The wet mop head was found resting in the bottom of the mop sink. Hang up this mop head above the mop sink to facilitate faster drying

Observed a significant gap under the exterior delivery door. Install a door sweep/repair threshold as needed to eliminate this gap

Observed both lids and both doors on the dumpster to be open. Ensure these lids and doors are maintained closed to keep out pests

Observed the floor sink serving the dish washing basins to have excessive debris build-up. Thoroughly clean this area now and more frequently in the future to eliminate odors and a possible fly breeding area

The drain for the clothes washer had overflowed last night. The manager has called for same day drain cleaning service to eliminate the drain restriction

Observed no pipe insulation on the copper drain line for the walk in cooler refrigeration system. Trays of pizza dough are stored under this drain line. Provide proper pipe insulation to eliminate the condensation that may drip onto food / food containers below

The exterior door on the north side of the building used for employees has a large gap under it. Repair the door sill to be level and provide a door sweep to keep pests out of the facility

The floor sink drain at the three compartment sink is heavily soiled. Clean this basin now and more frequently in the future (at least 3x per week) to maintain it clean

Observed a slow leak from the hot water heater mixing valve. Repair/replace as needed

Observed a handle-less measuring cup in both of the tomato products in the pizza make line rail. Provide a handled scoop for dispensing this product so the handle remains above the level of the food and does not come in direct contact with the food

(Critical) The pizza cutting wheel, bubble fork and the spatula used at the pizza oven/pizza cutting table are not being cleaned frequently enough to prevent bacterial growth. Ensure your store has back ups of these utensils to switch them out prior to 4 hours, and provide a way to remind staff this task needs to be done such as a kitchen timer that can be set for four hour intervals (or less)

(Critical) Observed +/-8 to 12 blue cheese sauce cups stored out at room temperature at the pizza cutting station. This product requires refrigeration. Corrected by discarding these sauce cups. In the future, store your working supply of these sauce cups in the bottom of the nearby pizza prep cooler.

(Critical) Observed the following previously washed items to be stored unclean ready for reuse: 13 of 21 squeeze bottle tops, three squeeze bottles, the equalizer pizza cutting tool, and a sauce measuring/mixing pitcher. These items were sent to be washed again. Ensure staff (including managers) are inspecting for cleanliness after these items have been sanitized and air dried prior to placing them in clean equipment storage tubs

1) the back-side of the randell two door upright cooler handles need cleaning now and more frequently in the future. 2) observed some dry crumbs and dead insects in the two large storage tubs for squeeze bottles and food storage containers/lids. Clean these tubs now and provide a tub cover to prevent misc. Debris accumulation in the future

(Critical) Observed a house-hold use only ant spray in the storage room. Only purchase and use insecticides that are approved for use in a restaurant and use according to the label directions. Restaurant approved insecticides are not sold at hardware, home improvement or grocery stores. Contact a food service supply company if you must purchase an insect spray. Corrected by removing from facility

(Critical) Staff were sanitizing equipment in a quat solution that was too weak (+/-100 ppm). Corrected by adding additional quat concentrate to achieve 200 ppm

The quat sanitizer test papers provided are old and discolored. Provide fresh test papers that will yield an accurate color reading when used

Upon verification with the bottle of concentrate, it was determined to be a general cleaner

(Critical) Observed a paint brush (with wooden handle and metal band) provided and used for oiling pizza pans. Paint brushes are not intended for use with foods as they are not easily cleanable due to the way they are constructed. Discard this brush and provide a commercial pastry brush with the bristles imbedded in food grade plastic

Observed crumbled/missing concrete at the side exterior door threshold. There are large gaps under the door itself. Repair this area to be at the proper elevation and smooth, and repair/replace the door sweep as needed to eliminate the existing gaps to prevent pest entry and to save on heating/cooling costs

Observed all three of the pizza prep cooler door gaskets to be torn and trapping food debris. Replace these gaskets to provide a smooth and cleanable surface
