(Critical) Used a quat test kit and found the quat sanitzing solution in the 3 compartment sink below manufacturer's recommended range of 150 to 400 ppm. Pic corrected by remaking sanitzing solution to about 200 ppm. This is a repeated violation and a risk control plan must be submitted.
(Critical) Used a temperature-sensitive tape and found the dishwashing machine not hot water sanitzing dishes. To correct call for repair and use the 3 compartment sink to wash, rinse and sanitize dishes.
(Critical) Found cold hold time/temperature control for safety (tcs) food, yogurt 50°F, fruit cups 46 to 50°F, milk 46°F, and salad 50°F in the display cooler. Pic corrected by discarding tcs food, and has been instructed not to use this cooler to hold tcs food, until repaired/replaced to hold tcs food at 41°F. Found hot hold tcs food, broccoli soup 99°F and chili 115°F in the hot hold soup well. Pic corrected by reheating soup to 165°F for 15 seconds, and was reminded to monitor and ensure hot tcs food is held at 135°F or above.
Found the display cooler not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F or lower.
Found the interior of the microwave oven soiled with splash and food debris. To correct clean microwave and clean as needed.
(Critical) Found spray bottles of cleaning chemicals stored next to food utensils on the shelves below the soup wells. Pic corrected by relocating chemicals to the chemical storage shelf, and has been reminded to monitor and ensure chemicals are stored properly.

Found two long stemmed thermometers which are used to monitor soups not working. To correct repair/replace thermometer. Found a dial faced thermometer that was out of calibration and with a limited range of -40 to 160°F. To correct calibrate thermometer and in future provide dial face thermometers with a range of 0 to 220°F.
(Critical) Found time/temperature control for safety (tcs) food, bagel dogs which are under a time as a public health control, without time control mark. Pic corrected by finding time bagel dogs were prepared and removed from temperature control, and set the timer to indicate the time that is 4 hours past the point in time when the food is removed from temperature control. Found tcs food, carafes of milk and half and half which are under a time as a public health control, on the self service without time control mark. Pic corrected by having employee verify time milk and half and half were filled in carafes, and filled in log sheet which is used to track 4 hour discard times. Pic has been instructed to monitor and verify that employees are following time policy, that is setting timer or filling in log sheets to indicate the time that is 4 hours past the point in time when the food is removed from temperature control.
Found the bottom shelves of the front counter soiled with accumulation of debris. Found the interior of microwaves soiled with splash and accumulation. To correct clean noted areas and clean on a more frequent schedule.
Observed the presence of flies in the front counter area. To correct keep area clean and dry, in good repair, and continue fly control measures.
Found bench brush which is used to clean counter tops stored on top of plastic wrap dispenser. Pic corrected by relocating bench brush to bottom shelf of front counter, as food wrap should be protected from contamination.
Found 1 of the evaporator fans in the walk in freezer not working. To correct repair fan.
Found the coffee level indicators of the coffee urns in the front counter area soiled with mineral build up. To correct follow manufacturers directions and clean indicators.
Found the basement storage room floor soiled with accumulation of debris. Found the floor behind the front counter cooler soiled with drips. Found the floor in the main level storage room soiled with accumulation of debris. To correct clean as soon as possible, and clean on a more frequent schedule.
Found the bottom shelf of storage room shelf not at least 6 inches off the floor. To correct raise shelf to be at least 6 inches off the floor.
Found the lights in the basement storage room floor not working. Found some of the lights in the main level storage room not working. To correct repair/replace lights in storage areas.
Found the storage cabinet shelves in the front counter area soiled with accumulation. To correct clean as soon as possible, and clean on a more frequent schedule.
Found slight accumulation on the internal diffusers of the pop nozzles of the self service pop machine. To correct follow manufacturers directions and clean pop nozzles and diffusers.
Found the floor (under shelves, edges and corners) in the basement storage area soiled with accumulation. To correct thoroughly clean floors and clean on a more frequent schedule.
Found the bottom shelf where dry goods are stored not at least 6 inches above the floor. To correct adjust shelf to be at least 6 inches above the floor.
(Critical) The four carafes of dairy product at the customer self service counter are not being reliably washed, rinsed and sanitized by staff on the four hour intervals. The chart for this chore was blank for the prior two days. Modify your current system (provide a timer for this specific task and provide use-by time stickers on the individual containers) to ensure staff are switching these carafes with clean ones on intervals not to exceed four hours. ,
The dish washing machine is over-due for de-liming.
Observed excessive dust accumulation on the four ceiling vent grates (two located south and two located east the walk in oven). Remove these grates and thoroughly clean them.
1) observed kitchen knives stored in a container of sanitizer between uses at the bagel sandwich prep station. Please only store your knives on a clean and sanitized surface (such as on a cutting board, in a clean 1/3 pan or on a counter with deli tissue, etc. ) between uses. Sanitizer is required to air dry prior to using equipment. 2) observed a few plastic 1/3 and 1/6 pans stored on a shelving unit that were stacked while still wet. Please allow equipment to air dry prior to stacking them in storage
Observed wet wiping towels stored in less than 200 ppm quat sanitizer. Provide fresh sanitizer solution for these pails
Observed the two long stemmed soup thermometers to be reading 105 and 120f in soup that was at 170/180f. Corrected by re-calibrating in an ice bath to 32f
Observed the two door low-boy cooler to be maintaining a slightly elevated temperature. Adjust this unit as needed to maintain 41f and below
