Found the ceiling lights in the back kitchen prep area not working properly. To correct repair/replace lights.

Found 3 sealed, unopened cases of frozen ducks thawing in a compartment of the 3 compartment with flowing water. Pic corrected by having employee open cases of frozen ducks and place frozen ducks in a sink of cool running water. Pic has been instructed to monitor and ensure employees are properly thawing foods.

Found the floor (under equipment, edges and corners) in the wait stations soiled with accumulation of debris. Found the floor on the cooking line soiled with heavy accumulation of grease and debris. To correct thoroughly clean floors and keep clean.

Found the exterior surfaces of the dishwashing racks soiled with build up. Found the cooking hood and hood filters soiled with heavy build up of grease. To correct properly clean dishwashing racks, cooking hood and hood filters and clean on a schedule to preclude accumulation

Found 2 dead roaches on the floor of the mop closet. To correct clean mop closet floor and clean on a schedule to remove dead roaches as soon as possible.

Found the tiled wall under the dishwashing machine counter and on the cooking line with missing tile and crumbling tile. To correct properly repair wall to be durable, smooth, and cleanable.

(Critical) Found time/temperature control for safety (tcs) food, steamed pork buns 51°F in the kitchen 2 door reach in cooler. Pic corrected by discarding steam pork buns, and has been instructed not to use this cooler to hold tcs food, until repaired/replaced to hold tcs food at 41°F. Found tcs food, rice noodles 49°F, cooked noodles 55°F, pooled eggs 60°F, chicken 55°F in the 2 door reach in cooler by the kitchen door. Pic corrected by discarding tcs food, and has been instructed not to use this cooler until repaired/replaced to hold tcs food at 41°F. ,

(Critical) Observed live roaches in the wall by the dishwashing area, and on the floor of the prep area. Pic provided statements indicating pest control treatments. To correct keep facility clean, in good repair, and evaluate plan with pest control company to effectively control roaches. ,

Found light in the cooking hood not working. To correct repair/replace light in cooking hood.

Found slight accumulation in the pop nozzles in the wait station. To correct follow manufacturers directions and clean pop nozzles. Found some tea leaves on the sides and edges of the tea pot lid of tea pots on the shelf under the tea urn. To correct rinse out all tea leaves from tea pot and properly wash tea pots.

Found the floor under equipment on the cooking line and storage area soiled. To correct thoroughly clean floors and keep clean

Found the edges and sides of the cooking hood soiled with grease. Found the bottom shelf where used oil is stored soiled with build up. To correct clean hood edges and shelf on a schedule to preclude accumulation.

Found lid of dumpster in open position. To correct close dumpster lids and keep closed.

Found the 2 door reach in cooler in the kitchen and by the kitchen door not holding tcs food at 41°F. To correct repair/replace coolers to hold tcs food at 41°F or lower.

Found single use utensils, spoons and fork stored in a jumbled, mixed manner. To correct store utensils either with handles up or handles in one direction.

Found the cap of the vacuum breaker protecting the water supply of the water inlet of the garbage disposal missing. To correct repair/replace cap of vacuum breaker, and ensure vacuum breaker is functioning properly.

(Critical) Found the water discharge drain line of the water cooled 2 door and drawer prep top cooler draining directly into the drain line of the handsink drain line, with no air gap. To correct re-plumb drain line and provide and air gap of at least 1 inch from the discharge drain line and the rim of the floor drain.

(Critical) Found only the shellstock tag for current oyster supply and previous shellstock tags unavailable. Found no shellstock tag for current mussel supply and pic states employee may have thrown tags out. To correct either find/provide shellstock tags for molluscan shellfish and retain shellstock tags for 90 calendar days from the dates of harvest.

(Critical) Found time/temperature control for safety (tcs) food, 2 plates of baked pork buns 80°F and plate of egg custard 80°F in the cheese melter/hot holding unit. Pic corrected by discarding tcs food, and has been instructed to monitor and ensure tcs food is held at 135°F or above.

Found the upper hood edges, lower sides and edges of the cooking hood soiled with grease and heavy build up. To correct thoroughly clean hood and clean on a schedule to preclude build up.

Found the floor under the fryer, wok line, and steam table soiled with heavy accumulation and build up. Found the floor under coolers and waitstation area soiled with accumulation. Found the mats, carpeting, and tiled floor in the kitchen door soiled with greasy build up. To correct throughly clean floors and noted areas and clean on a schedule to preclude accumulation.

Found 2 covered containers of chicken feet cooling on top of each other in the walk in cooler. Pic corrected by removing covering and separated containers for rapid cooling.

Found the pop nozzles of the pop machine in the dining room waitstation soiled with accumulation. To correct follow manufacturer directions and clean pop nozzles as soon as possible, and clean as directed.

Found the door gaskets of the 2 door and drawer prep top cooler cracked. To correct repair/replace door gaskets.

Found the floor (under equipment, edges and corners) in the kitchen and wait station soiled with accumulation and build up. Found the tiled wall under and around the back area prep table soiled. To correct thoroughly clean floors and walls, and clean on a schedule to preclude accumulation.

Found the hood edges and gutter of the stir fry cooking hood soiled with heavy grease build up. Found the walls and surrounding areas of wok line around burners soiled with greasy build up and burn on. To correct thoroughly clean hood and wok line areas and clean on a more frequent schedule to preclude accumulation.

Found stacks of steam buns in small stainless steel steamer pans cooling stacked and covered in the 2 door reach in cooler. Pic corrected by placing steam buns in stainless steel steamer pans on baking sheets (not covered and not stacked) in the walk in cooler to rapidly cool. Pic has been reminded that time/temperature for safety (tcs) food, must be cooled from 135°F to 70°F in 2 hours and then from 70°F to 41°F in 4 hours.

Found dead roaches under and around the ice machine in the waitstation area and in the back kitchen prep area. Pic produced statements indicating twice a month treatments. To correct keep areas clean and dry, in good repair, and continue pest control treatments.

(Critical) Found ready to eat (rte), time/temperature control for safety (tcs) food, various desserts- mango pudding, tarot cake, turnip cake, and almond pudding without 7 day (day made + 6 days) discard datemark. Pic corrected by verifying date made, and provided 7 day datemark for all noted foods. Pic has been instructed to monitor and verify that employees are properly datemarking items.

Found the front shield of the ice machine with taped surfaces that are loose and no longer smooth. To correct repair/replace front shield of ice machine to be durable, smooth, and easy to clean.

Found the faucet in the handsink in the dishwashing area loose. To correct secure faucet to handsink.

Found the edges of the rice warmer in the dining room wait station table soiled. Found the fan and surrounding areas on top of the 3 door reach in cooler soiled with dusty build up. Found the right side of the hood and filters soiled with greasy build up. Found the interior walls and shelves of the walk in cooler soiled with accumulation. To correct clean noted items, and clean on a more frequent schedule to preclude accumulation.
