser not dispensing sanitizer. Correct by repairing dispenser to dispense the proper concentration of sanitizer.

(Critical) Observed no thermometer with a thin metal tip in the facility. Facility cooks thin mass hamburgers from raw. Pic indicates thermometer became inoperable recently and has not been replaced. Correct by replacing thermometer immediately.

(Critical) Observed employee dispense sanitizer solution into red sani buckets. Employee did not use test strips to measure sanitizer concentration. Using baileys test strips observed the sanitizer solution concentration to be 0 ppm quaternary ammonia. It appears the solution dispenser is not dispensing sanitizer. Pic directed the employee to use another sanitizer solution dispenser. Educated staff about the importance of using test strips to ensure proper sanitizer concentration. Item corrected.

At the dipper well by the ice cream freezer observed the ice cream scoop stored in the dipper well. The drain stand pipe of the dipper well is dislodged. The velocity of the water is not sufficient to flush particles of ice cream from the ice cream scoop. Pic corrected by properly installing the standpipe to allow water to flush the ice cream scoop. The scoop was sent to the dishwashing machine for cleaning. Item corrected

(Critical) On the top rail of the reach in refrigerator/ice cream condiment cart by the ice cream freezer on the front make line observed a partially full gallon of milk at 48 degrees f. A manufacturers label on the top rail reads, "rail acceptable for non hazardous food products only. " the top rail appears to be designed for non hazardous ice cream topping storage and dispensing. Milk and other potentially hazardous foods shall not be stored on this rail. Pic corrected by discarding the partial gallon of milk. Item corrected

Observed sealed tubes of frozen ground turkey thawing in prep sink. Cool running water was cascading over the tubes. However, the tubes were not submerged as required. Correct by thawing foods as described above

(Critical) Observed both ice buckets with multiple holes broken through bottom of container. Ice buckets observed sitting on floor during filling. This is a potential source of contamination. Correct by discarding broken ice buckets and replacing with proper undamaged buckets. ,

Observed mulitple clean ready for use pans stored wet at the dish washing area. Wet pan storage can contribute to mold growth. Correct by completely air drying pans before stacking for storage

(Critical) Observed blade of can opener at kitchen table and two 1/3 pans located at clean pan shelf in kitchen to correct pic had employees rewash, rinse and sanitize utensils, pans and blade of can opener

Observed utensils stored in a drawer at kitchen table with can opener without being clean. The drawer will need to be cleaned out more frequently to provide a storage location for clean utensils

Observed an ice cream scoop stored in hand sink at shake area of kitchen. Pic corrected this by removing scoop and instructing employees to properly store utensils in running water dipper well as required
