(Critical) I observed 3 dirty knives put in the knife rack in the rear prep area. Staff must be thoroughly cleaning and sanitizing knives before storing in knife racks. Corrected by clean and sanitizing the knives and the knife holder.

(Critical) The refried beans were found at 123*f to 138*f and they were recently re-heated. Re-heated items must be heated to >165*f before storing on steam table, where the hot hold temp must be >135*f. Staff must be thoroughly re-heating items to above 165*f and be using thermometer to verify the temperatures. Corrected by further heating to >165*f.

(Critical) The can opener blade was observed dirty. This must be cleaned and sanitized after uses. Adjust the holder so the unit is easily removable for cleaning. Corrected by running cleaning and sanitizing.

(Critical) Detected sanitizer level below 50ppm on the dish machine. Must keep at 50-100ppm. Do not use until serviced and confirmed levels are between 50-100ppm. I will be back in 10 days to verify this.

(Critical) I found a can of wd-40 and stainless steel cleaner on a top shelf above clean pots in the dishwash room and a bottle of rubbing alcohol on a shelf above the main line prep table. Keep toxic items in a safe location away from foods and food contact items. Corrected by storing these items in chemical storage room. Managers need to monitor for proper storage of toxic items.

The interior of both microwaves was observed dirty, staff must be keeping them cleaned out throughout the day.

Replace the damaged gaskets on the main line reach in cooler.

The concrete floors on the main cook line and in the back rooms are chipped, cracked and unsealed in many areas. Within 6 months, have the floors re-surfaced and properly sealed.

Do not use cloth towels as cutting board stablizers. Obtain approved board stablizers (such as board mates. )

The handwash sink at the bar was observed dirty. I saw a putty scraper, beer caps and a lime wedge in the sink basin. Keep handwash sinks clean and do not use for anything except handwashing. Corrected

Health Report - 08/24/2010 (3 critical incident(s))

(Critical) There was a container of sour cream and shredded lettuce on the same shelf as the raw meats. Store the ready to eat foods above the raw meats. Corrected by doing such.

(Critical) There isn't air gaps on the drain lines out of the ice machine and at the server line ice bin. You need to have these lines permanently air gapped so the lines finish at least 1 inch above the top of the floor drains. I will be back in 2 weeks to verify this has been permanently corrected.

(Critical) I observed that the ice scoops were in contact with dirty water in the ice scoop holders in the back and at the server area. Ice scoop holders need to be run through the dishwasher every 24 hours. Corrected by running ice scoops and holders through dishwasher.

There are several areas where standing water was observed on the floors. The dishroom has a drainpipe that needs to be extended to be over the floor drain so it doesn't drain onto the floor. At the bar, the floor has depressions where standing water collects. Need to repair so water drains to the floor drains.

Several cutting boards have excessively grooved surfaces. Sand out the grooves so they are again easily cleanable.

You need to clean the area around and under the grease barrel and the grease bin itself.

Replace the burned out lights in the walk in freezer, the hood lights and over the main prep line.

There is condensation leaking from the compressor in the walk in freezer and in the make line reach in cooler on the left side. Have serviced asap!

(Critical) First aid products were found stored above one of the prep tables in the kitchen. Store toxic items in a safe location away from foods or food contact surfaces/areas. Corrected by storing in chemical room.

The following areas are in need of repair: the toilet seat in the employee restroom, the coving tile and wall in the employee restroom and by the back door, the leak on the pressure relief valve at the hot water tank, the plugged floor drain near the hot water tank and the auto urinal flush does not work.

Chlorine test papers are not being used regularly. Train staff on how to use them and be using them on a daily basis.

You need a light shield on the light in the walk in freezer or a shatterproof light bulb.

Be sure to clean the storage shelves that are above the rear prep tables and clean the ice scoop holders on a regular basis.

There are several areas where the light bulbs are not working. Replace burned out bulbs and have all light fixtures working.

You need to re-plumb the mop sink faucet and install a new vacuum breaker. Have this done within 2 weeks and fax over a copy of the work order. Also, take a picture of the repair and e-mail it to me by march 3, 2010
