Found a bucket of forks stored above the 3 compartment sink with tines up. To correct - store all utensils with handles up to avoid touching food surface areas with hands - possible contamination

Found stove door with an excessive accumulation of cooked on grease probably from the grease cup from the flat top overflowing many times. To correct - clean now and keep on a more frequent cleaning schedule

Found the men's restroom to be without the required handwashing sign. To correct - provide signage in the men's room that requires handwashing prior to returning to work

Found pop syrup boxes, cooking oil, chip boxes and single service items stored directly on the floor. To correct - store all food items and single service items at least 6" above the floor

The ceiling in the dry storage room appears to have a moisture problem as is evidenced by mold on the high wall area in the corner. To correct - perform needed repairs on roof. If leak has been corrected, clean the mold staining and re- paint as needed.

Establishment does not have an anti-choking poster displayed. To correct -post provided poster for anti-chiking directions

(Critical) Observed employee prepping and handling rte hot dog buns, deli meat, and vegetables with bare hands. All food employees shall not handle rte foods with bare hands and shall utilize proper utensils and plastic gloves at all times. Corrected by educating staff on bare hand contact, rte foods and foodborne illness risk factors. Critical corrected.

(Critical) Observed employee not sanitizing dishware and utensils after washing in 3 compt sink. Employee not aware of sanitizing requirement. All dishware and utensils in a commercial kitchen must go through a 3 step cleaning process (wash, rinse and sanitize). Will need to rewash and sanitize all dishware and utensils. Temporarily corrected violation by educating staff on proper dishwashing procedures and proper sink setup. I will check this item at follow up inspection to ensure compliance. ,

(Critical) Observed raw chicken, rte deli meats (ham, turkey), cheese, hot dogs, sausage, and housemade coleslaw in ric in prep area with an internal food temp of 60-66f (cooler is not functioning correctly). All phf must be cold held at 41f or below at all times. Do not use this unit to store phf until properly repaired and then inspected by the wash. County health dept. Pic temporarily corrected violation by discarding all above mentioned food items and calling for repair at time of inspection. I will check this item at follow up inspection. Correct within 10 days. ,

Please properly rewash and sanitize all dishware and utensils as needed. Corrected by educating employees on proper sanitizing procedures and demonstrating proper 3 compt sink set up including proper test strip use

(Critical) Observed pan of hot dogs and several cooked kielbasa being hot held in oven at 110-115f (oven turned down too low). All phf rte foods must be hot held at 135f or more at all times. Observed several cases of pop, beer, and water stored on floor in back storage area. All food and beverages must be stored in a protected manner and stored at least 6 inches off of floor at all times

Observed several uncovered 5 gallon buckets of used grease in back storage area (near back door). Facility does have a grease dumpster, but appears employees not taking out used grease on frequent schedule. Used grease cannot be stored in facility for long periods of time as they attract insects and can create an odor nuisance. To correct, please take all used grease to grease dumpster daily (or immediately after fryers are emptied)

Observed no paper towels at handsink in dishwashing area (dispenser is jammed/broken). All handsinks must be supplied with soap and paper towels at all times for proper handwashing

Observed chlorine level in wiping cloth bucket in bar sink under front counter under 25ppm of chlorine. All wiping cloths must be completely submerged in a concentration of 50-100ppm of chlorine at all times

Observed ric door (for kegs) in back kitchen area not able to shut properly (empty keg currently propped up against door to keep shut). To correct, please repair or replace ric door as soon as possible

Observed no chlorine test strips in facility to measure bleach concentration (facility did have quat test strips, but they do not use quat sanitizer). To correct, please obtain proper chlorine test strips as soon as possible
