Pic corrected by discarding tcs above 41°F and has been instructed not to use cooler, until repaired/replaced to hold tcs food at 41°F. Found tcs food, meatballs 50°F in the top portion of an overloaded container in the pasta cold hold well. Pic corrected removing meatballs in the top half of container and placed them in reach in cooler. Pic has been instructed to monitor and ensure employees are not overloading food containers.

(Critical) Found time/temperature control for safety (tcs) food, hummus 45°F and garlic hummus 46°F on the left side of the glass display cooler in the vegetarian side of the facility. Pic corrected by discarding hummus and has been instructed not to use this side of the cooler as other items in this cooler were at 41°F, until repaired/replaced to hold tcs food at 41°F.

Found the glass display cooler in the vegetarian side of facility not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F or lower. Found the handles on both rice cookers lids missing. To correct replace missing handles on rice coolers.

Found facility using pop carrying trays to store clean utensils. To correct discontinue using pop carry trays to store utensils and replace with commercial grade storage containers.

Found the floor under equipment, edges and corners soiled with accumulation of debris. To correct thoroughly clean floors and clean on a more frequent schedule.

(Critical) Found an opened carton of cottage cheese passed it's 7 day datemark in the island court prep area cooler. Pic corrected by discarding cottage cheese, and has been reminded to verify staff are checking dates and discarding food passed it's 7 day datemark (day opened + 6 days).

(Critical) Used a quat test kit and found no quat sanitizer residual in the sanitizing compartment of the 3 compartment sink. Pic corrected by changing empty bottle of sanitizer, adding quat sanitizer, and verified with a test kit. Pic has been instructed to monitor and verify that staff are adding sanitizer, re-stocking supplies when necessary, and checking sanitzing concentration with a test kit.

Found the floors under shelves in the storage room soiled with accumulation and spider webs. Observed excess water on the floor in front of the pizza prep top cooler. To correct thoroughly clean floors and keep clean.

Found wicker type baskets used to hold whole fruits on the cooler in the self service area. To correct either line baskets or replace with containers that are durable, smooth, and cleanable.

Found a case of disposable towels on the floor under the island court counter top. Pic corrected by placing disposable towels on shelf.

Found a covered container of soup cooling in the walk in cooler. Pic corrected by discarding soup and has been reminded to rapidly cool tcs food, that is to divide/reduce size of food, place in shallow pans in an ice bath, and then place in walk in cooler uncovered. Pic has been reminded to cool tcs food from 135°F to 70°F in 2 hours and then from 70°F to 41°F in 4 hours.

Found the veg display cooler, i door under the counter cooler, front counter display cooler, and the 2 ice cold wells on top of cold hold plates not holding tcs food at 41°F. To correct repair/replace coolers and cold hold wells on top of cold plates to hold tcs food at 41°F or lower.

Observed the presence of flies in the back kitchen areas. To correct keep areas clean and dry, in good repair, and continue measures to control flies.

Pic corrected by providing 7 day datemarks (day opened/prepared + 6 days) for all noted items. Pic has been instructed to monitor that employees are properly datemarking items.

Vegetarian area: found the floor under shelf soiled with wrapped sleeves of cups and debris. To correct clean area and keep utensils on shelves.

Found no handwashing sign in the handsinks of the island court area. Found no handwashing sign in the mens public restroom handsink area. To correct provide signs reminding employees to wash their hands at all locations where they may wash thier hands.

(Critical) Observed an improper consumer advisory for the sushi items that contain raw fish/seafood. Currently, the statement on the label only indicates the item contains raw fish (disclosure). The reminder statement is missing. This food is made on campus by a contracted sushi operation. This issue was brought to the attention of the manager for the sushi operation. This manager indicated his regional manager would contact me regarding the required revisions to the consumer advisory

(Critical) Observed the multiple quat sanitizer buckets at the island shops to be mixed way too strong. Corrected by providing 200 ppm quat solutions

(Critical) Observed the following items to be at improper temperature: 1) cooked spaghetti noodles at 53f in a plastic dispensing container (ice below was melted and not in contact with the container holding the pasta)--additional ice was added to the pan below. 2) sour cream and a house made cilantro mayo dressing were found at 50 to 52f in an ineffective cold holding set-up (a metal box with food insert pans in it located on top of a freezer plate)--these foods were relocated to a reach-in cooler. This set-up is not going to maintain phfs at proper temperature. Do not use this set-up for phfs. 3) shredded cheese and sliced cherry tomatoes were found in metal insert pans at the hot dog station to be at 52 to 49f, respectively--these items were placed in a reach-in cooler. 4) prepackaged sushi (only four containers left) was observed in an open air grab & go display cooler at 47 to 54f. The sushi was discarded. This cooler may only be used for the storage/sale of non-potentially hazardous foods

Observed the two rice cooker/warmer insert pots to have some of the non-stick coating worn off of them. The manager indicated they just received replacement units for this school year. After cleaning these new insert pots begin using them asap. Ensure staff are only using plastic utensils and non-metal scouring pads for cleaning

Observed no sell by dates provided on the emu baked and packaged cookies. Provide sell by dates on the labels when the cookies are packaged

Observed old food particles stuck in the orifices of the turbo wash basin of the three compartment sink. This food debris needs to be eliminated. Contact the manufacturer to find out what cleaning methods will be effective for removing this build-up

Observed the door gasket to be torn on the one door low-boy cooler at the vegetarian station. Please replace this gasket

Observed the hot dog station metal stem food thermometer to be reading +/- 7f too high. Please re-calibrate this thermometer prior to it being used again

1) one of the freezer plates is being used to hold phfs at 41f and below, however, this set-up is not effective. Discontinue the use of this set-up for phfs. 2) the open air grab & go cooler for bottled water, juice and non-potentially hazardous salad dressings was also being used for selling the sushi. This unit is not maintaining the sushi at 41f and below and may not be used for sushi sales or any other phf
