(Critical) In the dry storage room observed mouse feces: 1. On the floor under the shelving unit in several areas 2. On items located on the first shelf of the shelving unit correct by thoroughly cleaning and sanitizing the area and equipment. Discard any affected food and single service food containers. Inspect the area for openings. Contact your pest company to schedule increased control.
(Critical) At the handsink in the kitchen area observed no cold water available, the cold water valve does not operate. Using only hot water creates an uncomfortable and possibly unsafe handwashing experience because the water becomes too hot (140 degrees f). Correct by replacing/repairing the cold water valve to provide comfortable water temperature at the sink.
At the fryer/oven area observed a heavy accumulation of grease, food debris, and dust on the floor under and the walls around the equipment. Correct by cleaning now and on a more frequent basis to prevent the heavy accumulation

Cardboard boxes 6. Empty food containers correct by removing unnecessary items. A clutter free area allows for easy cleaning and inspection for pests.
Observed the rear screen door with holes and rips in the screening. Correct by repairing or replacing the screening to prevent the entry of pests. This is a repeat violation
At the donut filling area on the butter cream dispenser observed duct tape holding the nozzle in place. The tape does not contact the filling. Correct by repairing or replacing the nozzle and eliminating the tape so the surfaces are smooth and easily cleanable

Cook line: the gap between the cutting board and the unit is full of crumbs. Also, there is a considerable amount of bacon and other crumbs located in the gap to the left of where the bacon is stored. Thoroughly clean these areas to eliminate this food debris

Thoroughly clean these surfaces using an appropriately sized bottle brush on a regular basis, then rinse and sanitize these surfaces. 2) the dispensing spouts for the hot chocolate machine (several flavors) are overdue for cleaning

Observed a doughnut filling machine on the walk in cooler floor. Elevate all food and equipment containing food at least 6" off of the floor for protection

(Critical) Observed an employee who set up the three compartment sink with no water in the wash basin, hot water & bleach in the middle compartment, and rinse water in the sanitizing basin. The employee indicated the wash basin is not used because water leaks out on the floor when the sink is drained. Corrected by the employee adding bleach to the clear water in the sanitizing basin. The dishes were rinsed off in the middle compartment with the wash water by running the water from the spout. Then rinsed dishes are sanitized in the last basin prior to air drying them
Observed the soda dispenser spouts, diffusers and the mounting bases to be overdue for cleaning. Clean these surfaces nightly
The owner has hired an employee with the serv-safe certification, however the employee indicated the certificate is in storage at the previous city she lived. Locate and provide this office a copy of your certification as verification of passing the course/exam
