(Critical) Observed an employee washing chef knives with soap, rinsed with clean water and place knives on a lined tray in the cooking prep area prep sink. Asked employee chef knife washing procedure and employee states knives are washed with soapy water, rinsed and then placed on tray. Pic corrected by educating employee that utensils are to be washed, rinsed, and sanitized, and had employee sanitize knives. Pic has been instructed to monitor and ensure employees are properly washing utensils, and reminded pic that 3 compartment sink should be used to wash knives. ,

(Critical) Used a quat test kit and found the sanitizer flowing from the quat dispenser in the mop sink, where employees may fill sanitizing buckets below manufacturers specification of 150 to 400 ppm. Pic temporarily corrected by placing a do not use sign on dispenser and states facility will use the 3 compartment quat dispenser. To correct repair quat dispenser to dispense sanitizer at proper concentration. This is a repeated priority violation and a risk control plan must be submitted. ,

(Critical) Used a quat test kit and found quat sanitizer dispenser dispensing sanitizer into the 3 compartment sink below manufacturer's specification of 200 ppm. Used a quat test kit and found the quat dispenser in the mop closet, which is use through out facility to wipe down food contact surfaces dispensing below manufacturer's specification of 200 ppm. Pic corrected by flushing quat dispensers and replacing sanitizer with a multi-range quat sanitizer and having service tech adjust dispenser to dispense at manufacturer's specification of 150 to 400 ppm, violation corrected. Note: this facility normally uses a quat sanitizer with a manufacturer's specification of a range of 150 to 400 ppm.

Observed employees with beards working in the food prep area not wearing beard restraints. To correct provide beard nets and remind employees to wear them.

Found the hood filters soiled with dusty build up and sides of cooking hood soiled with splash on the main kitchen cooking line. Found the gutter and edges of the cooking hood in the sandwich station area soiled with greasy accumulation. Found the toaster in the self service bread and cereal station soiled with food debris. To correct properly clean all noted areas and clean on a schedule to preclude accumulation.

Found the sundae self service cooler with items such as (sundae nut toppings) without labels of the common name of food and ingredients. To correct provide labels.

Found the hood filters on the main kitchen line misaligned and not positioned properly with gaps. To correct properly install hood filters and ensure all spaces are filled.

Found numerous containers stacked wet on the clean utensil storage shelves. To correct allow containers to air dry prior to stacking and storing.

(Critical) Found a bowl/container of marshmallows left on self service hot beverage counter without spoon/utensil and protection. Pic corrected by discarding marshmallows and has been instructed to protect food on display by providing a cover or other protective means and to provide a dispensing utensil. ,

(Critical) Found ready to eat, time/temperature control for safety (tcs) food, salami and pepperoni passed it's 7 day discard datemark. Pic corrected by discarding salami and pepperoni, and has been instructed to monitor and ensure staff are checking dates and discarding items passed it's 7 day datemark. ,

(Critical) Found time/temperature control for safety (tcs) food, shredded cheese 49°F, lettuce 48°F and spinach 44°F in the self service salad bar cooler. Pic corrected by placing cheese in walk in cooler to quick chill back to 41°F as this item was recently placed in salad cooler, and discarded lettuce and spinach. Pic has been instructed not to use this cooler to hold tcs food until repaired/replaced to hold tcs food at 41°F. ,

(Critical) Found recently made ranch dressing cooling covered in plastic containers in the walk in cooler. To correct cool dressing using appropriate measures such as in metal containers, shallow pans or using ice wands and monitor and ensure dressing is cooled from 70°F to 41°F in 4 hours. ,

Found a tray of waffle cones stored on the prep table next to handwashing sink. To correct relocate waffle cones to a more protected location.

Found a row of spray bottles on a rack stored under mopheads of inuse mops. To correct relocate spray bottles away from mopheads.

Found employee personal drink (cup with straw) stored on the bottom corner of the 1 door reach in cooler in the dessert station. To correct store employee drinks in designated areas only.

Found food license mounted to wall in the kitchen out of the view of public. To correct display food license in a conspicuous area for public inspection.

Found the shelf under the beverage station soiled with rusty stains and accumulation. To correct thoroughly clean shelf and keep clean.

Found the wall behind the rice cookers in the stir fry station damaged with scratches and uneven surfaces. To correct repair/replace wall to be durable, smooth, and cleanable.

Found the floor under and around the self service fruit bar soiled with stains. Found the floor (under equipment, edges and corners) in various areas of entire facility soiled with accumulation of debris. To correct thoroughly clean floors and clean on a schedule to preclude accumulation.

