Pic corrected by placing pasta and apples in numerous shallow pans without covers and placed them in the walk in cooler for rapid cooling. Pic has been instructed to monitor and ensure employees are properly cooling tcs foods. ,

(Critical) Found the drain lines of the ice machine draining into a floor drain receptacle without air gap. Found the drain line of the deli prep top cooler draining into the floor drain without adequate air gap. To correct provide an air gap(s) of at least 1 inch from the ice machine and cooler drain line(s) and rim of the floor drain receptacle. ,

(Critical) Found hot time/temperature control for safety (tcs) food, hash browns 112°F in the left hot holding unit of the kitchen. Pic corrected by having employee reheat hash browns to 165°F for 15 seconds and adjusted hot holding unit to a hotter setting. ,

This is a repeated violation and a risk control plan (rcp) has been issued. ,

Found the plastic coated holder/guide of the slicer damaged with rusty surfaces. To correct repair/replace holder/guide.

Found debris on the floor surrounding the compactor dumpster. To correct clean floor surrounding dumpster and keep clean.

(Critical) Found time/temperature control for safety (tcs) food, cheese sticks 45°F, sour cream 45 in the 6 door reach in display/walk in cooler. Pic has called for repair and a repair tech is working on cooler. To correct repair/replace cooler to hold cold tcs food at 41°F or lower. Found hot tcs food, containers of mac and cheese 114°F and 130°F in the left hot holding unit. Pic corrected by discarding mac and cheese, and has been instructed not to use this hot holding unit, until repaired/replaced to hold hot tcs food at 135 and above.

(Critical) Found the condensation drain line of the 6 door reach in freezer draining into floor drain without air gap. To correct provide an air gap of at least 1 inch from the drain line and the rim of the floor drain.

Found the light in the baking oven hood not working. To correct repair/replace lights in baking oven hood.

Found the bottom shelf displaying numerous candy bars not at least 6 inches off the floor. To correct raise shelf to be at least 6 inches off the floor.

Found the 6 door reach in/walk in cooler not holding cold tcs food at 41°F or lower. Found the left hot holding unit not holding hot tcs food at 135°F and above. To correct repair cooler to hold cold tcs food at 41°F or lower, and hot holding unit to hold tcs food at 135°F or above.

Found the spaces of the grilling surface soiled with carbon build up. To correct follow manufacturer's directions and clean grill.

Found the handle of the ice scoop laying on top of ice in the ice machine. To correct either store ice scoop in a holder or clean container.

Found ceiling tile missing in the kitchen prep areas and storage areas. To correct replace missing ceiling tile.

(Critical) Used a heat strip and found the dishwashing machine not hot water sanitizing dishes. To correct call for repair, and use the 3 compartment sink to wash, rinse and sanitize dishes. ,

Found covered shallow pans of beans and cheese cooling in the walk in cooler. Pic corrected by loosening the plastic wrap at corners allowing heat to escape from plastic covering. Pic has been reminded that tcs food must cool from 135°F to 70°F in 2 hours and then from 70°F to 41°F in 4 hours, and to cool foods uncovered or loosely covered.

Found the upper spray nozzles of the dishwashing machine clogged with debris. To correct clean upper spray nozzles and clean on a more frequent schedule.

Found no handwashing sign in the cooking/prep area of the cafe area. To correct provide a sign reminding employees to wash their hands.

Found light not working in the cooking hood above the grill. To correct repair/replace light.

Found the floor (under equipment, edges and corners) in the entire facility soiled with accumulation of debris. To correct thoroughly clean floor and keep clean.

Found slight accumulation in some of the pop nozzles of the self service pop machine. To correct follow manufacturer's directions and clean pop nozzles.

Found heavy ice build up on the interior evaporator coil of the 3 door sandwich prep top cooler. Found heavy dust accumulation on the coil of the salad display cooler. Found duct tape holding the interior door panel trim of the beverage walk in cooler door. To correct properly repair all noted equipment and keep equipment in good repair.

Observed the presence of flies in the sandwich prep floor drain area. To correct keep facility clean and dry, in good repair, and continue pest control measures.

