(Critical) The dough stretching sticker for the artisan pizza is damaged to the point it is not easily cleanable (corners peeling up, the top clear layer is de-laminating, and a portion of the top clear layer is missing and separating). Discontinue the use of this sticker and either replace this sticker with a new one or provide another smooth and easily cleanable surface to stretch this dough. ,

Observed a small open-top box of seldom used individual portion cups on the back kitchen storage shelf that were all dusty. Corrected by removing these items from the store

Observed the wet mop head resting on the floor by the mop sink. Hang up you wet mop heads over the mop sink to allow them to fully dry more quickly

The owner had both exterior doors propped open for some maintenance work being done today. Ensure the exterior doors are kept closed to prevent pest entry

(Critical) Observed the top rail of the pizza refrigerator to not have any fronst like typically observed. The potentially hazardous foods were found to be at 48 to 50f. These foods were discarded. Call for repairs of this unit and augment its cooling with ice in the top rail section.

(Critical) Observed a spray bottle (on the back kitchen chemical rack) labeled degreaser that contained a clear chemical (the degreaser used in this store has a purple/blue color). Staff were not sure what this chemical was, so it was dumped out and the bottle and spray attachment were rinsed with fresh water. Ensure staff properly label spray bottles. Another spray bottle at the back kitchen chemical rack that was not labeled as to the contents. Corrected by labeling this pink chemical "general cleaner"

(Critical) 1) observed a spray bottle of quat sanitizer at the front kitchen window sill to be too strong (>>500ppm). Corrected by diluting to 300 ppm. 2) observed a spray bottle of quat sanitizer on the back kitchen chemical rack to be too weak (<100ppm). Corrected by providing 200 ppm quat sanitizer from the wall mounted dispenser at the three compartment sink

Observed the following items to be unclean: 1) the white mat used at the dish washing sink drain board. This item was placed in the washing sink to be cleaned. 2) several blue dough trays that are being used to store the small packages of food in the walk in cooler. Clean these trays to remove misc. Foods that have accumulated in them.

The top rail of the pizza prep refrigerator is allowing foods to be at a slightly elevated temperature. Repair/adjust as needed to maintain foods at 41f and below

The air conditioning system for the front of the store is not functioning. Repair asap to provide proper temperatures in the kitchen to allow your refrigeration system to work properly

Staff are not using the available sanitizer test papers to verify the sanitizer concentration. Observed a spray bottle of sanitizer at the front kitchen window to be too strong. Ensure staff are checking the concentration with the test papers

(Critical) Observed 11 squeeze bottle tops in a storage tub for "clean" equipment to be found with oil and/or food residue on them. These tops were set aside to be re-washed. Ensure staff are thoroughly rinsing and soaking the dirty bottle tops prior to cleaning them. After they have been rinsed, sanitized and had a chance to air dry, staff need to inspect them for cleanliness. If they are not fully clean soak them until they can be re-washed, and if they are clean place them in a clean storage container for later use. Consider purchasing some small diameter brushes that will fit inside these spouts to facilitate thorough cleaning by staff

(Critical) Observed a spray bottle of quat sanitizer near the front kitchen pizza prep cooler to contain about 1 inch of liquid in it. This sanitizer must have been full strength (test paper turned a bright blue). Corrected by diluting to 200 ppm

Observed the catch trays under the ends of the conveyor unit for the pizza oven to have excessive spilled food (cheese, etc. ) from a previous day. Ensure this food source is eliminated nightly to prevent the attraction of pests.

Observed the probe food thermometer (stored in a squeeze bottle in the bottom of the pizza prep cooler) to be reading five degrees too cold. Corrected by adjusting this unit to be accurate

Observed the aluminum foil pan used to store utensils to have crumbs accumulated on the bottom. Corrected by discarding this aluminum foil pan and transferring the utensils to a new pan

Observed single service aluminum foil pans stored upright in the back room near the bulk soda bottle storage. Corrected by inverting these pans and placing them on the dry goods storage shelf

Observed wet wiping towels stored improperly: one on the back ledge of the hand washing sink and one on the back prep counter. Provide a bucket of properly mixed hand sanitizer to store your wiping cloths in when they are not being used

Observed the floor material (diamond tread sheet metal) in the walk in cooler (especially at the entry) to be in need of replacement to eliminate the current trip hazard

