(Critical) Found mouse droppings and what appeared to be a gnawed/chewed hole in a sack of popcorn on a dunnage rack in the basement storage area. Pic placed contaminated bag of popcorn in a garbage bag to be discarded, and has been instructed to contact pest control company to place traps and treat areas to control and eliminate mice, as premises shall be free of pest. ,
Main concessions: found the floor under equipment, edges and corners soiled with accumulation of debris. Found the ceiling tiles above the front counter area soiled with splash. Concession #121: found the floor under equipment, edges and corners soiled with accumulation of debris. Basement storage room: found the floor under some shelves soiled with accumulation of debris. Concessions #227 and pizza booth: found the edges of the front counter damaged and missing trim. To correct repair/replace trim to be nonabsorbent, durable and smooth. Pizza booth: found the 1 door reach in cooler not in use or working. To correct repair cooler. Concessions #247: found the paper towel dispenser in the front counter handsink not working properly. To correct repair/replace paper towel dispenser to dispenser paper towels properly. Concessions #242: observed staff moving hot dog roller to another location as fuses were blowing when plugged in power outlet. To correct determine cause of fuse blowing or repair hot dog roller as fuses blowing may indicate a fault in equipment.
Basement storage room: observed mouse droppings under the shelves in the basement storage room floor. To correct clean areas under shelves, keep facility in good repair, eliminate all gaps and holes. And continue pest control measures.
Pizza booth: found the interior bottoms of the 2 door prep top cooler soiled with spills. To correct clean cooler and clean as needed.
Office and storage room: found no thermometers in the freezers. To correct provide internal hanging thermometers.
Basement storage room: found numerous old, and broken equipment in the basement storage room area. To correct determine if equipment is needed for operations, and either repair equipment or remove from facility.
Observed mouse droppings in the basement storage area floor. To correct keep areas clean, in good repair, seal all cracks, holes and gaps, and continue pest control measures.

Pic corrected by having employee reheat cheese to 165°F for 15 seconds. Concession #222: found a new and unopened bag of cheese not heated to 135°F in the hot holding cheese dispenser. Pic corrected by having employee re-heat bag of cheese to 165°F for 15 seconds. Pic has been instructed to monitor that employee are reheating previously used and cooled bags of cheese to 165°F for 15 seconds, and reheat new and unopened bags of cheese to at least 135°F prior to hot holding. ,
(Critical) Concession #236: found opened cases with numerous bags of cocoa and coffee with holes and what appeared to be gnaw marks. Storage room #133°Found a large bag of popcorn with a hole/gnaw mark on bottom of bag with crumbs on top of pallet it was stored on with mouse droppings on floor around pallet. Found bags of cocoa with holes/gnaw marks on lower shelf with mouse droppings below shelf. Pic corrected by discarding damaged packages of cocoa, coffee, popcorn. Pic has been instructed to have staff check packaging of all food products before using and better protect food. Note: observed bags of chips thorough out concessions stands which were several months passed it stamped "sell by date". Please rotate stock and it is recommended that items be used or sold by stamped date. ,

Storage room #204: found a bottle of bleach and glass cleaner stored next to cartons of popcorn. Pic corrected by relocating cleaning chemicals to a location below and away from food and food utensils. Unused concessions: found cleaning chemical spray bottles left on counter tops next to food and food utensils. Pic corrected by placing cleaning chemicals in proper storage area below and and away from food and food utensils. (Critical) Concession #222: found time/temperature control for safety (tcs) hot dogs 114°F to 124°F in the hot dog hot holding unit. Pic corrected by discarding hot dogs, and has been instructed to review and monitor that hot holding procedures are being followed and hot dogs are held at 135°F or above. ,
(Critical) Storage room 133: found mouse dropping on floor of entire room, and on shelves on top of cases of food and utensils. Found what appeared to be gnaw marks and holes on bags of cocoa and popcorn. Concession #247 and #242: found mouse droppings in corner of back room floor. Concession #236: found mouse droppings on floor of back room and in storage bins, and found what appeared to be gnaw marks and holes of bags of cocoa and coffee. To correct keep eliminate all gaps and holes, keep areas clean, and contact pest control company to deal with mice. All areas should be cleaned as soon as possible, and ensure food is protected. I will return for a follow up inspection, to verify areas are cleaned, food is protected, and that mice are under control. ,
Concession #212 and #242: found ceiling vent covers missing from ceiling above prep area. To correct replace missing ceiling vent covers.
Found no thermometer in the 1 door ice freezer. To correct provide thermometer.
Found the upper tray of the hot holding cheese dispenser filled with plastic portion cups of cheese. To correct discontinue this practice of placing plastic portion cups of cheese in hot holding unit or provide written documentation from manufacturer that portion cups can be left in hot holding unit.
Found the floor in the entire facility (under equipment, edges and corners) soiled with mouse droppings. Found the ceiling air vents in numerous concessions soiled with heavy dust build up. To correct clean air vents, and thoroughly clean floors.
Several concession room handsinks: found soap in dispenser, but no soap coming out of dispenser. Pic corrected by cleaning out soap dispenser nozzles to provide soap.
Found holes/gaps along the edge of the rolling door in the basement of the garbage storage area. To correct eliminate gaps/holes along rolling door and floor.
Concession #212: used an ice bath and found 2 of 3 thermometers not accurate. To correct follow manufacturers directions and calibrate thermometers.
Concession #236 and all public restrooms: found no handwashing sign in handsink areas. To correct provide and post handwashing signs reminding employees to wash hands at all locations where employees may wash their hands.
Concession #212: found the trays in the hot holding units soiled with accumulation and build up. To correct clean trays as soon as possible, and clean on a schedule to preclude accumulation.
Concession #222: found the door of the pizza hot holding unit not staying shut and held shut by a box of single use gloves. To correct repair door to stay shut without the use of a box of gloves. Found the 2 door sectional reach in cooler not in use or being use to cold hold food. To correct repair cooler.
(Critical) Booth 242: observed an open cup of coffee on the back prep table. Corrected by discarding
Main booth: please clean the interior of the cappuccino machine prior to your next event
Booth 234: the interior of the pizza prep cooler needs cleaning
Booth 236: the right-hand pepsi cooler needs an air thermometer.
(Critical) Air gaps are not provided on all of the ice bin drain lines (pepsi installed all new equipment last fall). Provide 1. 5" air gaps on all ice bin drain lines in the booths

A few of the seldom used booths do not have trash cans at the back hand washing sinks
Observed some old, spilled food (pizza sauce and pepperoni, etc. ) in the pizza booth (#234).
Replace the internal components of the watts 9-d back flow preventer on the water line to the main booth cappuccino machine--the current one is leaking
Most of the new pepsi coolers are lacking air thermometers
Observed several of the food thermometers to be out of calibration. Corrected by recalibrating some of them and by removing others from use
Provide bulb tube end caps for the light bulb in the main concession pepsi cooler
After cleaning, please seal the cracks in the bottom of the delfield cooler in booth 222
