(Critical) Found ready to eat (rte), time/temperature control for safety (tcs) food, a container of hummus without 7 day (day made + 6 days) in the 2 door sectional reach in cooler. Found rte, tcs food, sliced turkey in the 2 door prep top cooler without 7 day datemark. Found rte, tcs food, wrapped slices of cheese cake without 7 day datemark in the dessert cooler. Pic corrected by providing 7 day datemark for all noted items, and has been instructed to monitor and ensure appropriate items are date marked.
Observed an employee working in cooking/prep area not wearing hat/hair restraint. To correct provide hat/hair restraint and remind food employees to wear them.
Found the shelves in prep areas and coolers lined with an absorbent type liner. To correct replace absorbent type liner with a nonabsorbent type liner.

Found the tiled floor in the back kitchen area worn to bare wood. Found the tiled floor and parts of tiled wall in the restroom with damaged and missing tile. Found the ceiling in the back kitchen area missing tile. To correct repair/replace tiled floor, wall, and ceiling in noted areas.
Found no hot water flowing from the handsink faucet in the unisex restroom. Pic states repairman must have shutoff hot water when recently repairing sink and did not turn on hot water when repair completed. Pic corrected by turning on hot water valve, providing hot water and was reminded to provide hot water at handsinks at all times.
Found the lights in the kitchen back prep area not working. To correct repair/replace lights.
Observed wood/particle board used as shelves under the front counter. To correct either seal wood to be nonabsorbent or replace with a nonabsorbing surface.

Found facility without certified manager. To correct provide certified manager as soon as possible. Note: this is the third administrative conference notice issued for not having a certified manager, as facility has stated and provided written statement indicating registering for a food safety class, but has failed to provide a certified manager.
Found the handsink in the cooking prep area and the restroom without handwashing sign. To correct provide sign reminding employees to wash hands at all locations where they may wash their hands.
Found the door gaskets of coolers torn and loose. Found the back kitchen prep area cooler with heavy ice and frost build up. Found sink stoppers in the 3 compartment sink not holding water in compartments. Found the handsink counter top in the front counter area cracked with deep grooves. Found no thermometer available to take food temperatures. To correct provide thermometer, such as a metal stemmed thermometer with a range of 0 to 220°F, to measure food temperatures.

(Critical) Found the critical level of the fill valve in the toilet tank not 1 inch above the overflow pipe. To correct repair/adjust fill valve so the critical level is at least 1 inch above the overfloor pipe. ,

Found the drain line of the front counter ice drain draining into the floor drain receptacle without air gap. To correct provide an air gap from the drain line of the cooler and ice bin and the rim of the floor drain receptacle. ,
(Critical) Found time/temperature control for safety (tcs) food, a package of packets of cream cheese 54°F in the 1 door reach in cooler. Pic temporarily corrected by discarding cream cheese packets, and has turned the cooler to a colder setting. To correct repair/replace cooler to hold tcs food at 41°F. ,
Found front and back prep area handsinks without handwashing signs. To correct provide signs reminding employees to wash their hands at all locations where employees may wash their hands.
Observed some grease on the cooking hood edges. To correct clean cooking hood edges on a more frequent schedule.
Found employee personal items stored on the middle shelf of the 2 door sliding glass reach in cooler. Pic corrected by relocating items to a location below food in designated storage area.

Found the floor in the back kitchen prep area in poor condition (damaged and missing floor tile). Found water stained ceiling tiles and missing tile through out facility. Found ceiling vent covers missing from ducts. To correct properly repair ceiling and floors, and replace missing duct air covers.
Found no thermometer in the 1 door reach in cooler. To correct provide thermometer.
(Critical) Observed two, five gallon containers of sour cream and feta cheese setting on the kitchen floor upon arrival. These foods need to be maintained at 41f or below, and must be put away in coolers when delivered. Both of these foods were found to be at 57 to 60f. Corrected by denaturing them with a blue dish soap and set by the back door for disposal
(Critical) Observed the cook cracking several raw eggs with bare hands and then handle rte food. Corrected by discussing this concern with the employee and by the employee washing her hands in the future after handling raw animal foods
(Critical) 1) observed three bleach bottles in the facility (two of which are used for wiping cloths and dish sanitizing) that are not approved for this purpose (they only have directions for laundry and bathroom cleaning). Remove and replace with a chlorine bleach that has an epa registration, and has proper usage directions for restaurant sanitizing. 2) observed a spray can of hot shot bug killer in the back kitchen that is only approved for use in home kitchens. Corrected by removing this insecticide from the facility
Reattach/repair the door closer on the back delivery door so it functions as intended
Observed a tall trash can stored in front of the kitchen hand washing sink. Please either relocate this trash can to another nearby suitable location or replace it with a small one that can fit under the hand sink
The floor in the back of the facility under the three compartment sink and the floor sink drain for the dish sink are overdue for cleaning. Please clean these areas now and more frequently in the future
Observed the chlorine level to be too weak in the wiping cloth bucket at the front counter area. Please provide a solution of sanitizer with a concentration of 50-200 ppm to prevent bacterial growth on the wiping cloths between uses
Quantities of old, damaged items and equipment are still present in the basement. Recycle, sell or discard these items to eliminate the clutter in the basement
Drink station: observed the handle of the plastic ice scoop to be laying in the ice inside the ice bin. Please store the ice scoop in a clean cup or container on the counter
Observed the kitchen hand washing sink to be draining slowly. Clear the drain line blockage to provide a properly draining sink basin
Observed a large number of cracked/damaged linoleum floor tiles in the kitchen and dish washing areas. Repair the floor and replace these tiles as needed
This facility still does not have a certified food safety manager. This citation was on the last routine inspection report and was required to be corrected by 8-24-2010. An administrative conference notice was provided with this report. Contact me to schedule this meeting asap
