(Critical) Observed the following: 1. Asse 1056 removed from the mop sink chemical tower set up. 2. Observed a hose with a sprayer attached to the faucet equipped with an atmospheric vacuum breaker. A plumbing system shall be installed to preclude back flow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the food establishment, including on a hose bib if a hose is attached or on a hose bib if a hose is not attached and back flow prevention is required by law, by: (a) providing an air gap as specified under # 5-202. 13 (p); or (b) installing an approved back flow prevention device as specified under # 5-202. 14. (p) to correct reinstall the asse 1056 and provide a vented t to the faucet equipped with an atmospheric vacuum breaker. ,

(Critical) Observed an inconsistent date marking system in use throughout the facility. Potentially hazardous food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded when held at a temperature of 41�f or less for a maximum of 7 days. (pf) to correct put a consistent date marking system into place for use in the facility. ,

(Critical) Observed multiple blanched veggies holding 50-63f in the two top loading coolers due to being stored over the fill line on the pans. Note: food was stored in the cooler for less than 1 hour. Potentially hazardous food that is to be cold shall be held at 41f or below. To correct move the noted items to the bottom cooler to rapidly cool them to 41f or below and discontinue storing the food over the fill line. Corrected: person in charge moved the noted food to the main cooler to rapidly cool it to 41f and replaced the items with product that was below the fill line

(Critical) Observed the following food contact surfaces soiled with a food debris build up: 1. Underside of shelves over the cook line coolers and steam table. 2. All parts of the slicer. 3. Mandolin slicer. 4. Can opener blade. Equipment food-contact surfaces and utensils shall be cleaned: (1) except as specified in # (b) of this section, before each use with a different type of raw animal food such as beef, fish, lamb, pork, or poultry; (p) (2) each time there is a change from working with raw foods to working with ready-to-eat foods; (p) (3) between uses with raw fruits and vegetables and with potentially hazardous food (time/temperature control for safety food); (p) (4) before using or storing a food temperature measuring device; (p) and (5) at any time during the operation when contamination may have occurred. Observed the following: 1. Ceiling in the prep area by the cook line and the coolers to have holes in the ceiling and or peeling paint. 2. Floors throughout are in very poor repair. Physical facilities shall be maintained in good repair. To correct: 1. Repair the ceiling and peeling paint. 2. Repair or replace the flooring. ,

Observed the following equipment in poor repair: 1. Chef drawer under the cook top has a sagging bottom drawer. 2. Chef drawer under the cook top has a torn gasket. 3. Three door prep cooler has two torn gaskets near the cook line. 4. Downstairs walk-in freezer is leaking water and causing a major ice build up in the cooler. 5. Downstairs walk-in freezer no longer has a gasket. 6. Left side top loading cooler on the cook line is leaking water into the bottom cooler and then leaking water onto the floor. Equipment shall be maintained in a state of repair and good condition. To correct repair all the noted coolers and replace all the noted gaskets. ,

(Critical) Observed the following at the mop sink: 1. Shut off valve down stream of atmospheric vacuum breaker (avb) at mop sink, directly next to the avb. 2. Hose with sprayer attached without a hose bib vacuum breaker. A plumbing system shall be installed to preclude back flow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the food establishment, including on a hose bib if a hose is attached or on a hose bib if a hose is not attached and back flow prevention is required by law, by: (a) providing an air gap as specified under # 5-202. 13 (p); or (b) installing an approved back flow prevention device as specified under # 5-202. 14. (p) to correct remove the shut off valve from the avb and add a hose bib vacuum breaker. ,

Observed the following: 1. Gaskets on the middle cooler on the cook line in poor repair, doors are not properly shutting. 2. All three gaskets on the three door upright cooler in the prep area. 3. Gasket on the downstairs freezer. 4. Gaskets on the top loading cooler by the hand sink, near the dish line. Note: door is not properly shutting. 5. Cooler on the far left of the cook line needs the door liners repaired as they are loose and not easily cleanable. Equipment shall be maintained in a state of repair and good condition. To correct repair or replace all the noted items.

Observed the ceiling over the cook line to have a dust build up. Physical facilities shall be cleaned as often as necessary to keep them clean. To correct clean the noted ceiling. ,

Found the door handle missing on the top door of the 2 door salad and dessert prep top cooler. To correct repair/replace missing door handle. Found water pooling on the bottom of the 2 door prep top cooler on the cooking line and the under the counter cooler in the bar. To correct clean coolers and repair coolers to drain properly. Found the door gasket on the basement walk in freezer damaged and torn. To correct repair/replace door gasket.

