Observed several mops stored with the damp/wet mop heads resting on the floor adjacent to the mop sink. To correct, mops shall be hung to dry on the wall mounted storage device

Observed several wipe down rags stored on the counter. When using cloth wipe down rags, they must be stored in a sanitizing solution (concentration per manufacturers label) to prevent the growth of bacteria on the rags. Otherwise, use disposable paper towels when using the santizing solution from spray bottles. To correct, use one of the methods mentioned above

(Critical) Observed seven squeeze bottle tops in storage after washing to still have oil residue on them. Corrected by re-washing. Ensure staff are inspecting these items for cleanliness after air drying and prior to placing in storage for re-use

(Critical) The back kitchen hand washing sink was again blocked by rolling carts. Corrected by relocating the carts

(Critical) The artisan dough sticker was observed to be all peeled up along the edges and the clear top-layer was torn and peeling up in the center of the sticker. Remove and re-place this sticker with a new one. Ensure the plastic tray is fully clean and free of oil prior to application of the new decal. Take care in applying the new sticker to prevent any air bubbles under the sticker

A large gap is present on the bottom of the back delivery door. Provide a replacement door sweep to prevent pest entry

At both facility hand washing sinks: clean equipment/single service items on shelving units are located directly adjacent to the hand washing sinks. Provide a splash barrier/guard (must be smooth and easily cleanable) between the shelving units and the sinks.

This store still is lacking a certified food safety manager. Provide me verification with in two weeks that a full time manager has registered and paid in full for a food safety course to occur within 60 days, and proof of passing the exam within 90 days

(Critical) Observed eleven square food tubs and nine squeeze bottle tops that were found to be unclean after washing them. Most items had an oily residue on them, but a couple had some old food particles on them. Corrected by re-washing them. Ensure staff use fresh, hot soapy water to scrub these surfaces to remove the oily residue on the squeeze bottle tops/containers

This store has a new general manager. Neither this employee, nor any other managers currently have a food safety certification. Within 90 days provide an employee with a manager level food safety certification that works a minimum of 30 hours per week

The vertical, white plastic piece attached to the pizza dough prep counter to keep the corn meal off of the floor has become separated from the counter in several places. Please remove this plastic piece, remove old sealant, clean and dry these surfaces and reinstall with new silicone to seal the gap

Observed a large, black salad bowl to be stored upright. This bowl had collected some small pieces of dry debris. Corrected by washing and storing this bowl inverted and/or covered for protection

Observed a wet mop head stored on the tile floor next to the mop sink basin. Corrected by hanging up the mop

(Critical) Walk in cooler: observed the probe food thermometer in the container of water in the walk in cooler to be reading 25f. The water temperature was actually 35f. Corrected by adjusting the thermometer to be accurate

The mop sink faucet and the back kitchen hand washing sink faucet were observed to be dripping. Repair as needed to eliminate the dripping water

Observed many of the square white plastic food containers to be stacked prior to being fully dry.

The wet mop head was stored in the mop wringer at the mop sink basin. Corrected by hanging up the mop in the hanger provided above the mop sink

A manager with the food safety certification quit recently (2-3 weeks ago). Jared indicated he would take the manager level food safety on-line course in november. Provide me a copy of your certificate when you receive it

At the back kitchen hand washing station

Observed an employees jacket stored on top of a stack of bottled soda by the back door. Ensure staff are utilizing the provided coat rack

(Critical) Observed no c-fold paper towels provided in the wall dispenser in the back kitchen/dish washing area. Provide paper towels for staff to dry their hands

(Critical) Observed the shaved philly steak in the top of the make line to have expired yesterday (4-24). Corrected by discarding this beef

Several staff coats were observed hanging on food racks in the back of the store. Ensure staff are utilizing the provided coat hooks. The gm relocated these coats to the hooks

Provide a full time (min. 30 hours per week), certified food safety manager for this store within 90 days

Provide appropriate sink drain stoppers for the three compartment sink so the dish washing/sanitizing solutions stay in the basins when staff are washing dishes. Currently inverted bottle caps are being used

The wet mop head was resting in the bottom of the mop sink. Ensure staff are hanging up the mops above the mop sink so they can dry faster after use

Observed an employee meal (sub sandwich) in the middle of the two door upright cooler. All employee foods must be stored on the bottom of the cooler to prevent possible contamination of customer food

