(Critical) Observed an employee washing chef knives with soap, rinsed with clean water and place knives on a lined tray in the cooking prep area prep sink. Asked employee chef knife washing procedure and employee states knives are washed with soapy water, rinsed and then placed on tray. Pic corrected by educating employee that utensils are to be washed, rinsed, and sanitized, and had employee sanitize knives. (Critical) Used a quat test kit and found the sanitizer flowing from the quat dispenser in the mop sink, where employees may fill sanitizing buckets below manufacturers specification of 150 to 400 ppm. Pic temporarily corrected by placing a do not use sign on dispenser and states facility will use the 3 compartment quat dispenser. To correct repair quat dispenser to dispense sanitizer at proper concentration. This is a repeated priority violation and a risk control plan must be submitted. ,

Used a quat test kit and found the quat dispenser in the mop closet, which is use through out facility to wipe down food contact surfaces dispensing below manufacturer's specification of 200 ppm. Note: this facility normally uses a quat sanitizer with a manufacturer's specification of a range of 150 to 400 ppm.
Found numerous containers stacked wet on the clean utensil storage shelves. To correct allow containers to air dry prior to stacking and storing.

Observed employees with beards working in the food prep area not wearing beard restraints. To correct provide beard nets and remind employees to wear them.

Pic has been reminded to monitor and ensure cold tcs food is held at 41°F or lower and hot tcs food is held at 135°F or above, and not to use cold or hot holding units until repaired to hold tcs food cold at 41°F or lower and hot 135°F or above. Cold hold violation has repeated and a risk control plan has been issued. ,

Provide sanitizer wipes for staff to use at this station. 2) ensure staff are sanitizing the steam kettles after cleaning. Provide a convenient way for staff to sanitize these kettles after cleaning
Provide a probe thermometer with a small diameter tip to accurately temp. The center of your chicken, fish and hamburger patties being cooked at the grill station
Observed the following items to be in need of cleaning at this time: 1) the fan grates for the cooling system in the dairy walk in cooler; 2) the outside of the red, two door hot box at the grill station; 3) the spill tray for the pepsi soda dispenser is over-due for cleaning
A pressure vacuum breaker was removed from a water supply serving a flexible hose with a spray nozzle on the end of it at the steam kettle station. The water supply serving this hose is now provided with an a. S. S. E. 1052 atmospheric vacuum breaker. Water supplies serving hoses with down stream shut-offs are not compatible with atmospheric vacuum breakers and must be configured with either a pressure vacuum breaker (a. S. S. E. 1020/1056 device) installed at 7. 5' above the floor or an rpz (a. S. S. E. 1013 device) at any elevation.
Observed a metal stem dial-type food thermometer in the kitchen to be out of calibration (reading 13f too high), and a second thermometer to have a damaged needle. Corrected by discarding these thermometers
Observed some cracked door gaskets on the glass door pass through salad bar area cooler. Replace damaged gaskets as needed to provide a cleanable surface that doesn't collect food debris
The floor in the dish washing room (under the shelving unit with the colanders on it) is in need of cleaning. The floor under the soda dispensing stations and the shelf below the dispensing unit are in need of cleaning at this time
