(Critical) On follow up inspection found tcs food, ham 41°F, and cheese 41°F in the top portion, and chorizo 41°F, and beef patties 41°F, in the drawers of the 2 drawer prep top cooler. Note: ice baths were used to supplement coolers to hold tcs food at 41°F. Pic states they are using ice bath to help cooler keep tcs food at 41°F and lower. Pic states management is working on reducing heat in kitchen to allow coolers to work properly. Violation corrected. On follow up inspection found tcs food hot and spicy mayo 41°F, and scented mayo 41°F in the counter top cold hold well. Note: ice bath was used to supplement cold hold well. Pic states they are using ice baths to help cold hold well to keep tcs food at 41°F. Violation corrected.

(Critical) On follow up inspection found a t-valve/sidekick installed on the mop sink faucet spout, a y shutoff installed on the sidekick, and water supply line of chemical tower on one shut off valve and other shutoff valve not in use. Violation corrected.

(Critical) On follow up inspection found tcs food (avocado spread and blanched plantains) on a time control policy, with labels indicating the 4 hour discard not passed it's discard time. This violation was corrected on routine inspection, and reverified on follow up.

(Critical) Found a hose with spray nozzle attached to y type shutoff valve installed downstream of mopsink faucet with an atmospheric vacuum breaker. To correct install a t-valve/sidekick on spout of mopsink faucet,install y type shutoff on t-valve/sidekick, connect water supply from y type shutoff to chemical tower, and connect hose with spray nozzle to the other side ot the y type shutoff valve. ,

(Critical) Found time/temperature control for safety (tcs) food, a container of avocado spread on a time control policy in a double container on the top portion of the 2 door prep top cooler passed it's discard time of 4 hours. Found tcs food, fried plantains on a baking sheet on the speed rack by fryer on a time control policy, without time control mark. Pic corrected by discarding avocado spread and provided time control mark for plantains, which were recently fried. Pic has bee instructed to monitor and ensure items on a time control policy are labeled with time marks, and discarded at it's 4 hour discard time. ,

Pic corrected by discarding all tcs food above 41°F in this cooler, and has been instructed not to use this cooler to hold tcs food, until repaired/replaced to hold tcs food at 41°F. Found tcs food, containers of creme fresh 50°F, and chipotle mayo 47°F in the counter top cold hold well. Found the 2 drawer cold hold well on the cooking line, and the cold hold well on the counter top not holding tcs food at 41°F. To correct repair/replace coolers to hold tcs food at 41°F or lower.

Found faucet in restroom missing aerator and water spashing out of hand sink when turn on. To correct replace aerator.

Found the lids of dumpster left in open position. To correct close dumpster lids, and close after each use.

Found the ceiling tile in the cooking and prep area soiled with greasy build up. To correct clean ceiling.

(Critical) Found a y type shutoff valve downstream of an atmospheric vacuum breaker on the mopsink faucet. To correct either remove y type shut off or install a hose bibb vacuum breaker downstream of shut off.

(Critical) Found the underside of the blade on slicer and various areas on slicer soiled with food debris. To correct follow manufacturer's directions and clean slicer, that is wash, rinse and sanitize food contact surfaces of slicer.

(Critical) Found time/temperature control for safety (tcs) food, bags of shredded cabbage 50°F in the 3 door vegetable reach in cooler. Pic corrected by discarding shredded cabbage. Found tcs food, marinated pork 50°F, fish mix 50°F, and ground beef 50°F in the 2 door meat reach in cooler. Pic corrected by discarding meats. Found tcs food milk 45°F and half and half 45°F in the bar cooler. Pic has been instructed to relocate milk and half and half to another cooler. Pic has been instructed not to store tcs food in all noted coolers, until repaired/replaced to hold tcs food at 41°F. A cold hold risk control plan must be submitted.

Found grease on the hood edges and build up on some of the hood filters. To correct clean hood filters and hood surfaces and clean on a schedule to preclude accumulation.

Found the 3 door vegetable reach in cooler, 2 door meat reach in cooler, and bar cooler not holding tcs food at 41°F. To correct repair/replace coolers to hold tcs food at 41°F.

Found a table top fryer on the prep table of the kitchen. To correct provide information fryer is commercial grade and place under hood if building department requires fryer to be under hood.

Found the floor, under equipment, edges and corners in the kitchen soiled with accumulation of debris. Found the ceiling soiled with dust and build up. To correct thoroughly clean floor and ceiling.

Found the ceiling above reach in cooler with peeling paint. To correct repair ceiling and finish to be smooth and cleanable.

Found a dry wiping towel wrapped around steam wand of the espresso machine. To correct store wiping towel of espresso machine submerged under sanitizing solution after each use.

Found the spray rinse nozzle hose mount no longer attached to the wall. To correct re-attach the spray rinse nozzle mount to the wall.

Found the floor covering of the office/store room with cracks, open seams and uneven surfacing. To correct repair floor to be smooth and cleanable.

Found the sides and edges of the cooking hood soiled with greasy build up. To correct clean cooking hood and clean on a more frequent schedule.

Found cooking hood filters not properly placed in cooking hood. To correct properly place filters in cooking exhaust hood. Found 3 scoops in containers of food on the grill with melted handles. To correct replace scoops.

Observed the presence of flies in the dining area. To correct keep facility clean, in good repair, and continue fly control measures.

