Found the sides of the back kitchen table drawers to need routine cleaning. Correct by increasing the frequency of cleaning

Observed the back kitchen areas to need routine maintenance: found loose base coving along exterior base of walk-in cooler. Wall surfaces under hand sink at dishwashing area needs repairs. Floor tiles surfaces nolonger easily cleanable at back areas. Provide service to worn ceiling tiles as needed. Correct by providing scheduled maintenance as needed

(Critical) Observed no soap available at the hand sink located in the front. Note: only hand sanitizer available for use. Each hand washing sink or group of 2 adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap. (pf) corrected by providing hand soap to the noted sink.

(Critical) Observed several items that were past the facility use by date: note: today's date is 7/8/2014. Note: all noted food was not in the cooler designated for waste food. Some of the items were as follows: 1. Two containers of smoked white fish spread 6/11. 2. A plate of salmon 7/7. 3. Facility made thousand island dressing 7/7. Potentially hazardous food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded when held at a temperature of 41�f or less for a maximum of 7 days. (pf). Correct by discarding the noted food into the garbage or placing into the waste cooler and filling out the wasted food log. Corrected: person in charge discarded the noted food into the garbage.

Observed the following non-food contact surfaces with a build up of food debris: 1. Interior of several grey food storage containers in dry storage. 2. Interior cupboard area of the front lobby display cooler, especially around items such as metal casing for wires. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Correct by cleaning the noted items and cleaning them at a frequency that maintains them free of soil build up

Observed a home use safety razor and shaving cream on the prep table with food near next to the one door freezer. (a) dressing rooms shall be used by employees if the employees regularly change their clothes in the establishment. (b) lockers or other suitable facilities shall be used for the orderly storage of employee clothing and other possessions. Corrected by placing the noted items into the designated area for employee belongings.

Observed the lid to the robot coupe food processor to be in poor repair. Nonfood-contact surfaces shall be free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance. Correct by replacing the noted lid to the food processor

Observed the front long top loading display cooler in the lobby to have an ambient air temp of 55f and food that was just put in the cooler was rising in temperature. Note: the coolers condenser could not get air with the wooden housing around the condenser. Equipment shall be maintained in a state of repair and good condition. Corrected by removing the wooden housing to the condenser. Note: with wooden housing removed cooler was noted with an ambient air temperature of 37f.

(Critical) Found no cold water running from kitchen handsink faucet and employees not able to properly wash hands as hot water was to hot to wash hands for a minimum of 20 seconds. To correct repair faucet to provide cold water so warm water is provided to properly wash hands. ,

(Critical) Found an empty box, case of bottled drinks and step ladder stored in front and around handsink in the front counter corner area. Pic corrected by removing items and has been instructed to keep handsink accessible at all times. ,

(Critical) Found time/temperture control for safety (tcs) food, pork roast with vegetables 115°F in the self service buffet. Pic states he may not have added enough water in steam table to keep tcs food at 135°F and would add more water. Pic corrected by reheating pork roast to 165°F for 15 seconds and was instructed to monitor and ensure tcs food is reheated to 165°F for 15 seconds and held at 135 and above. ,

Found the tiled floor in the kitchen with chipped missing tile and coving missing in various spots. To correct repair wall and coving to be smooth, durable and cleanable.

Found the hood filters with greasy build up on edges. Found the exterior cover of robot coupe soiled with build up. Found the nonfood contact surfaces of slicer soiled with food debris. To correct clean noted areas and clean on a more frequent schedule.

Observed employee dry dishes with a towel, corrected by educating employee to allow dishes to air dry.

Found the floor under equipment, edges and corners behind freezers in the prep area soiled with build up. To correct thoroughly clean floor and keep clean.

Found no manager certified in food safety. To correct provide a manager certified in food safety.

Found no handwashing sign in the dishwashing handsink area. To correct provide sign reminding employees to wash hands.

Pic corrected by discarding tcs food, and has been instructed not to use this cooler to hold tcs food, until repaired/replaced to hold tcs food. Note: this is not a repeated cold hold violation, as this is a different cooler. ,

Found the floor under equipment, edges and corners in the front counter area and back kitchen cooking and prep areas soiled with build up and accumulation. To correct thoroughly clean floors and clean on a schedule to preclude accumulation.

Found numerous wrapped sliced bread without labels indicating ingredients and food allergens. To correct have ingredients and food allergen information available upon customer request.

Found the interior bottom of the 3 door prep top cooler soiled with accumulation. Found the storage shelves under the buffet cooler soiled with accumulation. To correct clean as soon as possible, and clean on a more frequent schedule.

Found the 2 door display cooler not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F or lower.

(Critical) Found time/temperature control for safety (tcs) food, hummus 45°F, walnut feta 45°F, gazpacho 45°F and chicken noodle soup 45°F in the 2 door sliding glass reach in cooler. Pic temporarily corrected by moving tcs food to a properly working cooler, and has been instructed not to use this cooler to hold tcs food, until repaired/replaced to hold tcs food at 41°F or lower. This cold holding violation has repeated in 3 of 5 routine inspections, a risk control plan and administrative conference notice has been issued.

Found the floor in the kitchen under equipment, edges and corners soiled with accumulation and build up. To correct thoroughly clean floors and clean on a more frequent schedule.

Found the wall in the wall way leading to the kitchen with patches that need painting. Found the floor by the prep and 3 compartment sink chipped with damaged surfaces. To correct properly repair wall and floor to be smooth, durable and easy to clean.

Found the floor (under equipment, edges and corners) in some areas of facility soiled with accumulation. To correct thoroughly clean floors and clean on a more frequent schedule.

Found the walls in the front counter area and in the back kitchen with scratchs, and uneven surfaces. To correct repair and finish walls to be durable, smooth and easy to clean.

Found no handwashing sign in the kitchen dishwashing area handsink. To correct provide a sign reminding employees to wash their hands.

Found the interior bottoms and door tracks of coolers soiled with accumulation of debris. Found the top of the cooking hood and hood filters soiled with greasy build up. To correct clean as soon as possible, and clean on a more frequent schedule.

Found tcs food, pork 45°F, corned beef 45°F in the shelves of the walk in cooler in the kitchen prep area. Pic corrected by having employees moving tcs food from 2 door glass sliding door cooler and the walk in cooler to properly working coolers and has called for repair. Pic has been instructed not to use these coolers until repaired to hold tcs food at 41°F. Pic states cooler has been repaired and has submitted a rcp for cold holding for this cooler, which was reviewed, and appears to be working (log sheet for cold holding was taped on cooler). Violation corrected. On follow up inspection found tcs food, beef 41°F, cheese 41°F, and rice and beans 41°F in the walk in cooler. Pic states walk in cooler has been repaired and is holding tcs food at 41°F. Violation corrected.

Found the 2 door sliding glass cooler and the walk in cooler not holding tcs food at 41°F. To correct repair/replace coolers to hold tcs food at 41°F.

Found the drain line of the salad buffet cooler which drains into the handsink loose and not secured to wall. To correct secure drain line and ensure an air gap of at least 1 inch above the rim of the handsink is maintained.

Found the floor under equipment on the cooking line soiled with accumulation. Found the ceiling air vents in the dining room soiled with dusty build up. To correct thoroughly clean floor and air vents, and clean on a schedule to preclude accumulation.

Found the bottom cabinet shelf of the salad buffet cooler soiled with accumulation. Found the edge of the cooking hood soiled with grease. To correct clean as soon as possible, and clean on a more frequent schedule.