Found some of the door gaskets of coolers on the cooking line soiled. To correct clean door gaskets and clean on a more frequent schedule.

Found lid of community cardboard dumpster missing. To correct repair/replace missing lid.

Found the light shield above the cooking line missing. To correct either repair/replace light shield or provide plastic tubes and end caps.

Found the floor coating in the entire kitchen floor worn, uneven, and chipped. Pic states kitchen is scheduled for remodeling and will be replaced in the near future. Found the floor in the bar with cracked and chipped tiles. To correct repair and finish floor to be durable, smooth, and cleanable. Found the ceiling on the back, side kitchen area with possible water damage and peeling paint. To correct repair source of leak, repair area, and finish area to be smooth, durable, and cleanable.

Found the floor on the cooking line, under equipment, edges and corners soiled with accumulation of debris. To correct thoroughly clean floors and clean on a more frequent schedule.

Found the drawer and door prep top cooler on the cooking line not holding tcs food at 41°F. To correct repair cooler to hold tcs food at 41°F. Found the handles of the covers of food pans in the hot holding unit missing. To correct either resecure handles to lids or replace lids. Found the door gaskets of the walk in cooler and walk in freezers worn, torn and damaged. To correct repair/replace door gaskets.

Found bags of wiping cloths stored on the floor of the basement liquor room. To correct store wiping cloths on shelf at least 6 inches off the floor.

Found no handwashing sign in the bar handsink area. To correct provide a sign reminding employees to wash hands.

Found the floor in the entire kitchen with peeling coating, various damaged spots and uneven areas. Found the ceiling in the basement with water stains and missing ceiling tile. To correct repair/replace floor and ceiling.

Found a pitcher with a handle being used as a ice scoop stored on top of the ice machine. To correct store ice scoop in a more protected manner such as in a container or holder.

Found several spray nozzles of the dishwashing machine clogged with debris. To correct clean spray nozzles and keep clean.

Found the entire kitchen floor in poor condition, numerous worn sports and uneven surfaces. To correct repair and re-surface floor to be durable, smooth, and cleanable. Found the floor in the back hall way with chipped and missing tile and found the grout surrounding the floor drain of the bar ice bin worn and holding water. To correct properly repair floor. Found the ceiling in the back side kitchen with peeling paint. To correct verify that source of leak has been repaired, and repair and finish damaged area.

Found the lights in the back kitchen ceiling with plastic tubes but no endcaps. To correct replace missing endcaps on light plastic tubes.

Found the door gaskets of the coolers on the cooking line cracked. To correct repair/replace door gaskets. Found the chemical sanitizer dispenser not dispensing at proper concentration and facility manually mixing sanitizer to manufacturers specification of 150 to 400 ppm. To correct repair chemical dispenser.

Found the lights in the kitchen above the plating area and salad and dessert area not working. To correct repair/replace lights.

Found the cutting board on the steam table with deep grooves. To correct resurface cutting board to be smooth.

Found the floor in numerous areas of facility with chips, cracks, damaged and uneven surfaces. Found the walls in various areas of facility with peeling, missing tile, with peeling surfaces in hallway. To correct repair/replace floors and walls to be smooth, durable, and cleanable.

Found the lights in the back kitchen dish and prep area with plastic tubes, but no endcaps. To correct provide endcaps.

Found in use utensils stored in sanitizing buckets. To correct discontinue this practice of storing in use utensils in sanitizing buckets, and for example store on clean portion of prep table.

Found the light in the basement hallway leading to storage room and walk in freezer not working. To correct repair/replace light.

(Critical) Found time/temperature control for safety (tcs) food, prosciutto 54°F, ricotta cheese 53°F, shredded cheese 56°F, in the bottom portion of the 2 door salad cooler. Pic corrected by discarding tcs food in the bottom portion of the 2 door salad cooler, and has been instructed not to use the bottom portion of this cooler until repaired/replaced to hold tcs food at 41°F. This is a repeated cold hold violation (same cooler) and a risk control plan form has been issued. ,

Found slight accumulation on the interior front shield of the ice machine in the basement. Found slight accumulation on the interior of the beer spouts in the bar. To correct follow manufacturers directions and clean interior front shield of the ice machine and beer spouts, and clean on a schedule to preclude accumulation.

