(Critical) Found facility using bleach (chlorine) without a test kit available to verify proper concentration. To correct provide a chlorine test kit and verify proper concentration.

(Critical) Found facility using unapproved bleach (lemon scented) to sanitize dishes. To correct discontinue using unapproved bleach and replace with approved bleach, that is bleach labeled with an epa registration number and use instructions for sanitizing dishes.

(Critical) Found surfaces on the entire slicer on the front counter prep table soiled with food debris. Found the interior surfaces of the small blender soiled with food debris. To correct follow manufacturer's directions and clean slicer and blender.

(Critical) Found an opened, unsliced portion of turkey in display cooler without 7 day (day opened + 6 days) datemark. Employee corrected by providing 7 day datemark. Found sliced ham and turkey with 10 + day datemarks in the 2 door reach in cooler. Employee corrected by providing 7 day datemark on ham and turkey. Found opened packages of feta cheese, cream cheese, and smoked salmon without 7 day datemarks. Pic corrected by providing 7 day datemark on noted items.

Found no manager certified in food safety employed at facility. Pic states she has taken a servsafe class. To correct have a manager's certificate available upon request.

Found a commercial grill (electric 220 v) on prep table with stainless steel panel on wall behind grill without cooking exhaust hood. This grill was not in facility at time of preopening/new owner inspection. To correct discontinue using grill and either remove commercial grill or install a cooking exhaust hood.

Found heavy ice and frost build up on the coil of the display cooler. To correct monitor and ensure cooler is defrosting or defrost as needed.

Found household equipment ( crock pot and blender) on front counter used to prepare/hold food. To correct replace household equipment with commercial grade equipment.

(Critical) Found ready to eat (rte), time/temperature control for safety (tcs) food, wrapped and portioned lunchmeats (ham, turkey, beef and salami) and an opened carton of half and half without 7 day (day prepared + 6 days) datemark in the 2 door prep top cooler. Pic corrected by providing 7 day datemarks and has been instructed to monitor and ensure appropriate items are datemarked.

(Critical) Found no paper towels in the paper towel dispenser in the front counter handsink. Pic corrected by providing paper towels and was reminded to keep dispenser filled.

Found the floor edges in the front counter area soiled with accumulation and build up. To correct thoroughly clean floors and keep clean.

Observed mop drying in mop wringer which was in the mop sink. To correct hang mop and allow it to properly air dry.

Observed a commercial counter top fryer on the back kitchen prep table not under a cooking exhaust hood. To correct either discontinue using fryer or provide a cooking exhaust food for fryer.

Found a household rice cooker being used in the front counter area. To correct replace household rice cooker with a commercial grade rice cooker.

Found the interior of the microwave soiled with accumulation. To correct clean interior of microwave when soiled and at least daily.

Found no paper towels in the paper towel dispenser in the back kitchen handsink. To correct provide paper towels.

Found the 2 door prep top cooler not in use. To correct repair/replace cooler to hold tcs food at 41°F.

(Critical) Used a quat test kit and found the quat sanitizing solution (from the quat dispenser) in the sanitizing compartment of the 3 compartment sink below manufacturers specification of 200 ppm. Pic corrected by manually mixing quat solution and states he will follow manufacturers directions and manually mix solution, that is add 1 ounce of sanitizer concentration to 1 gallon of water. Pic disconnected the quat dispenser and placed a sign on 3 compartment sink stating do not use quat dispenser, with directions on manually mixing quat sanitizing solution. Violation corrected.

Observed water dripping from the drain line of the evaporator coil and back wall of the 2 door reach in cooler. To correct repair source of leak and ensure water is draining properly. Found the pop machine (coke) no longer dispensing soft drinks. Pic states they are swithing beverage company. To correct repair/replace pop machine or remove if no longer needed.

(Critical) Found, ready to eat (rte), time/temperature control for safety (tcs) food, portioned roast beef, a container of salami and pepperoni, an opened gallon of milk and an opened carton of half and half without 7 day (day opened/prepared + 6 days) datemarks in the front counter 2 door prep top cooler. Found opened packages of rte, tcs food, unsliced turkey and corned beef without 7 day datemarks in the back kitchen 2 door reach in cooler. Pic corrected by providing proper 7 day datemarks for all noted items, and has been instructed to monitor the proper datemarking of appropriate items.

(Critical) Found raw sushi items (tuna and salmon) offered on menu on board without proper disclosure, that is no asterisk (*) by raw food item to a footnote stating items contain raw ingredients. Pic corrected by placing an asterisk by all raw food items.

Found wall with holes from previously mounted equipment, and access holes by 3 compartment sink. To correct properly repair and finish wall to be smooth, and easy to clean.

Found hand soap dispenser missing, but soap available in the 3 compartment sink area. To correct replace soap dispenser and provide hand soap in the handsink area.

Found the shelves of the front counter display cooler lined with an absorbent type liner. To correct either remove absorbent type liner or replace with a nonabsorbent type liner.

Found facility using a domestic type, wood block knife holder that has slots that are hard to clean. To correct remove wood block knife holder and store knives in a commercial knife holder or a clean tray.

Found a stack of unwrapped coffee filters stored on top of microwave. To correct store coffee filters in it's original packaging or in a covered storage container.

Found the bottom of the display cooler soiled with accumulation. Found the bottom shelves of storage cabinets behind the front counter soiled with accumulation. To correct clean as soon as possible, and clean on a more frequent schedule.

Found wiping cloths stored on top of counter tops in the front counter area. To correct store wiping cloths completely submerged under sanitizer.

Found the 2 door prep top cooler in the back kitchen area not in use and broken. To correct repair/replace cooler to hold tcs food at 41°F.

Found the bottom shelves of the food storage shelves in the back kitchen area not at least 6 inches off the floor. To correct raise shelves to be at least 6 inches off the floor.

Health Report - 08/11/2010 (1 critical incident(s))

(Critical) I found cooked pork at 100 degrees f. Approximately 1. 5 hours after preparation, stored in a covered pan. As discussed at the time of inspection, this does not meet rapid cooling of potentially hazardous food (phf) requirements. These cooling parameters are provided above. In addition, to using one of the rapid cooling methods listed above, do not cover storage containers, during the cooling process. Secondly, use a food thermometer to monitor the cooling rate of the phf. Lastly, do not wrap individual portions of the pork until they have been rapidly cooled down. Corrected at the time of inspection. The pork has been placed into a freezer unit, to rapidly accelerate the cooling process.
