(Critical) Observed the dishwashing machine at the bar to not be dispensing the proper amount of iodine sanitization solution. (nearly non-detect) person in charge immediately called ecolab for service and indicated that they were on their way and that the unit will be repaired immediately. While the bar dishwashing machine is not working properly, all washing must go through the back kitchen dishmachine. Minimum iodine concentration shall be 12. 5 ppm. Please have unit repaired prior to returing unit to service. To correct, have unit repaired where dishwashing machine dispenses a minimum iodine concentration of 12. 5 ppm. ,

(Critical) Upon arrival, two bags of frozen chicken were thawing in the left hand basin of the two compartment sink (the hand washing sink). Corrected by relocating to the right-hand food prep basin

(Critical) 1) the interior surfaces of the ice machine and ice bin in the back room have slime build-up on them. Thoroughly clean and sanitize this machine per the mfg. Directions. 2) observed a 1/6 pan on the storage rack in the back room to still have cheese sauce on it. Corrected by re-washing

The metal housing for the dish washing machine food trap screen is over-due for cleaning to remove accumulated food debris

The drain line for the ice bin in the back room no longer had the required air gap at the discharge point. Corrected by re-establishing the air gap

Two small cups were observed being used for dispensing sour cream and salsa. Corrected by replacing these cups with spoons

Both kitchen microwave ovens were observed with spilled food in them. Maintain clean

(Critical) The kitchen cook was handling a bun and condiments during assembly of a hamburger. Corrected by the employee putting on single use gloves

Wiping cloth buckets at the bar and the kitchen. Corrected by dumping out the solution and providing fresh sanitizer at +/-75 ppm

Observed one of the non-stick frying pans to have the coating coming off of it. Ensure staff are not using metal utensils with these coated pans and taking them out of use when the coating begins to deteriorate. Corrected by discarding

(Critical) Observed phfs (deli meat, cheese, hamburger, rice and beans, in-house made dressings/sauce, etc. ) in the make line prep cooler to be at 57 to 60f this morning. Corrected--these foods were discarded. The refrigeration technician was on-site prior to the end of the inspection to begin repairs. ,

(Critical) Observed a moderate amount of flies at the bar, the temporary empty beer bottle pass through window area between the bar and kitchen area. Provide approved traps in suitable locations (away from food, clean equipment & single service items) to reduce the adult population. Also, locate and maintain clean & dry any possible breeding areas. ,

Observed several bags of food items (chicken mostly) thawing in the empty food prep sink. Please use one of the approved methods to safely thaw foods

Provide a bead of silicone to the bottom of the bar keg cooler(s) as needed to prevent debris and moisture from migrating under these units

Observed a new employee drying food pans/equipment with a cloth towel. Instruct staff to allow equipment to air dry only prior to placing them in storage

Observed a few small areas of the floor at the bar with moist debris build-up. Thoroughly clean and dry these areas

1) the cook line prep cooler was operating at 60f. This unit needs repair and to be functioning at 41f or below prior to placing it back in use. 2) the small walk in cooler for produce and meats was operating at a slightly elevated temperature. Adjust this unit to a colder setting as needed

Observed some areas of the bar floor to have depressions (due to missing/worn tile grout) and/or rough surfaces. Repair these areas to eliminate the depressions that allow moisture and debris to accumulate

Observed a few out of the way floor areas at the bar to have debris and moisture accumulated. Maintain these areas clean and dry to eliminate fly breeding areas. Consider running a floor drying fan in the bar when the facility is closed (after mopping) to help dry out the floor

A couple flies were observed by the bar pass through window into the kitchen where empty beer bottles are temporarily stored. Ensure staff are regularly monitoring the facility for the presence of flies and possible breeding locations.

Observed the following areas to be in need of more frequent cleaning: the interior of the ice making machine, and inside the spout of the soda dispensing gun

Observed beer residue from the previous day on the interior surfaces of the draft beer tower spouts. Ensure staff are cleaning these spouts nightly

Observed some spilled food (chicken wings?) on the floor of the walk in beer cooler. Please clean the floor to remove this food

No soap was available at the bar hand washing sink. Corrected by an employee purchasing a small pump dispenser

Observed food debris build-up on the sides of the dish washing machine drain pan housing. Corrected by cleaning this equipment. Ensure staff clean these surfaces regularly to prevent excessive build-up

(Critical) Observed foods in the small walk in cooler to be at 42 to 48f. The kitchen manager indicated the door was opened a lot prior to my arrival to put away an order. The foods checked (oranges, pickles in a 5 gallon bucket, hot dogs in an enclosed plastic drawer unit, and cooked and portioned chicken wings) were not foods just delivered. Repair/replace this unit as needed to maintain foods at 41f and below, and keep the door closed as much as possible

(Critical) No chlorine residual was observed in the rinse water of the kitchen dish washing machine. A leak in the chlorine tube was discovered between the pump and the inlet to the dish washing machine. Corrected by eco-lab repairing the unit to provide 50- 100 ppm chlorine

Observed the potential for bare hand contact when an employee used a beer glass to scoop ice out of the bar ice bin for a personal drink. In the future, only utilize a handled ice scoop to fill glasses with ice. Additionally, the risk to your patrons from broken glass is much higher

Observed many droplets of oil on the bottom side of several upper level shelves on a unit adjacent to the three door freezer. Corrected by cleaning these shelves

Observed two cardboard boxes being used to store small utensils on the shelving unit in the dish washing room. Replace these boxes with plastic or metal ones that are cleanable

Ensure the dish washing machine is tested at least daily for proper chlorine concentration with the provided test papers

Grease/oil recovery vessel outside: clean the cover, front panel and the pavement located in front of this unit to eliminate the spilled oil

(Critical) Observed foods in the following locations to not be date marked: kitchen: two door upright cooler--observed numerous rte, phfs (spanish rice, saut√Čed onions, cole slaw, pinto beans, hard boiled eggs, milk and half and half, etc. ) bar: two open quarts of half and half (corrected by discarding). Please ensure your ready to eat, phfs are date marked after preparation/opening

(Critical) Upright freezer: observed two small packages of ground wild game (venison). Corrected by removing this meat from the facility. Ensure unapproved foods are not brought into the facility

The small walk in cooler was observed to be at a slightly elevated temperature. Adjust this unit to maintain 41f or below

Walk in beer cooler: observed two to three florescent light bulbs with out a shatterproof coating nor a bulb guard.

Observed the ice transfer pail stored upright on a wire shelf in the back storage room. Please keep your ice pails stored inverted on a clean and sanitized surface to allow water to drip out and to prevent misc. Debris accumulation.

Observed a package of chicken breasts thawing in a pail of standing water in the food prep sink. In the future, please thaw frozen food either, 1) under refrigeration, 2) under cold running water (only long enough to thaw the food--food must stay below 41f), or 3) as part of a continuous cooking process
