(Critical) Found ready to eat (rte), time/temperature control for safety (tcs) food, an opened block of feta cheese passed it's 7 day (day opened + 6 days) in the walk in cooler. Pic corrected by discarding feta cheese and was instructed to monitor and ensure rte, tcs food passed it's 7 day datemark are discarded. Note: found yogurt about a month passed it's dated mark on carton in the walk in cooler. Pic discarded yogurt and was reminded to check dates and rotate stock.

(Critical) Found the blade and gear of the can opener on the prep table soiled with accumulation. Found the blades of the cutters on the storage shelf and clean side of the drain board soiled with dried food debris. Pic corrected by placing can opener and cutters in the 3 compartment sink to be washed, rinsed and sanitized. Pic has been instructed to monitor and ensure equipment is properly cleaned.

Found the pot and pan detergent dispenser on the wall above the wash compartment of the 3 compartment sink missing piping from the pump assembly. To correct repair/replace missing piping from pump. Found excess ice build up on the edge of the walk in freezer door. To correct defrost and keep ice build up to a minimum.

Found the floor under equipment, edges and corners of the kitchen soiled with accumulation. To correct throughly clean floor and keep clean.

Found several lights in the ceiling lights in the kitchen not working. To correct repair/replace lights.

(Critical) Found a tightly covered tray of quinoa cooling on a speed rack in the walk in cooler. Pic corrected by partially removing plastic covering allowing quinoa to more rapidly cool. Pic has been instructed to monitor and ensure tcs foods are cooled uncovered

Found the floor under shelves in entire facility soiled with accumulation of debris. To correct thoroughly clean floors and clean on a more frequent schedule

Found employee personal drinks, that is bottles with lids that have to be handled with hands prior to drinking stored under the prep tables. To correct replace bottle drinks with beverage containers that prevents contamination of employee hands, such as cups with lids and straws.

Found a plastic squeeze bottle of olive oil without label. To correct provide label.

Pic corrected by having employee mark items with 7 day (day opened/prepared + 6 days) datemarks. Pic has been instructed to re-train employees on properly datemarking appropriate items. ,

(Critical) Found no sanitizer in the sanitizing compartment of the three compartment sink. Pic temporarily corrected by re-making sanitizing solution, and verifying with test kit. To correct have sanitizing dispenser repaired/calibrated to dispense at (manufacturer's)proper concentration(150 to 400 ppm). ,

Found chipped tile in the walk in freezer. To correct repair/replace tiled floor in the walk in freezer.

Found staff using inappropriate quat test kit (qt-10) to test multi-range quat sanitizer. To correct provide quat test kit (qt-40) to test sanitizing solution.

Found the floor in kitchen, under shelves and edges soiled with accumulation. Found the window screen soiled with heavy dusty accumulation. To correct thoroughly clean floors and window screen.

Found the quat dispenser not connected to sanitizer and employee states dispenser broken and they manually add sanitizer. To correct repair quat sanitizer dispenser.

Found numerous signs on the right walk in cooler floor. To correct determine if signs are needed, and either secure signs to shelves or discard signs.

Found the tiled floor by the entrance of the walk in freezer chipped and damaged. To correct repair walk in freezer door floor to be smooth.

Found an opened container of salad dressing in the walk in cooler without lid. Pic corrected by covering salad dressing with plastic wrap as food shall be covered/protected.

Found a wood pallet use to store beverages in the walk in cooler. To correct either use a shelf/pallet that is nonabsorbent or seal/paint pallet to be nonabsorbent, as nonfood contact surfaces of equipment shall be nonabsorbent.
