(Critical) Found several dented and rusty cans on the ready to use storage room shelf. Pic corrected by discarding some dented cans, and placed the rest of the dented cans in the dented can section for return to supplier. Pic has been instructed to review with staff to check for dented cans when stocking, and before use.

(Critical) Found recently made sushi rice and sushi rolls on display for self service not identified to indicate that it is 4 hours past the point in time when the food is removed from temperature control. Pic corrected by having employee mark proper time on the time control log sheet for sushi rice/sushi rolls.

(Critical) Used a test kit and found the chlorine sanitizer in the sanitizing bucket in the cooking/prep area in the kitchen with low chlorine concentration. Used a test kit and found the chlorine sanitizer in the sanitizing buckets in the sushi bar area with low chlorine concentration. Pic corrected by adjusting chlorine sanitizing solutions to manufacturers specification (50 ppm minimum and 200 ppm maximum). Pic has been instructed to monitor and verify that staff are properly mixing sanitizing solutions.

Found the shelves of the cabinets in the wait station areas water stained and worn. To correct repair/replace shelves.

Found slight accumulation in the pop nozzles of the pop machines in the wait stations. To correct throughly clean pop nozzles, and clean as needed.

Found numerous cases of food on the floor of the walk in freezer. To correct store food on shelf at least 6 inches off the floor.

Found the floor (under equipment, edges, and corners) soiled with build up. To correct throughly clean floors and clean on a more frequent schedule.

Found the edges of the cooking hood soiled with grease. Found the door gaskets and interior surfaces of cooler soiled. Found the floor of the walk in coolers and walk in freezer soiled with accumulation of debris. To correct clean as soon as possible, and clean on a more frequent schedule.

Found the "meat" walk in cooler condensation drain leaking water , and a pan catching drips. To correct properly repair drain line of the walk in cooler.

(Critical) Observed dishwasher wash and rinse large bowls and colanders without sanitizing them. Observed a food employee wash and rinse pizza cutter, and various other kitchen utensils, without sanitizing items. Pic corrected by taking all noted utensils to the 3 compartment sink to pre-washed and then placed in the dishwashing machine. Pic has been instructed to ensure that employees are washing, rinsing, and sanitizing food utensils. , observed dishwasher wash and rinse large bowls and colanders without sanitizing them. Found the edges of the hood and areas around cooking hood in the dining area grill soiled with grease. Found the bottom of the cabinets below the pop machines in the wait stations soiled. To correct set up cleaning schedule, clean all noted items, and keep areas clean.

Found wiping cloths stored on top of counters of prep top coolers in the sushi bar area. To correct store wiping cloths submerged under sanitizer in the sanitizing buckets.

Observed the interior of the dishwashing machine with some lime build up. To correct de-lime dishwashing machine, and de-lime on a regular schedule to preclude accumulation.

Found the drain line of the ice cream dipper well loose and no longer secured. To correct re-secure drain line and ensure air gap is maintained. Found the handsink in the kitchen by the dishwashing/prep area slow draining. To correct clear drain line of handsink and ensure it drains properly.

Found a plastic covered piece of plywood between dishwashing drain board and prep sink. To correct replace plastic covered plywood with a material that is durable, smooth and easy to clean. Found plastic grocery bags used to hold/store food items in the chest freezer. To correct discontinue using grocery bags to hold food and replace with food grade storage bags.

Found plastic containers of salt and sugar on the counter of the prep area without labels. To correct provide labels.

Found the floors (under equipment, edges and corners) in the entire facility soiled with accumulation of debris. To correct set up cleaning schedule, and ensure facility is cleaned on a regular schedule.

Found the tiled floor in the dishwashing and prep area in poor condition. To correct repair/replace tiled floor in noted areas.

Found the handsinks- in the dishwashing/prep area, employee restroom, and waitstation, blocked with various items stored in front of handsink. To correct re-locate items so handsinks are easily accessible at all times.

Found customers able to self serve themselves hand packed ice cream. Although pic has provided a dipper well with running water, management must do more to prevent customers from self servicing themselves ice cream. To correct re-post sign stating customers may not self service themselves, re-locate bowls, and have employees serve hand packed ice cream to customers.

Found facility using grocery bags to store various meats. To correct replace grocery bags with food grade storage bags.

Found water dripping from the 2 door prep top cooler in to pans in the sushi bar area. To correct properly repair/maintain cooler. Found the floor mixer in a soiled condition, but pic states is broken and not in use. To correct clean mixer and have mixer repaired.

Found several plastic bowls laying in bulk food bins. To correct replace plastic bowl with scoops with handles, and store scoop with handles in an up position.

Found the lid of dumpster in an open position. To correct keep lid closed.
