Once removed from original container please label all spray bottles with common name of chemical to avoid misuse. Corrected at time of inspection by properly labeling all chemical spray bottles with common name of chemical

(Critical) No sanitizer present at automatic warewashing machine. Sanitizer shall be provided at proper concentration per manufacturer's label use instructions. Not corrected. Eco-lab has been notified. All dishes and utensils shall be manually sanitized in 3-compartment sink until dishmachine repaired. Facility has chlorine test kit and bleach. ,

Observed employee with jewelry on wrists. Food employees may not wear jewelry on their arms and hands except for a plain ring such as a wedding band when preparing food. Food employee teaching a cooking class at time of inspection

These areas in need of cleaning: top of knife rack front kitchen and top of some bulk food bin lids front kitchen. Please clean now and as often as necessary to maintain in clean condition

Observed leak at end of spray rinse arm at 3-comp sink; currently wrapped in plastic wrap. Please repair; per pic part is on order to repair.

Observed two of the handwash sinks without paper towel dispensers. Please provide paper towel dispensers at all handwash sinks that dispense the paper towels easily

(Critical) Found commercial hot water tank replaced with a residential hot water tank which has a significant smaller btu rating of 40,000 and capacity of 40 gallons. The original hot water tank specifications in the plan review had a 197,000 btu and 100 gallons capacity. To correct replace residentail hot water tank with one that meets original specifications.

Found the floor under equipment, edges and corners soiled with some accumulation. To correct thoroughly clean floors and keep clean.

Observed operator repairing dishwashing machine at time of inspection and unable to test dishwashing machine. To correct repair dishwashing machine and use test kit to verify that it is chlorine sanitizing dishes.

(Critical) Found time/temperature control for safety (tcs) food, cut and diced watermelon 45°F in the produce walk in cooler. Pic temporarily corrected by placing cut watermelon in another properly working walk in cooler to cool back down to 41°F. Pic has been instructed not to use the produce walk in cooler to hold tcs food, until repaired/replaced to hold tcs food at 41°F. ,

Observed some mouse droppings on the floor under shelves in the storage room. To correct clean floor, keep facility in good repair by sealing all cracks and gaps, and continue mouse control measures.

Observed water on the floor around the dishwashing area. To correct find source of water, and repair as needed.

Observed numerous single use cups and lids drying on a table. To correct discontinue washing and reusing single use containers and lids, and replace with commercial grade equipment.

Found a cracked grill plate on the range top under the cooking hood. To correct replace cracked grill plate. Found produce walk in cooler not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F or lower.

Found the floor in the storage room soiled with debris. To correct thoroughly clean floor and keep clean.

Found an opened cup of beverage, opened uncapped bottle of vitamin water, and an opened bag of cheese crackers on prep table in the kitchen. Pic corrected by relocating food items to employee table away from prep areas. Pic has been reminded to monitor and ensure employees are eating and drinking in designated areas only, and if desired employee may drink from a covered cup with straw in prep areas.

Employee explained how prep table is cleaned, that is table is washed with soap, and sanitizer is sprayed on table. Corrected by educating employee that table is to be washed/wiped down with soapy water, rinsed with clean water, and then sanitized. Pic has been instructed to monitor and ensure food contact surfaces are properly cleaned, that is washed, rinsed, and sanitized.

Observed the light shield above the dishwashing area with cracks. To correct repair/replace light shield.

Found a household grinder on prep table. To correct replace with a commercial grade grinder. Found particle board on the back area shelves. To correct seal particle board shelving to be non-absorbent.

Found the edges of the cooking hood and hood filters soiled with build up. To correct clean as soon as possible, and clean on a more frequent schedule.

Found cut fruit (honey dew and cantalope) in the produce walk in cooler without 7 day datemark. Pic corrected by providing 7 day datemark for cream and discarded goat cheese and cut fruit. Pic has been instructed to monitor employees datemarking appropriate items. This is a repeated critical violation and a risk control plan for datemarking has been issued. ,

This is a repeated critical violation and a risk control form and an administrative conference notice has been issued. ,

Found the coving on the corner of the cooking line missing. Found the stainless steel paneling raised on the wall on the end of the cooking line. Found the floor in various areas of the kitchen with pitting and uneven surfaces. To correct repair/replace coving, stainless steel paneling, and floor.

Found the interior of the microwave soiled. Found the handles of knives in the knife holder stickyy. To correct clean as soon as possible, and clean on a more frequent schedule.

Observed opened seams on the ceiling of the back kitchen prep area. To correct secure paneling and caulk/fill seams of cooking hood and ceiling.

Found a spray bottle of oil without label on top of the prep table. Employee corrected by labeling spray bottle of oil.

Found the floor ( under equipment, edges and corners) in the front kitchen and back storage area soiled with accumulation of debris. To correct clean as soon as possible, and clean on a more frequent schedule.

Found the door handles and exterior of the 2 door reach in freezer doors soiled. Found the shelf under the cutting board storage rack in the front prep area soiled with accumulation. Found the interior of the microwave in the front kitchen soiled. Found the lids of the bulk food storage containers in the front kitchen soiled with accumulation. To correct clean as soon as possible, and clean on a more frequent schedule.

Found coffee filters stored on top of coffee machine outside of packaging. To correct store coffee filters in a more protected manner, that is leave coffee filters in packaging or in a covered storage box.

Following rcp is essential in facility overcoming repeated violations of items passed it's datemarked time.
