(Critical) Observed several refrigerated, ready-to-eat, potentially hazardous food items without date marking. Once opened, prepared or removed from the freezer these food items shall be clearly marked with the discard date. Add 6 days. Corrected at time of inspection by clearly marking opened milk, soups and several food containers in upright reach-in-coolers. Date prepared or opened known by pic. If the food item is prepared/opened and consumed within 24 hours that food item is exempt from date marking. Now that business is slower the food may exceed the 24 hour time period. Please begin marking all food items that are not consumed within 24 hours
No ambient air thermometer present in front counter display cooler. Please provide accurate ambient air thermometer and place for easy viewing.
Observed several clean pans stored improperly on clean dish shelving unit rear kitchen. Please invert clean dishes to protect against potential contamination
Observed food employee without hair restraint. Food employees shall wear hair restraints that effectively restrain their hair. Food employees may contaminate their hands when they touch their hair. Hair restraint keeps dislodged hair from ending up in the food and may deter employees from touching their hair
Observed some food containers that are not durable. Please provide durable, food grade containers that are smooth and easily cleanable
(Critical) Observed improper cooling method in place for two rice dishes. Cooked potentially hazardous food shall cool within 2 hours from 135°F to 70°F and within a total of 6 hours from 135°F to 41°F using one or more of the following methods listed. Corrected at time of inspection by placing rice in shallow pans uncovered and stirring while on counter until they are placed in reach-in-cooler or reach-in-freezer to finish the cooling process. Pic will continue to use thermometer throughout the cooling period to assure proper cooling has occured. Provided pic with cooling chart; please complete for all food items that you cook and cool. Please include the cooling method as part of the recipe. You may fax or e-mail cooling chart when completed

Observed employee washing hands in 3-compartment sink. Please wash hands in handwash sinks only. There is no handwash sink on prep side of kitchen. There is one handwash sink in dishwash area and one handwash sink at front counter. Per owner facility went through plan review for remodel approximately 11 years ago; did observe approval letter in our files for addition in april 2004. It appears no additional handwash sinks were required; will discuss with our department.
Observed some food containers with cracked lids and one with cracked top ledge. Please replace to provide smooth, easily cleanable containers

(Critical) Observed 2 different employees handling and/or cutting rte pita bread in prep area with bare hands. Food employees shall not handle rte foods with bare hands and shall utilize proper utensils and plastic gloves at all times. Corrected by educating staff on bare hand contact, rte foods and foodborne illness risk factors at time of inspection. Please educate and train other staff members on proper glove use and handwashing as needed.