Pic corrected by providing proper datemark for chicken. Found various rte, tcs food in other coolers thorough out facility with prepared dates and some with discard dates. Pic corrected by providing 7 day ( day opened/prepared + 6 days) discard datemarks for all improperly marked foods, and has been instructed to monitor and retrain employees on proper datemarking, and to use a consistent system. ,

(Critical) Found an unopened container of creamy caesar pasta salad passed its manufacturer use by date in the meals on wheels walk in cooler. Pic corrected by discarding pasta salad, and has been instructed to monitor that employees are checking dates and discarding items passed it's use by date.

(Critical) Found time/temperature control for safety (tcs) food, a container of fruit salad (watermelon and cantaloupe) 47°F to 49°F in the salad bar buffet cooler. Pic corrected by discarding fruit salad and has been instructed to either use a time control policy or other means to hold fruit salad at 41°F, as other foods in cooler were at 41°F. This is a repeated cold hold violation and a risk control plan for cold holding has been issued. Found tcs food, a container of fish 47°F, beef patties 47°F, and chicken 49°F in the bottom portion of the 1 door reach in cooler by the grill. Found the lights in walk in coolers and walk in freezer not working. To correct repair/replace lights.

Found a case of bananas stored on the floor of the basement storage area floor. Pic corrected by placing bananas on a table.

Found the edges and non food contact surfaces of the meals on wheels sealing machine soiled with build up. Found the popcorn machine in the store areas soiled with food debris and accumulation. To correct clean as soon as possible, and clean on a more frequent schedule.

Found a covered container of boiled eggs cooling in the walk in cooler. Pic corrected by removing cover on eggs, and has been instructed to monitor that employees are cooling foods properly before covering and storing foods.

Found coolers- salad buffet cooler, 1 door reach in cooler, and display cooler not holding tcs food at 41°F or lower. To correct repair/replace coolers to hold tcs food at 41°F or lower. Found the 1 door glass reach in freezer not holding tcs foods in a frozen state, that is chicken was soft and at about 32°F. To correct repair/adjust freezer to hold tcs food frozen at about 0°F. Found the gauge of the hot water sanitizing dishwashing machine not operating properly, that is showing a maximum temperature of about 130°F, when is should show at least 180°F as heat strip did turn, indicating hot water was hot enough to raise surface temperature to at least 160°F. Found water pooling on the floor behind the display cooler. To correct ensure cooler is functioning properly, and repair as needed.

Health Report - 11/04/2010 (6 critical incident(s))

(Critical) Observed numerous small flies (appear to be fruit/vinegar flies) in multiple areas of the back kitchen area (dish washing room, at food prep sink stations and their drains). Ensure the following occurs: 1) staff are frequently monitoring problem areas for presence of these flies or their larvae, 2) the facility is being maintained clean (especially floors and drain pipes) and as dry as possible. Physical scrubbing will be required to remove built-up debris in the drain basins and drain pipes

(Critical) Observed an employee remove a chicken breast from the grill without verifying the cooking temperature. When asked, the employee did not know or could not recall the minimum required temperature for poultry. I checked the internal temperature of the chicken once placed on the bun. It was initially at 150f but rose to 160f after a short period of time. Corrected by returning the chicken to the char-grill for a short time to achieve 165f internal temperature. Ensure your staff are familiar with the required cook off temperatures and that they are using thermometers to verify cook off temperatures of raw animal foods

(Critical) Large walk in cooler: observed cross contamination situations on two of the speed racks. One rack had ground beef stored above fish fillets, and the other rack had both ground turkey above ground beef & whole beef, and ground beef stored above whole beef. Corrected by discussing this with two full time cooks and by rearranging the raw animal products such that fish is above whole beef, whole beef is above ground beef, and ground beef is above chicken and turkey

(Critical) Observed an employee unloading clean equipment from the conveyor dish washing machine with hands that were soiled from loading dirty dishes into the dish washing machine racks. Corrected by discussing this concern with the employee and by the employee washing their hands

(Critical) Meals on wheels cook line area: observed no back-flow preventer installed on the water line for the blue spray hose. Either install 1) a pressure vacuum breaker (a. S. S. E. 1020 or 1056 device at the required elevation), or 2) an rpz unit (a. S. S. E. 1013 device) on the water line preceding the hose connection, or 3) eliminate this hose connection altogether if it is unneeded

