Observed the following wall areas with an accumulation of grease and grime: kitchen 1. Behind the main cook line basement 2. Behind the soda box rack 3. Behind the single service storage area correct by cleaning now and on a more frequent basis to prevent the accumulation of grime
Observed the facility has no employee on staff who is currently certified under a personnel certification program accredited by ansi, utilizing the conference on food protection standards. Correct by providing proof of certification for a member of staff who is regularly engaged in food preparation on a full time basis. Item shall be corrected within 90 days.
Observed a large cutting board next to the main cook line prep area. This cutting board had deep grooves. Correct by replacing or resurfacing the board so it is smooth and easily cleanable
Observed three small containers suspended from the ceiling over the main prep area. The pic indicates these are to capture water leaking from the roof. No water was observed leaking from the roof today. It does not appear water from the roof is able to contaminate food or food contact surfaces. The pic indicates the roof needs repair and repairs are in progress
On main cook line observed sides of stove and fryer below the cooking surface with an accumulation of grease. Also observed the shelf below the stove top with a heavy accumulation of grease and charred material. Correct by cleaning now and on a more frequent basis to prevent the accumulation of grease.
At the receiving drain for the walk in cooler condensate line and culinary sink observed no air gap. It appears the drain line for the wic and culinary sink have slipped down into the receiving drain. Correct by providing a proper air gap for each item
In basement observed a few out of service and unused pieces of equipment. Correct by continuing to remove unnecessary items from the facility. This item is improved from the last inspection
In basement, observed light bulb without shielding located over single service item storage area. Correct by installing properly shielded bulb. Note: other bulbs in the basement have been replaced with shielded bulbs since the last inspection

Observed no shielding on the lights bulbs in the basement storage area. Light bulbs are located over open boxes of single service items. Correct by installing properly shielded light bulbs. This is a repeat violation from the last inspection

In the kitchen on the floor under equipment on the main cook line observed a few areas with a heavy accumulation of grease. Correct by cleaning now and on a more frequent basis to prevent the accumulation of grease. This is a repeat from the last inspection. However, in general the floor is much cleaner on this inspection compared to last time.
Observed outside entry screen door to kitchen not tight fitting in the opening, with no self closer, and with holes in the screen. Correct by repairing the door to prevent the entry of pests

The owners couldn't find chlorine test strips. Obtain a supply of chlorine test strips for checking sanitizing solutions at the three compartment sink and in sanitizing buckets
The owners couldn't find a food thermometer, when asked. Provide 2-3 new food thermometers, for checking food thermometers.
Observed steak knives being stored in container that had an accumulation of dust and debris on shelf in server area (near swinging doors). All clean utensils and silverware must be stored in a clean, dry location and protected from sources of contamination. Pic corrected by washing and sanitizing all knives and utensil container at time of inspection
Observed interior of chest freezer lid in basement held together with duct tape. To correct, please repair or replace lid so all surfaces are smooth and cleanable
(Critical) Observed slicer on shelf in dry storage area with an accumulation of debris. Slicer was not cleaned after last use. All food contact surfaces of equipment must be washed, rinsed and sanitized after every use and must be clean to sight and touch. Pic corrected by disassembling slicer and washing, rinsing and sanitizing all slicer components at time of inspection. Critical corrected.
Observed plastic trash can in cooking area across from 3 compt sink soiled with greasy debris. To correct, please replace trash can as soon as possible. , observed wall across from chest freezers in basement with an accumulation of mold and debris (owner stated that wall leaked after heavy rain last week). To correct, please ensure that leak has been sealed then wash and repaint wall as soon as possible. , observed floors and sides of equipment in cooking area with a heavy accumulation of grease and debris. To correct, clean now and on a more frequent schedule.
Observed kitchen area significantly smoky from cooking on grill. Ventilation hood is on and appears to be working, but ventilation system does not seem to have enough capacity. To correct, please have ventilation system cleaned and inspected by licensed contractor as soon as possible