Found tcs food, chipotle sauce 54°F and sliced pork 46°F in the top portion of the left prep top cooler on the cooking line. Pic corrected by discarding chipotle sauce and placed sliced pork in freezer for rapid cooling. Found tcs food, shredded cheese 46°F and coleslaw 48°F in the top portion of the right prep top cooler on the cooking line. Pic corrected by placing cheese in freezer for rapid cooling, and discarded coleslaw. Note: this kitchen is hot and should have ventilation system evaluated, as coolers may not work properly in hot environment.

Found the floor under the bar counter besides the ice cream drawer freezer soiled with accumulation of debris. To correct clean floor and keep clean. Found the ceiling areas surrounding the air vents soiled with dusty, greasy build up. To correct carefully clean ceiling area.

Found a household fryer used to deep fry items for customers with special requests. To correct provide a commercial grade fryer to prepare customer special requests.

Found the hood filters and hood edges soiled with grease and burnoff. To correct thoroughly clean hood and hood filters.

Found employee personal drinks in various parts of coolers. To correct designate an employee storage area and store employee personal food and drinks in designated areas only.

Found the 2 door cooler in the bar not holding tcs food at 41°F. To correct repair cooler to hold tcs food at 41°F or lower.

(Critical) Found time/temperature control for safety (tcs) food, cheese 61°F, beef patties 55°F, black bean patties 55, avocado 56°F, in top portion and eggs 64°F in the bottom portion of the 2 door sliding glass prep top cooler. Pic temporarily corrected by discarding tcs food, and has been instructed not to use this cooler to hold tcs food until repaired/replaced to hold tcs food at 41°F. Cold hold violation has repeated and a risk control plan has been issued.

(Critical) Found raw meats (beef and pork patties) stored next to ready to eat (rte) foods (cheese slices)/foods that require little or further prep, and raw shell eggs stored next to avocado sauce in the top portion of the 2 door sliding glass prep top cooler. Pic corrected by discarding all foods as they were out of temperature (see cold violation) and instructed to store raw foods away from ready to eat foods. Found raw meats (ground raw meats and raw shell eggs stored next to rte foods in the bottom of the 3 door reach in cooler. Pic corrected by moving raw foods away from rte foods. Cross contamination has repeated and a risk control plan has been issued.

Found the can opener holder soiled with accumulation. To correct clean as soon as possible, and keep clean.

Discussed the cleaning of slicer with employee, who states slicer is broken down, parts run through the dishwashing machine, and base and parts of slicer wiped with sanitizing cloth. Corrected by educating employee that slicer parts not run through the dishwasher, must be washed, rinsed and sanitized. Pic has been instructed to monitor and retrain employees to properly clean slicer.

Found a hose with spray nozzle connected to the mop sink faucet which has an atmospheric vacuum breaker. To correct either remove hose or spray nozzle after cleaning task has been completed, as a shutoff valve should not be downstream of an atmospheric vacuum breaker for extended periods of time.

Found the light above the fryers of the cooking hood not working. To correct repair/replace light.

Found dented cans on the ready to use shelf in the back storage area. To correct clearly label/identify dented cans as return to supplier for credit.

Pic corrected by moving raw meats to the lower portion of the 2 door prep top cooler, which was below and away from ready to eat foods. This is not a repeated violation as previous violation was in a different cooler and situation. ,

Found the vent covers and surrounding ceiling area soiled with dusty build up. Found the floors (under equipment, edges and corners) in the entire facility soiled with accumulation. To correct clean as soon as possible, and clean on a more frequent schedule.

Observed the presence of flies in the kitchen prep area and in the corner area of the bar. To correct keep areas clean and dry, in good repair, control and keep fruits to a minimum, and continue treating for flies.

Found the floor the between the kitchen and storage area not smooth. To correct smooth out the transition from kitchen to storage room. Found a spot light hanging on shelf providing lighting for storage area. To correct provide a more permament type lighting.

Found a bag and box of coffe stored in a crate and on the floor not at least 6 inches off the floor. To correct store food items on shelf at least 6 inches off the floor.

Found the 2 door sliding glass cooler not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F or lower.

Found no thermometer in the bar under the counter cooler. To correct provide thermometer in all coolers.

Critical corrected: at time of inspection discarded overstacked meat and cheese items that were maintaining >41°F; food items on top >50°F. Please discuss food levels with all employees and continue to monitor with your thermometer to assure all potentially hazardous food on top of the line remains at 41°F or below at all times

Potentially hazardous food shall be maintained at 41°F or below or 135°F or above. Critical corrected: at time of inspection discarded salsa, cut tomatoes, sour cream, lemon mayonnaise and evaported milk-based products found at improper temperature (50°F - 52°F). Facility will look into long-term solution for maintaining these food items at proper temperature, until then will maintain all potentially hazardous food items at 41°F or below at all times by maintaining in refrigeration units or using ice properly with ice level above the food level. Observed employee filling a water container from handwash sink front counter; per manager also using handwash sink to rinse steaming pitchers front counter. A handwash sink may not be used for purposes other than handwashing. Facility planning on installing a rinse-o-matic type device to rinse steam pitchers. Please consult with our office prior to installation for proper backflow requirements. Please consider installation of an additional rinse sink front counter to allow for filling of water bottles for dining area and rinsing steaming pitchers ( steam pitchers will be sent back for warewashing a minimum every 4 hours throughout the day )

Please provide paper towel dispenser at front counter handwash sink

Please provide some type of cup dispenser for single-service cups on tables to properly protect the cups