(Critical) Observed the manager eat some pizza toppings from the prep cooler as he walked by the unit. The potential for contaminating one's hands with saliva exists when putting food in to your mouth and then touching food or equipment subsequently is a public health concern. Food must only be eaten by staff in an approved location where the contamination of food / equipment cannot occur

(Critical) Observed the following food contact surfaces to be smooth and not easily cleanable: 1) a metal paint mixer (used with a drill) is not easily cleanable and does not have an nsf or equivalent certification. Cease the use of this mixer and either mix your pizza sauce by hand with a spoon, whisk or purchase and use a commercial stalk food mixer. 2) observed the two sauce buckets that were on the back shelf (clean storage) were observed to be scored from the use of the paint mixer--discard these and any other damaged sauce buckets and provide new ones. 3) observed the artisan pizza sticker (used to judge the proper stretching of the pizza dough to be missing sections and peeling up in other areas. Install a new sticker

Observed the serrated metal bread knife to be stored on a wooden cabinet shelf. This area is not always maintained clean and sanitized. Provide a magnetic knife holder on the side of the cabinet (toward the top of the cabinet for protection of employees) for safe knife storage

Observed the wet mop head stored in the bottom of the mop sink. Apparently the wet mop head is too heavy for the newer rack. Provide a way to hang wet mop heads over the mop sink to facilitate faster drying

The back delivery door is still lacking a door closer. Provide a door closer

Observed the back kitchen hand washing sink to be blocked by misc. Items. Corrected by providing proper access to the sink for use by staff

No paper towel is provided in the wall dispenser at the back kitchen hand washing station. Maintain paper towel at this location

No paper towel are provided at the back kitchen hand washing sink. This facility no longer purchases c-fold towels for this sink dispenser. The front kitchen hand washing sink was switched to a paper roll type dispenser. Either purchase some c-fold towels for this paper towel dispenser in the back of the facility or provide another towel dispenser that uses the same paper towel rolls at the front hand washing sink

Observed the back delivery door to still be missing a door closer. Provide a door closer

Observed the coats of several employees stored hung over the top corner of a food storage shelving unit in the back of the kitchen. Provide and utilize a coat rack or coat hooks in an out of the way area that won't contaminate foods or clean equipment

The wet mop head was found to be resting in the bottom of the mop sink. After use, hang up the mop head above the mop sink to facilitate faster drying and so any dripping water can be carried away properly

Observed the bread cutting knife being stored in a gap between the cabinet shelf and the sidewall of the cabinet. Provide a magnetic knife holder on the outside of this cabinet (toward the top of the cabinet for protection) for the safe storage of the knife

(Critical) Observed +/-8 squeeze bottle tops to be rough at the dispensing tips due to someone cutting the tips off to create a larger diameter opening. Corrected by discarding these bottle tops

Observed cardboard being used as a shelf liner for cutting / dispensing utensils at the pizza cutting station. Provide a smooth, easily cleanable surface that is re-useable in place of the cardboard such as a tray or pan. (reminder note: these utensils and the tray/container need to be switched at least every 4 hours)

(Critical) Observed the following items unclean: the cutting blade on the can opener, 21 of 44 white plastic food storage containers (most of which still had an oily residue on them, especially in the corners), a container of catering food service utensils were dusty, and numerous squeeze bottle spouts and one squeeze bottle. These items were sent to the three compartment sink to be washed

(Critical) Observed one container each of sausage and bagged pasta in the reach-in portion of the pizza prep cooler to have use-by dates of nov. 12 and 14. Also, a pre-made salad in the two door upright cooler had a discard label for the close of business on 11-21. Corrected by discarding these foods.

(Critical) The condensate drain line for the replacement walk in cooler does not terminate with an air gap. Provide a vertical air gap that is at least 2x the discharge pipe diameter

Observed a wet wiping cloth stored on the pizza cutting table. Corrected by placing the towel in a container of sanitizer

Two containers of catering food service utensils were stored unprotected. These utensils had a dust accumulation on them. In the future, please store these items in a bag or a container to protect them from misc. Contamination

Observed the door sweep for the delivery door to be missing. Provide a replacement door sweep to keep out pests/rain

The back kitchen/dish washing area hand washing sink was inaccessible due to many carts of empty pizza dough trays stacked in front of the sink. The delivery driver arrived during the inspection and removed these trays
