Additionally, a food portion scoop, several sheet trays and multiple donut filling hopper dispensing parts were observed unclean (apparently the multiple pieces were not disassembled when they were washed). These items were re-cleaned

(Critical) Three of the four hand washing stations were out of paper towel. No back-up paper towels were available in the facility. An employee was sent to a nearby store and picked up paper towel for each station.

(Critical) Again, the interior surfaces of the ice making machine in the back kitchen area were found to be unclean. A lot of black slime was noted in the ice chute area, and some in the ice production area. Schedule a service call for this ice machine with an ice machine service company to disassemble this machine fully and to wash, rinse, and sanitize this unit.

(Critical) Observed two containers of sugar stored on the bottom shelf in the mop area among chemicals. Corrected by relocating the two food containers to the shelf above the chemicals

Upon arrival, two open cup employee beverages were observed. One at the end of the front counter and the other near the hand washing sink at the drive through station hand sink. Corrected by discarding

The floor sink under the ice making machine and the tile around the floor sink are over due for cleaning

1) the blue plastic ice transfer pail is stored upright. Please keep this pail inverted on a clean and sanitized surface to allow moisture to drain out and to prevent debris accumulation. 2) observed two stacks of small, white square portion trays to be stored upright on a counter. The top trays were splattered with something. Corrected by discarding the two top trays and relocating the two stacks of trays to the back prep area

As of last week the manager with the food safety certification no longer works here. Provide another full time manager with the food safety certification

(Critical) The interior surfaces of the ice machine are in need of cleaning and sanitizing to remove pink, brown and black slime. ,

(Critical) An improper back-flow prevention device is installed on the mop sink faucet (two atmospheric vacuum breakers with down-stream shut-off valves). Install a "wasting tee" directly to the faucet threads. The hose with the spray gun may be connected to the threaded side connection of the wasting tee.

The ice transfer pail was found to be stored upright. Provide a clean and sanitized surface to invert this pail on during storage (or hang it up-side-down from a bracket)

(Critical) Observed most interior surfaces of the ice machine to be unclean. Contract with a refrigeration/ice machine cleaning company to disassemble and thoroughly clean and sanitize this machine. Provide me with a copy of the receipt for the work performed

(Critical) The mop sink faucet (containing a built-in atmospheric vacuum breaker) has connected to it a flexible hose with a spray gun at the end. This arrangement is not compatible for the atmospheric vacuum breaker. Install a wasting/bleed-off tee to the end of the mop sink spout and connect the flexible hose to the threaded side outlet on the tee to supply water to the hose & spray gun. The water will need to be turned off once the chore using the water hose is completed

Observed the white plastic ice scoop at the ice machine to be cracked and there were several pieces missing. Discard this scoop and provide a replacement one (consider purchasing an aluminum one to prevent the cracking issue)

Observed the floor sink underneath the ice machine ice bin and pipes draining into said floor sink to be covered in mold/slime and debris build-up. Thoroughly clean and sanitize these surfaces now and at least 2x per week in the future to maintain it clean/eliminate a possible pest breeding area

Observed the ice transfer pail to be hung upright by the handle. Provide a clean and sanitized surface to invert this pail on during storage to allow for moisture to drain out and to prevent misc. Debris from falling into this container

(Critical) Observed an unlabeled spray bottle with an unknown liquid in it stored at the back kitchen mop sink. Corrected by discarding this chemical

(Critical) Observed a spray can of raid ant and roach spray labeled for household use only to be located in the facility. Corrected by removing from this facility

(Critical) Observed black goodyear rubber hoses (no stamp indicating this hose meets potable water standards) connected to the potable water supply at the three compartment sink and mop sink areas. These hoses provide water to the chemical dispensers. No back-flow preventers are provided upstream from these hoses. As a simple/inexpensive means of correction remove these unapproved hoses and install a potable water hose bearing nsf or equivalent approval for potable water applications

Observed missing plastic on the bottom of the ice transfer pail. Replace this pail with a new one. Consider a new style that is made of a thicker more flexible plastic

Provide a replacement atmospheric vacuum breaker for the damaged one on the mop sink faucet

Observed two brooms and a dust pan stored between the main back kitchen hand washing sink and a wire storage unit used for clean equipment. Relocate these items to your designated storage area for these maintenance items

Observed the ice transfer pail to be stored upright. Store this pail inverted to protect the interior surfaces from misc. Contamination and to allow moisture to drain out

Observed the potential for contamination of the ice when being dispensed. An employee used a plastic cup to scoop the ice instead of the designated ice scoop. This issue was discussed with the employee

