Straws and drink lids stored next to shake machine found to have splash from shake machine use. Single service use items need to be stored in a clean, dry location where they are not exposed to splash and contamination. To correct, relocate straws and drink lids to a protected location or provide a splash guard in between the shake machine and single-service items.

(Critical) The underside/splash guard of the shake mixer is in need of cleaning. Clean now and retrain the employees on proper technique. Corrected at time of inspection by cleaning.

The oven in the rear where bacon and mushrooms are cooked are in need of cleaning. Clean now and place on a more frequent cleaning schedule

(Critical) Cooked onions and mushrooms are held on side of griddle after cooking. Onions were found at 145 degrees f but mushrooms were at 94 degrees f. Pic immediately indicated time control is in use, but the policy is not available and the food did not have a discard time on it. Provide written policy and use discard time sticker on the containers for onions and mushrooms if time control is to continue. If not a minimum temperature of 135 degrees f must be maintained for the mushrooms and onions. ,