Found the self service salad cooler not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F.

(Critical) Found time/temperature control for safety (tcs) food, shredded lettuce 45°F, shredded cheese 45°F in the sandwich area 1 door reach in cooler. Pic temporarily corrected by placing lettuce and cheese in a properly working cooler. Found tcs food, shredded cheese 50°F, in the cold hold well in the sandwich area. Pic temporarily corrected by discarding shredded cheese. Found tcs food, a container of portioned cups of cream cheese 69°F on top of the bagel storage cabinet in the self service area. Pic corrected by discarding cream cheese. Found tcs food bulk bag of chocolate and vanilla shake mixes 46°F in the 1 door glass reach in cooler in the dessert station area. Pic temporarily corrected by discarding shake mixes. Found hot tcs food 2 trays of baked fish 120°F in the hot holding drawers. Pic corrected by having employee re-heat fish to 165°F. Pic states employee may have left fish out at room temperture to long before placing in hot holding unit. Found hot tcs food 2 containers of grilled cheese sandwiches 120°F in the 2 door sectional hot holding unit. Pic corrected by having employee reheat cheese to 165°F and leave in oven to keep hot or at least 135°F. Pic has been reminded to monitor and ensure cold tcs food is held at 41°F or lower and hot tcs food is held at 135°F or above, and not to use cold or hot holding units until repaired to hold tcs food cold at 41°F or lower and hot 135°F or above. Cold hold violation has repeated and a risk control plan has been issued. ,

(Critical) Used a quat test kit and found the quat sanitizer flowing from the sanitizer dispenser in the mop room at about 500 ppm. To correct use test kit and temporarily dilute sanitizing solution to manufacturer specification of 150 to 400 ppm until quat dispenser is repaired and dispensing properly. ,

Found the 1 door glass cooler in the dessert area, 1 door glass reach in cooler and cold hold well in the sandwich area not holding tcs food at 41°F. To correct repair coolers to hold tcs food at 41°F or lower. Found the 2 door sectional hot holding unit not holding hot tcs food at 135°F. To correct repair hot holding unit to hold hot tcs food at 135°F or above.

Found food license not posted in a conspicuous place. To correct post food license in a conspicuous place for public inspection.

Found a container of spoons stored in a mixed, jumbled manner. To correct either store spoons in a container with handles up or store in a container with handles in one direction to minimize contamination,.

Found the floor in the beverage storage room soiled with accumulation of debris. Found the ceiling air vents in the main kitchen soiled with dusty build up. To correct clean floor and ceiling vents.

Observed flies in the pop machine area and numerous dead flies in the cabinets under pop machine in the side dining room. To correct keep areas clean and dry, in good repair, and continue fly control measures.

(Critical) Found time/temperature control for safety (tcs) food, red skin potato salad 45 in a large plastic container stored above the container insert of the cold hold well in the salad station area. Pic temporarily corrected by discarding red skin potato salad, and has been instructed to ensure tcs food is held at 41°F or lower. Note: food stored above a cold hold well may not be at proper temperature as food in cold hold well should be inside the cold hold well. Found tcs food, waffle batter 69°F and cone batter 59°F in the batter dispensers on the table of the dessert area. Pic temporarily corrected by discarding waffle and cone batter. To correct store tcs food at 41°F or use a time control policy. Found tcs food diced tomatoes 48°F. Sliced tomatoes 50°F, and sauce 49°F in the cold hold well in the sandwich grill station. Pic temporarily corrected by discarding tomatoes and sauce, and has been instructed not to use this cooler to hold tcs food until repaired/replaced to hold tcs food at 41°F. ,

Found the areas surrounding the spouts of the bulk milk dispenser soiled with splash and build up. Observed grease on the hood edges on the kitchen cooking areas. To correct clean as soon as possible, and clean on a more frequent schedule.

Found the tiled wall in the milk walk in cooler with damaged tiled wall. Found the wall behind the rice cookers in the stir fry station damaged with scratches and dents. To correct repair walls to be durable, smooth, and easy to clean.

Found a stack of unwrapped coffee filters stored on the counter behind coffee urn. To correct store coffee filters in a more protected manner, such as in original packaging or storage box.

Observed the presence of flies in fly traps on the shelves behind service lines. Observed the presence of ants in the dining room area. To correct keep areas clean and dry, in good repair, and continue pest control measures.

Found the cold hold well in the grill station not holding tcs food at 41°F. To correct repair/replace cold hold well to hold tcs food at 41°F or lower. Found water pooling on the floor and a pan catching drips from the dishwashing machine. To correct find source of leak/drip and repair.