(Critical) Three units: 1) the make unit at the grill station--the evaporator coil was iced up causing phfs (cooked vegetables, sliced tomatoes, several cheeses, marinara sauce, etc. ) in the top and bottom of the unit to be at 47 to 60f. These phfs were discarded. 2) the salad cooler--all phfs (deli meat, feta, diced tomatoes, chicken caesar salad, pasta salad, etc. ) were observed to be at 48 to 57f. These foods were discarded. Replacement food for most of these items was re-stocked. After +/-2 hours these replacement foods were checked again. The temperatures had risen again to 51 to 55f. These phfs were discarded and were not restocked. 3) observed pre-wrapped deli meat in shallow plastic pans in the top of the sandwich make line to be layered too high (the top layers were at 45-46f, while the bottom layers were at 40/41f). These foods were recently re-stocked (within the last hour). The top layers of deli meat were removed and relocated to the reach-in portion of the cooler where the back-up are located to chill back down

Observed mold/yeast growth on the meals on wheels walk in cooler fan grates. After turning off the cooling system, remove the grates and thoroughly clean them

1) the grill line two door make unit was only maintaining foods at 47 to 60f. The evaporator coil was iced up. Defrost this unit and repair/adjust as needed to maintain foods at 41f or below. 2) the salad make line refrigerator was only holding foods at 48 to 57f. Although a frost layer was observed along the perimeter rail of this unit a temperature setting adjustment may be needed. Further, the metal pans in use are likely too shallow to maintain proper temperature, and some of the foods were over stocked. Utilize deeper metal pans (and possibly pan covers) to maintain foods at 41f and below

Observed a sanitizer solution for wiping cloths in the grill station area to be too weak through usage during the shift. Corrected by providing a fresh sanitizer solution at 200 ppm

Provide a probe thermometer with a thin tip for checking hamburger patties and other thin foods at the grill station. Strongly consider a digital unit as they are much faster than the analog/dial type

Observed locally made, individually wrapped cookies (two different kinds) that are missing required information on the product labels. Contact the manufacturer and have them work with their mda inspector to revise their labels. The labels must include the following: name of product, business name and address, net weight, ingredient list, food allergens contained in product, and use-by date. Several of these required items are missing currently

The floor drains/sinks throughout the back kitchen area are in need of cleaning now and on a regular basis in the future (at least two times per week to eliminate debris build-up that the small flies are laying eggs in). Clean the floor in the mop closet underneath where the mops are hanging. Provide plastic tubs or containers on the floor under the wet mop heads to collect the dripping water. These tubs will need regular draining and cleaning

Observed a continuous leak on the water supply for the dish station overhead spray nozzle. Repair as needed to eliminate the water leak

Observed only a hose connection atmospheric vacuum breaker installed prior to the hose with a spray gun attached at the end. This type of back flow preventer is not compatible with down-stream shut offs. As a correction either 1) install a pressure vacuum breaker at the required elevation, 2) install an rpz back flow preventer, or 3) install a "sidekick/flow-through" device on this water hose connection

Observed the baker brand open air cooler in the market place to operating at a slightly elevated temperature. Adjust this unit as needed to maintain 41f and below

1) observed the spill tray basin for the market place pepsi dispensing station and the small diameter discharge drain line to be in need of cleaning at this time. 2) observed excessive dust accumulation behind the vented lower front grates on the baker brand open air coolers for the market place. Clean as needed

1) observed a ice transfer pail to be stored upright near the ice making machine. Corrected by inverting this pail on an equipment storage shelf. 2) basement storage area: observed some clam shell food storage containers stored upright. Corrected by inverting these containers

Observed the electric food slicer (clean in place equipment) to not be washed with hot soapy water, nor rinsed prior to sanitizing. Ensure staff are trained to the required washing procedures for food contact surfaces (pre-clean, wash, rinse, sanitize, air dry). The manager indicated (in addition to training) a step by step procedure will be created and posted for staff to refer to when cleaning this item
