Found shields still missing from light fixture above mop sink. To correct - replace shield. It is noted that it is an odd shield size and attempts have been made to cut down a larger shield without success. However, a shield must be replaced to provide protection from possible glass breakage
Consumers advisory must be added to your menu on your website. A copy of the table tent version of the consumers advisory was submitted at the time of this inspection
Hand sinks and bath sink are still not equipped with paper towel holders. To correct - please install paper towel holders at hand sinks and bath
Found cut flowers/ arrangements stored in close proximity to rte vegetables and other food items. To correct - provide separation of flowers and food. A possible solution would be to store flowers on the bottom shelf in the cooler with food above so as not to contaminate rte food.
Found some areas of needed cleaning: floors under equipment, floor sink at 3 compartment sink - and strainer at this floor sink. To correct - clean now and keep on a more frequent cleaning schedule
Found stock pot of soup cooling with a melted ice wand @ 100* f. Soup had been cooling for 1 hour, 40 minutes. Corrected - ice was added to soup, 20 minutes later @ 80* f. Soup was removed from wic and brought back up to temp @ 165* f, then put into shallow pans and put back into wic. Two stage cooling process was discussed with pic
Found several equipment surfaces to be in need of cleaning: steel walls behind the oven, oven doors, crumb tray under burners, sides of oven, mixer stand, shelves under work table, the wic door handle and surrounding surface. To correct - clean now and on a more frequent cleaning schedule
(Critical) Found a plastic tub of clean knives stored under the prep table that had an accumulation of food debris and crumbs in it. Also found the immersion blender wands stored under the mixer stand without protection on a shelf that also had food debris and crumbs. Corrected - pic corrected by sending all the knives and the plastic storage tub to the dishwasher for thurough cleaning. Immersion wands are not used regularly and always washed first. Recomment storing immersion wands in plastic bags to protect them from dust and debris
Found serveral areas that need more frequent cleaning: floors under equipment, mop sink area, walk-in cooler shelves and walls, freezer bottom. Some cieling tiles are also missing. Also found areas on walls where there are holes left from removal of shelves, etc. That need to be chalked or filled: over slicer, and along the staircase to the basement. To correct - clean floors, walls and equipment now and keep on a more frequent cleaning schedule. Replace ceiling tiles, and fill holes where noted above.
Found all hand sinks and bath sink to have roll of paer towel resting on a surface. To correct - mount a paper towel holder at each sink to allow continuous dispensing of towels withot picking up the rolls and placing them on a surface
Found overturned milk crates being used to raise detergent pails in mop area. To correct - remove crates as storage platforms. Milk crates are not easile clenable surfaces and are harder to and less likely to be moved for floor cleaning
Found lighting that was not shielded over mop sink area and outside walk-in cooler. To correct - replace light shields in these areas
Found several light bulbs and flourecent tubes burnt out. To correct - replace all burnt out bulbs and tubes.
Menus must carry the required language for comsumer advisory. To correct - provide reuired comsumer advisory on cover page of menu packets. Correct langusge given to pic at the time of inspection. (Critical) 1) observed the following spoiled/moldy foods in the walk-in cooler: curdled milk, sour cream, shredded cheese. 2) observed a jar of "home" canned salsa in the walk in cooler. The above items were corrected by discarding. Ensure expired foods are discarded on a daily basis during a cooler walk-through.
(Critical) Observed multiple rte, phfs (hard boiled eggs, open milk, feta cheese, red pepper cream cheese, roast beef, etc. ) in the walk in cooler without last day of use information. These foods were discarded. Begin providing last day of use stickers on required foods. The day of opening/prep is day number one, so add six days and provide the month and day of month on a sticker attached to the container
1) observed the kitchen window cracked open so an extension cord could be run out to the smoker. 2) the back hall way door was propped open to the exterior. 3) observed a gap at the lower level exterior door (at the bottom of the stairwell). To prevent pest entry keep your exterior entry points sealed/closed
Observed plastic and paper plates, bowls, etc. Stored upright while in storage that have collected dust & debris. Corrected by discarding the top bowl/plate and inverting the remaining items
Again observed the mop sink basin receiving waste water from the three compartment sink and the dish washing machine to have old food debris on the side walls. This basin needs regular cleaning (at least 2x per week) to eliminate a breeding area for flies. Also, the area behind the dish washing machine (a difficult to access location) is due for cleaning
Observed dry crumbs in the bottom of the three drawer storage container for utensils. Clean these drawers now and on a more frequent basis.
Mop sink faucet: the built-in back flow preventer on the mop sink faucet is not compatible with down-stream shut off valves. Someone has installed a flexible hose supplying water to a chemical feeder containing a shut-off valve. Install an eco-lab "side-kick" device, or equivalent, on the hose connection of the mop sink faucet, prior to the flexible hose to the chemical dispenser
(Critical) Observed an opened, partially consumed can of dijon mustard in the one door tall-boy cooler that was dented on the top and bottom at the side seam. Corrected by discarding. In the future, do not utilize foods in severely dented or bulging cans
(Critical) None of the required foods were date marked in the walk in cooler, such as: cream cheese, fresh mozzarella, deli turkey and roast beef, brie cheese, 1/2 & 1/2, etc. Ensure your rte, phfs are date marked at the time of opening (commercially prepared foods) or when prepared (in-house made foods)
Numerous insulated, bulk beverage dispensers were observed stored in the facility. The spouts on the dispensers that were looked at were due for cleaning and sanitizing. Disassemble the spouts on all of these beverage dispensers for a thorough cleaning at this time
The floor sink serving the three compartment sink has excessive food debris accumulation in it. Begin cleaning this basin at least 2x per week to maintain it clean. Doing so will eliminate odors and fly breeding areas
1) observed a "paint brush" (unused) style brush in the kitchen on the spice rack. Do not use this kind of brush--instead purchase a commercial pastry brush, bbq or sauce brush with food grade bristles that are imbedded in food grade plastic. 2) observed three spatulas that are worn. Discard and provide replacement ones
Condensate is leaking from a joint in the pvc exhaust pipe for the furnace onto the floor. Repair this joint to be water tight
Observed staff storing two ice scoops on the unclean top of the ice machine. A stainless steel ice scoop holder was observed resting on a shelf above the ice machine. After washing the scoop and the holder, attach the scoop holder to the wall on the screws provided to store the scoop when not in use
Observed several large holes/gaps in the facility walls (one at the kitchen window sill, another at the base of the kitchen wall at the deep fryer). Inspect the facility for additional pest entry points and seal them with appropriate materials that exclude pests and that are smooth and easily cleanable

Light bulbs were burned out in the following areas: the furnace/mop closet and two of three bulbs in the exhaust hood for the cooking equipment. Provide replacement bulbs
There are several pieces of equipment in the facility that are domestic and not nsf approved. This includes the panasonic microwave on the bar counter (personal use only); the viking mixer and the viking blender in the kitchen. When in need of repair, replace with commercially approved equipment only.