(Critical) Most of the squeeze bottle tops for sauces in a storage tub for clean items were found with old food debris in/on them (also two squeeze bottle tops on recently prepped sauces found in the walk in cooler (had not been used yet) were observed to be unclean). Corrected by setting up the 3 comp. Sink and having staff wash them. Obtain a small diameter bottle brush to clean the interior of the spout nozzles and ensure staff pre-soak these items in hot soapy water, use fresh hot soapy water for washing them, and verify cleanliness after these items have air dried on the shelf above the sink prior to placing them in storage

Only one partial sanitizer test kit is provided in the store. These test papers are either very old or have gotten wet, as evidenced by discoloration. Obtain a fresh quat sanitizer test kit to verify your sanitizer concentrations (200 ppm)

Observed a metal mixing tool used in an electric drill for mixing the pizza sauce that is not easily cleanable and is not a kitchen utensil. Cease using this metal mixing tool and purchase and utilize a heavy duty wire whisk

(Critical) Observed a large, round white plastic food tub that someone made into a strainer. Rough, irregular holes were cut into the bottom of the container. Replace this uncleanable strainer with a commercial plastic or metal strainer that is easily cleanable

(Critical) Observed multiple squeeze bottle tops, a spatula, a sauce pump and a couple other utensils to be unclean. These items were stored in a large tub for clean items. These items were sent to the dish washing sink to be cleaned. Asap provide a very small diameter bottle brush to be able to clean the interior of the squeeze bottle tops. I will return for a follow-up inspection to verify these items are being properly cleaned and that a brush is provided for this task

(Critical) Observed the pink general cleaner stored in a spray bottle labeled sanitizer. Corrected by emptying out the general cleaner, cleaning the bottle and sprayer and providing fresh sanitizer in the spray bottle at 200 ppm

1) observed the removable pizza oven exhaust hood filters to be overdue for cleaning. Please clean them now and more frequently in the future. 2) observed the interior surfaces of the walk in cooler evaporator coil to be covered with yeast? schedule a contractor clean the coil. This will increase the efficiency of the unit and allow it to maintain cold temperatures better, especially in the warmer times of the year

1) observed the hot water valve on the dish washing sink to have a slow leak. Repair the valve to operate properly. 2) also a slow water leak is located around the bolt under the toilet tank. Provide a new rubber seal/washer to eliminate the water leak

Observed the bottom section of the pizza prep cooler to be running slightly warm. Adjust this unit as needed to maintain 41f or colder

(Critical) Again observed the sanitizer (quat) mixed too weak in the sanitizing basin of the three compartment sink (only one cap full was added to the water). Corrected by adding additional quat concentrate to achieve 200 ppm

Observed a few plastic food containers stored upright on the back prep area shelving units. Please keep these containers stored inverted to prevent debris accumulation. Corrected by inverting

Observed the pizza prep cooler door gaskets to have food debris accumulation and mold growth on them. Corrected by cleaning these gaskets

Observed the following areas to be in need of cleaning: 1) the floor drain grate that receives condensation from the walk in cooler (on the outside of the walk in cooler), and 2) the receiving drain for the three compartment sink. Please clean these areas now and more frequently in the future

Multiple ceiling tiles are still missing in the facility. Please install missing tiles

Please ensure staff are checking each batch of sanitizer made to verify 200 ppm

1) the exhaust hood filters are due for cleaning to remove dust and oil accumulation. 2) observed yeast/mold growth on the fan grate and on the interior surfaces of the coil housing in the walk in freezer. Please clean these areas now and place them on an appropriate cleaning schedule to prevent such build-up

(Critical) 1) observed the spray bottle of quat sanitizer to be mixed too strong. 2) observed the freshly made quat sanitizer in the three compartment sink to be mixed too weak (+/- 100 ppm). Both corrected by diluting/adding more quat sanitizer as needed to provide 200 ppm. In the future, ensure staff are running the water first (75 degrees) then adding the quat sanitizer (to prevent foaming), mix the solution to distribute the chemical, and then checking the concentration with the test papers (10 seconds in the solution) to verify 200 ppm on the color chart

Observed several missing/out of place back kitchen area ceiling tiles. Please provide replacemet tiles or re-install existing ones available

Provide a light shield for the flourescent light fixture in the back kitchen area

Observed the door gaskets on the pizza prep cooler, and the blue dough tray used for storing the salad fixings on in the walk in cooler to be in need of cleaning at this time. The dough tray was cleaned during the inspection