Found water pooling under and around the drain line of the walk in freezer condensation drain line. Found the ice bin drain line draining onto the floor and not into the floor drain. To correct repair drain lines to properly drain.

Found the floor on the basement storage room floor soiled with accumulation. Found the floor (under equipment, edges and corners) in the kitchen soiled with accumulation. To correct thoroughly clean floors and clean on a more frequent schedule.

Found cheese on top of a napkin without cover or protection in the outside waitstation cart, side walk dining area. To correct store cheese in a more protected manner such as in a covered container.

Found the floor under equipment on the cooking area with raised and uneven surfaces. Found the tile floor in the back hallway chipped and damaged. To correct repair/replace floor to be durable, smooth, and easy to clean.

Found the bottom portion of the 2 door salad cooler not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F. Found the sink stopper of the sanitizing compartment of the 3 compartment sink not working properly. Found the interior door panel of the produce walk in cooler missing plate cover. Found the cutting board holder tied together with cotton string. Observed water pooling on the bottom of the 2 door prep top cooler on the cooking line. Found the left door of the 2 door salad prep top cooler misaligned. Found the door handles of the bar cooler loose. To correct repair/replace all noted areas and keep equipment and utensils in good repair.

(Critical) Found time/temperature control for safety (tcs) food, a container of prosciutto 62°F on top of containers on the top portion of the 2 door salad prep top cooler. Pic corrected by discarding prosciutto, and has been instructed to monitor that employees are placing items back in cooler in designated storage/holding area, and tcs food is held at 41°F or lower. Note: this is not a repeated cold hold violation.

Found several coolers in the kitchen without thermometers. To correct provide internal hanging thermometers in coolers.

Found cheese in the outside patio dining area waitstation uncovered in a basket. Pic corrected by wrapping cheese. Found food on the floor of the walk in freezer. To correct store foods on shelf off the floor.

Found a large stock pot of soup left on counter top to cool. Pic corrected by checking log sheet and found there was still time to cool soup from 135°F to 70°F in 2 hours, and immediately placed soup in an ice bath for rapid cooling. Later verified that soup had cooled to 70°F within 2 hours. Violation corrected. Pic has been instructed to monitor and ensure tcs food is cooled from 135°F to 70°F in 2 hours and then from 70°F to 41°F in 4 hours.

Found the floor of the kitchen with peeling coating, holes and uneven spots. To correct repair and re-surface floor to be durable, smooth, and easy to clean. Found the ceiling access panel in the main kitchen missing. To correct replace missing access panel. Found a large hole in the basement ceiling and drywall missing in storage room. To correct ensure ceiling is not leaking, repair and finish ceiling and wall.

Found the right door of the right door/drawer prep top cooler broken and held together with strings. Found water pooling in the bottom of the left prep top cooler. Observed the nut holding the slicer blade appear to have several grooves/notches. To correct verify that nut is designed for this slicer, and if not replace with appropriate nut. To correct repair/replace cooler door and eliminate water pooling in cooler.

Observed flies in the bar floor drain and in the back kitchen area. To correct keep areas clean and dry, in good repair, and continue fly treatments.

Found the light in the back cooking hood not working and possible damaged light shield. To correct repair/replace light and light shield if necessary.

Found water seeping from the floor drain area under the glass washer in the bar/dining room waitstation area and the floor in surrounding area with pooling water and damaged floor tile, and found the basement ceiling tile under this area damaged with water dripping in to a bucket. To correct repair source of leak and repair damage tiled floor and affected ceiling tile in the basement. Found a hole in the wall by the drain line leading to sump pump, and water stained ceiling tile through out basement. To correct repair wall and repair source of leak and replace ceiling tile.

Found water pooling on the bottom of the 2 door prep top cooler and on the drawer cooler on the cooking line. To correct locate source of water, and clean/repair as needed.

Observed the presence of flies in the bar/dining area and in the keg beer drip tray. To correct keep areas clean and dry, in good repair, and continue fly treatments.

Found the basement employee dressing/break room soiled with napkins on floor and debris. Found the floor edges of the bar and basement storage area soiled with accumulation. To correct thoroughly clean floors and clean on a more frequent schedule.

Found opened/cut prosciutto with cut portion of casing without wrapping in the ice cream freezer. To correct wrap prosciutto before placing in cold hold storage.

Found wiping cloths stored on counter tops in the salad prep area and on the cooking line. To correct store wiping cloths completely submerged under sanitizer.