Observed the floor sink located under the ice making machine to be long over-due for cleaning. Thoroughly clean this basin now and a couple times per week in the future to maintain it clean

A light bulb is missing a cover in the middle donut display rack (second bulb from the bottom). Provide a replacement cover

Observed the ice machine ice scoop stored on the top of the ice holding bin. Provide a clean container to store your scoop in when it is not being used

The interior surfaces of the ice making machine (including the ice chute area) are in need of cleaning at this time and more frequently in the future. Observed a couple donut filling spouts to be unclean on the interior surfaces. Corrected by cleaning

Mop sink area: observed several shut-off valves down stream from the built-in atmospheric vacuum breaker. This type of back-flow preventer is not compatible with any down stream shut-off valves that can leave the vacuum breaker under pressurized conditions. As a simple correction have the company who services your chemical dispensing systems install a bleed-off tee between the chrome mop sink faucet and the existing "y" fitting

Observed the floor in the following areas to be unclean: under base cabinets at the drive through station (especially at the junction to the wall), and the floor sink located under the ice machine by the back delivery door. Clean these areas now and more frequently in the future to maintain them clean

Observed a couple bags of food stored in a base cabinet storage compartment below a drain pipe for a spill tray at the front counter beverage station. Corrected by relocating the food to another area. Ensure no foods are stored in compartments with waste water drain lines located above them

Cooler #3: the thermometer inside this unit was inaccurate due to the interior tube being detached from the scaled gauge. Corrected by providing a replacement thermometer

1) replace the internal components for the atmospheric vacuum breaker on the mop sink faucet that is currently leaking. 2) extend the drain pipe for the back-flow preventer of the ice machine water line to be situated above the floor sink (must maintain the required air gap). This way any water leaking out of this unit will be directed to waste and not on the floor.

1) observed a pair of food tongs resting in the drive through area hand washing sink. Corrected by sending the tongs to the dish washing sink. Inform all staff to not place anything in the hand washing sink basins. 2) observed a large square trash can stored directly in front of the hand washing sink in the back food prep area. Corrected by relocating the trash can

Found the back kitchen area hand washing sink with utensils placed in the sink and a large waste basket placed in front of the sink limiting access for employee hand washing at this sink. To correct hand washing sink is to be used for hand washing only and relocate waste basket or use a smaller waste basket at the back kitchen area. This is to encourage proper employee hand washing at the back kitchen area hand washing sink as required

Found the restroom exhaust fan vent at the ceiling with dust, should schedule more frequent cleaning of the vent at the ceiling of the restrooms

Found the thermometer located at the front counter reach-in refrigerator with loose fitting temperature guage off scale preventing proper operation of the thermometer. To correct should replace the thermometer with one that is in good repair to be easily readable

(Critical) Observed mop sink with a "y" type connection device downstream of the built in vacuum breaker at the faucet to the mop sink. To correct install an atmospheric hose bibb type vacuum breaker a. S. S. E. 1011 device down stream of the shut off valve on the "y" type connection device at the mop sink faucet, found shut off valve on the terminal end of the hose beside mop sink. To correct should install a hose bibb type vacuum breaker at hose connections remove terminal shut off valve from the end of the hose and replace nozzle with a straight nozzle without a shut off mechanism. The reason is when the faucet water valve is turned to the on position water pressure will be able to drain out of the hose. If continuous water line pressure remains on an atmospheric vacuum breaker it causes the vacuum breaker to fail to properly protect the water supply. , observed mop sink with a "y" type connection device downstream of the built in vacuum breaker at the faucet to the mop sink. (Critical) Sanitizer concentration at improper level in sanitizer compartment of manual warewashing operation. Critical corrected: at time of inspection emptied sanitizer compartment and set up sanitizer compartment at proper quaternary ammonium concentration, 300 ppm. Facility has proper quat test kit to monitor. Called kay chemical at time of inspection to verify quat range; per kay chemical range is 100 pmm - 400 ppm

These areas in need of cleaning: floors and floor/wall juncture under coffee station near drive-thru window, floors under ice machine, floor/wall juncture under shelving units rear kitchen and hot/cold control knobs rear handwash sink. Please clean now and place on routine cleaning schedule

Facility not using quat test kit to monitor sanitizer concentration. Please use quat test kit each time sink set up to verify proper sanitizer concentration

Please keep dumpster lids in closed position. Contact waste hauler after unloading dumpster, lid should be in closed position not wedged between rear wall and dumpster to prevent closing