Found containers of oil and vinegar in the self service salad station without labels. To correct provide labels.

Found several containers of food (soy beans, bacon bits and croutons) with spoons on counter top, not under sneeze guards. To correct relocate items to a more protected location, such as under sneeze guards.

Found the floor (under shelves, edges and corners) in the storage rooms soiled with accumulation of debris. To correct clean as soon as possible, and clean on a more frequent schedule.

Found the bulk milk tube of bag of milk in the bulk milk dispenser cut longer than manufacturer's specification. To correct cut bulk milk tube per manufacturers specification, that is cut bulk milk tube to be flush with dispensing chilled head.

Found the tiled wall of the milk walk in cooler chipped, damaged and with missing tile. To correct repair/replace wall in the walk in cooler.

Found slight accumulation in the beverage nozzles of the aquahealth beverage dispenser in the private dining room. To correct follow manufacturers directions and thoroughly clean beverage nozzles.

Found the baking oven hood missing hood filter. To correct replace missing hood filter.

Found the storage rooms filled with items that may no longer be needed for operations and in a disorderly manner. To correct clean and organize storage rooms and remove items no longer needed for operations.

Found the door gaskets of the 6 door sectional cooler soiled with accumulation. To correct clean as soon as possible, and clean on a more frequent schedule.

(Critical) Found time/temperature control for safety (tcs) food, 3 canisters of whipped dessert topping 59°F, in the dessert cold hold well. Pic corrected by discarding whipped dessert toppings, and has been instructed to either hold dessert topping at 41°F or place item on a time control policy. ,

(Critical) Used a quat test kit and found sanitizing solutions originating from the mop sink quat dispenser below manufacturer specification of 200 ppm. To correct call for repair and have dispenser adjusted to dispense at 200 ppm. Pic has posted a sign instructing employee not to use this dispenser but to use the 3 compartment sink dispenser. ,

Found wiping cloths not stored completely submerged under sanitizer through out facility. To correct store wiping cloths completely under sanitizer in sanitizing buckets in entire facility.

Found the light shield above the kitchen prep area missing light shield. To correct replace missing light shield.

Found 2 covered containers of recently made chicken stock cooling in the walk in cooler. Pic corrected by removing covers and place ice wands to rapidly cool chicken stock. Pic has been reminded to cool foods from 135°F to 70°F in 2 hours, and then from 70°F to 41°F in 4 hours.

Found the floor (under equipment, edges and corners) in the kitchen and under the north self service beverage station soiled with accumulation. To correct thoroughly clean floor and clean on a more frequent schedule.

Found the wall and coving in the self service beverage station damaged. To correct repair wall and coving.

Found the drip tray missing from the pop machine in the south self service beverage station. To correct replace missing drip tray. Found the baking oven hood missing a filter. To correct replace missing hood filter.

Health Report - 10/26/2010 (1 critical incident(s))

(Critical) 1) observed an employee not sanitize a probe food thermometer before or after use at the grill station. Provide sanitizer wipes for staff to use at this station. 2) ensure staff are sanitizing the steam kettles after cleaning. Provide a convenient way for staff to sanitize these kettles after cleaning

A pressure vacuum breaker was removed from a water supply serving a flexible hose with a spray nozzle on the end of it at the steam kettle station. The water supply serving this hose is now provided with an a. S. S. E. 1052 atmospheric vacuum breaker. Water supplies serving hoses with down stream shut-offs are not compatible with atmospheric vacuum breakers and must be configured with either a pressure vacuum breaker (a. S. S. E. 1020/1056 device) installed at 7. 5' above the floor or an rpz (a. S. S. E. 1013 device) at any elevation.

Observed some cracked door gaskets on the glass door pass through salad bar area cooler. Replace damaged gaskets as needed to provide a cleanable surface that doesn't collect food debris

Observed a metal stem dial-type food thermometer in the kitchen to be out of calibration (reading 13f too high), and a second thermometer to have a damaged needle. Corrected by discarding these thermometers

The floor in the dish washing room (under the shelving unit with the colanders on it) is in need of cleaning. The floor under the soda dispensing stations and the shelf below the dispensing unit are in need of cleaning at this time

Observed the following items to be in need of cleaning at this time: 1) the fan grates for the cooling system in the dairy walk in cooler; 2) the outside of the red, two door hot box at the grill station; 3) the spill tray for the pepsi soda dispenser is over-due for cleaning

Provide a probe thermometer with a small diameter tip to accurately temp. The center of your chicken, fish and hamburger patties being cooked at the grill station
