Observed a lack of hand washing signage in the bathrooms by the kitchen. To correct, please provide proper reminder signage that employees must wash hands prior to returning to work in kitchen
Observed the mop in the storage area to be in the mop bucket. Mops should be hung to dry. To correct, please hang mop on existing fastner system when not in use to allow for proper drying.
Observed the lid for the outside dumpster to not be covered. When not in use, opening to dumpster shall be kept covered to avoid attracting unwanted pests or odors. To correct, please keep lid on dumpster down when not in use

The following non-food contact surfaces of equipment were observed to be unclean: 1) the food spill trays for the range cook-tops; 2) the dish washing machine food trap basket; 3) the open pvc drain receiving wastewater from the dirty side drain board of the dish washing station (located to the right of the garbage disposal). Clean these areas now and more frequently in the future to prevent this build-up.
(Critical) Observed no back-flow preventer installed on a hose connection located under the dirty dish drain board near the garbage disposal. Correct by installing an a. S. S. E. 1011 device (atmospheric vacuum breaker) on this threaded hose connection. ,

Observed some plastic ware (mostly knives) stored in the bottom of an uncovered large plastic tub on a bottom shelf at the west end of the kitchen (near the kitchen exit door to the social room). Some dust/misc. Debris has accumulated in the bottom of this tub. Discard all of these knives and ensure they are protected from contamination in the future
(Critical) Observed chemicals to be stored with clean equipment/single service items in s storage cabinet for the church (bottle of bleach and sterno among aluminum foil food pans), and liquid soap and sterno stored above food and single service items/equipment (beverage stir sticks, sweetener packets, bread baskets and utensils) in a storage cabinet for the temple. Corrected by relocating these chemicals away from foods and single service/equipment

Observed the following items to be unclean: the spill trays (2) for the range top burners; the removable insert and the housing located below it for catching food scraps at the dirty dish drain board (between the garbage disposal and the dish washing machine); and the outsides of the salt and pepper shakers in the temple's storage cabinets. Thoroughly clean these surfaces at this time and more frequently in the future

One of the kitchen hand washing sinks is out of order. Marvin indicated a replacement faucet is on order and will be installed when it is received. Additionally, some sediment in the cold water lines has collected in the valves of the kitchen fixtures. Purge/clean these faucets and valves as needed
(Critical) Two door upright cooler (st. Clare side on left)--the following rte, phfs were marked with a prep date of jan. 30th (beyond the 7 day shelf life): cooked potatoes, cooked sausage links and cooked eggs. An opened, one gallon container of milk was also on the left side of this cooler without a use-by date. Corrected by discarding these foods.

Observed two bare wood 2x4's under the microwave oven. All bare wood needs to be sealed on all sides to facilitate easy cleaning and to prevent moisture absorption (consider using a polyurethane or varnish as these products hold up better long-term than paint)
