These in-use toppings for pizzas may not also be used for salads because employees have already had bare hand contact with these toppings while making pizzas. Corrected by discussing this issue with the manager and by using the back-up containers of toppings with gloved hands to prepare salads
Observed the wet mop head hung up along a wall near the mop sink such that moisture is dripping onto the wall and floor. Provide a tub under this wet mop head to collect any dripping water. This tub will need regular draining and cleaning
Ensure staff are thoroughly cleaning the three compartment sink and drain boards on a daily basis. Observed some old stuck-on oil residue in the corners of the middle and sanitizing basin, and some small debris on the clean dish drain board. Clean and sanitize these areas prior to washing dishes next
Repair or replace the door sweep on the back delivery door to eliminate the gaps that can allow pest entry
Observed ants on the floor and drain under the three compartment sink. Ensure the facility is kept clean to prevent the attraction of pests and that employees are performing frequent inspections to identify pest concerns promptly.
Observed some food crumbs and some splattered pizza sauce on the floor sink basin under the three compartment sink
(Critical) Observed two small plastic containers of philly steak in the pizza make line cooler (one in the top and one in the bottom) to be missing use-by dates. The manager provided date stickers for these foods based upon when the original packages were opened
(Critical) Staff indicated the removable pizza oven conveyor trays and exhaust hood filter sections are cleaned outside the building using a spray hose and a degreaser chemical. This activity must cease immediately. This wastewater must be routed to the city's sanitary sewer for treatment via your sinks inside your building.
(Critical) Observed the walk in cooler condensation drain line to be re-plumbed without the required air gap. Provide a vertical air gap above the flood rim of the mop sink basin that is 2x the pipe diameter. Please have the owner contact me regarding this item prior to this work being conducted
Observed some personal food from an employee stored on one of the top shelves in the walk in cooler. To protect your customer's food all employee food/beverages must be stored in a container on the bottom shelf away from customer's food. This food was discarded
Observed multiple employee coats hung over the shelf corner post that is used to store single service items such as plates, bowls and cups. Corrected by relocating these coats to the coat rack in the employee restroom
The same cracked/damaged light fixture cover is present in the walk in cooler. Either 1) provide a replacement light cover; 2) remove this damaged cover and provide shatter-proof bulbs; or 3) provide bulb tubes with end caps to prevent glass shards from contaminating foods/equipment should a light bulb break. I will be conducting a follow-up in inspection to verify this issue is corrected

Observed several foods stored on the tile floors (two partial cases of pizza sauce in the dish washing/storage room, two blue dough trays with pizza cheese in them under the inlet conveyor to the oven, and a full sack of corn meal/flour in a blue dough tray under the pizza oven). All foods must be stored elevated off of the floor at least 6 inches for protection. Corrected by elevating these foods at least 6" off of the floor
1) again, the drain line (corrugated, semi-flexible hose) for the walk in cooler has slumped down into the mop sink. Re-plumb this drain line with rigid pvc piping and re-route it terminate with a vertical air gap that is 2 x the discharge pipe diameter above the open floor sink (serves the three compartment washing sink). 2) also, the trap for the hand washing sink drain located in the dish washing room is partially blocked--water drains out of the basin slowly. Remove trap and unclog as needed
Observed a wet wiping towel stored on the pizza cutting table. Wet wiping towels need to be stored in a container of sanitizer when not being used to prevent bacterial growth.
Observed no ceiling light fixture cover on the light located near the bathroom, back hallway storage and entrance to the pizza oven. Also, the ceiling light fixture cover in the walk-in cooler is cracked with pieces missing. Provide light covers for these two units
Re-establish the drain line air gap (consider using a rigid plastic pipe that will maintain its shape instead of the current corrugated variety that gravity has pulled down over time) for the walk-in cooler drain line. The pipe must terminate in a air gap (vertical gap height = 2x the discharge pipe diameter) above the flood rim of the mop sink basin
Observed the metal holder for the rolls of date labels to be overdue for cleaning. Please clean now and maintain clean
(Critical) Observed the quat sanitizer concentration in the sanitizing basin of the three compartment sink to be too high (500 ppm++). Corrected by diluting this solution to 200 ppm as per the label directions. Ensure staff verify the concentration of each batch prepared
Observed spilled food in the bottom, reach-in portion of the pizza prep cooler (right end) that has become moldy. Thoroughly clean and sanitize the interior of this unit asap (once foods are removed) and more frequently in the future. The pizza oven exhaust hood filters (four total) are in need of cleaning to remove oil/grease and dust accumulation
Observed two aluminum foil catering food pans stored upright on the top shelf in the dish washing room. Either store these containers inverted or in a bag for protection of miscellaneous debris accumulation
Ensure staff verify the sanitizer concentration (200 ppm) with the provided quat test papers
Repair/replace the detached wall base coving under the dish washing sink
Observed moisture dripping on the tile floor from a recently used mop head. Store you wet mop heads so they are hung above the mop sink allowing the moisture to drain away. Provide a mop handle hook on the wall above the mop faucet
Clean the floor under the desk and under the clean equipment and chemical storage shelf
(Critical) Quaternary ammonia sanitizer in the three compartment sink was too weak. Corrected by adding more sanitizer to provide 200 ppm
One of your store managers (full time employee) needs to obtain a food safety certification
The sanitizer test papers at the dish washing area were damaged from being wet. Provide additional test strips and keep a back-up in the office for emergencies.
Adjust/repair the front door closer so it fully closes the door
Provide and use a sanitizer bucket at your work station to store wet wiping cloths in when not in use. Two wet towels were observed on the pizza cutting table
