(Critical) On follow up inspection found a y type shut off valve installed on mop sink faucet, which has on one side a vented device and hose leading to chemical dispenser, and on the other side a hose bibb vacuum breaker downstream of shutoff valve attached to a filling hose. Violation corrected.

(Critical) On follow up inspection found no chemical spray bottles stored above food or food utensils. Pic has submitted a risk control plan (rcp), which was reviewed and should work if followed. Violation corrected.

(Critical) On follow up inspection found no food prepared and packaged by a food processing plant passed it's use by date. Pic has submitted a risk control plan for adulterated food, which was reviewed and should work if followed. Violation corrected.

(Critical) On follow up inspection did not observe employee handle raw meat with gloves. Pic states employees are trained to either handle raw meats with tongs or if using gloves, to remove gloves, wash hands and wear new gloves. Violation corrected.

(Critical) Observed employee handle/place portioned raw meat on grill and continue working with ready to eat foods, that receive little or no cooking without changing gloves. Employee corrected by removing gloves, wash hands and place a new pair of gloves on hands. Pic has been instructed to monitor and ensure employees are changing gloves after different tasks, that is changing gloves after handling raw meats and before handling ready to eat foods. ,

(Critical) Found chemical spray bottle hanging on shelf above slicer on the prep table and another chemical spray bottle on prep table between slicers. Employee corrected by removing spray bottles from food prep area and placed chemical spray bottles in the chemical storage area. Found a bottle of honey stored in the chemical storage cabinet. Pic states honey is for baiting flies and corrected by labelling honey for baiting only. This is a repeated violation and a risk control plan must be submitted. ,

(Critical) Found food prepared and packaged by a food processing plant, an unopened case of rotelli pasta salad passed it's use by date (2/1/15) in the walk in cooler, and not honestly presented. Employee corrected by discarding case of rotelli pasta and also discarded all portioned containers of rotelli pasta salad as they were probably from this batch. This is repeated violation and a risk control plan must be submitted. ,

(Critical) Found a hose attached to a shutoff valve downstream of an atmospheric vacuum breaker on the mop sink faucet. To correct install a vented t valve on faucet spout, then install a y type shutoff valve on t valve, (one shutoff to chemical dispenser and one to fill hose. ,

Found the chemical dispenser (soap and sanitizer) on the wall mounted above the 3 compartment sink not in use, with a sign indicating "do not use". To correct repair chemical dispenser.

Observed flies in the back kitchen prep area. To correct keep facility clean and dry, in good repair, and continue fly control measures.

Found cases of gloves stored on the floor behind the front counter. To correct store gloves on shelf at least 6 inches off the floor.

Found employee clothing stored on top of single use containers and on shelf post in the storage area. Found employee personal food items stored on numerous shelves through out storage room. To correct store employee clothing and food in designated areas only.

Observed baker placing dough on used baking liners on baking sheets. To correct either show baking liners can be reused or discontinue reusing single use baking liners.

Found numerous shelves through out facility lined with cardboard. To correct remove cardboard.

Found interior edges, bottoms and interior door gaskets of coolers soiled with accumulation of debris. To correct thoroughly clean coolers and clean on a more frequent schedule.

(Critical) Asked employee how soiled slicer will be cleaned, employee states he will unplug slicer and clean slicer with sanitizer and wiping cloth, sharpen slicer, and wiped down slicer with sanitizer. Corrected by educating employee slicer should be pre-rinsed/wiped to remove debris, washed with soapy water, rinsed with clean water, wiped with sanitizer and allowed to air dry. Pic has been instructed to monitor and ensure employees are properly cleaning slicer. ,

(Critical) Found chemical spray bottles hanging on utensil and food shelf racks. Found a large bottle of slicer oil stored on shelf next to food supplies. Pic corrected by relocating chemicals to chemical storage area. ,

(Critical) Found food prepared and packaged by a food processing plant, 3 gallon size containers of macaroni salad passed it's use by date (1 opened and 2 unopened) and not honestly presented in the walk in cooler. Pic corrected by discarding macaroni salad and has been instructed to check product dates on receipt, to rotate products and to discard items passed it's use by date. ,

(Critical) Found time/temperature control for safety (tcs) food, flat beef 49°F on the bottow drawer of the 2 drawer under the counter cooler. Pic temporarily corrected by discarding beef and has been instructed not to use this cooler to hold tcs food, until repaired/replaced to hold tcs food at 41°F. ,

(Critical) Found the pop nozzles of both pop machines in the front counter soiled with build up and accumulation. Found the can opener blade and surrounding areas soiled with accumulation. To correct follow manufacturers directions and clean pop nozzles and can opener. ,

Found the floor under equipment, edges and corners in the entire facility soiled with accumulation of debris. Found some of the floor drains in facility covered with plastic and cardboard which were soiled with accumulation of debris. To correct thoroughly clean floor and drain covers and clean on a more frequent schedule.

Found the door gaskets, door handles, exterior door panels, and interior surfaces of coolers soiled with accumulation. To correct properly clean coolers. Found the exterior of clean food containers on the clean utensil storage shelf with old datemarking stickers. To correct remove stickers prior to washing.

Found numerous employee personal drinks stored on utensil shelf in the back kitchen area. To correct designate an employee area and have employee store personal drinks in designated area only.

Found the 2 drawer under the counter cooler not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F. Found water on the bottom of coolers. To correct verify coolers are draining properly, clean coolers, and repair if necessary.

Observed flies in fly traps by floor drain and by the 3 compartment sink area. To correct keep areas clean and dry, in good repair, and continue fly control measures. Suggest facility contact pest control company and use drain fly treatment to enhance fly control measures.

(Critical) At the reach in cooler on the main cook line observed the following sliced foods at 50 degrees f. : turkey ham spicy ham salami corned beef mozzarella provolone cheese these items had been in the cooler for about an hour. The air temp thermometer inside the unit registered 48 degrees f. Non potentially hazardous foods were found to be at 49 degrees f. The thermostat on the unit was found to be set improperly as to not provide proper cooling. The pic adjusted the thermomstat to a cooler setting. After an hour food temperatures were still found at or above 48 degrees f. The above mentioned phf items were moved to another proper temperature unit for rapid cooling. ,

On the make line prep top unit nearest the soda merchandiser is not cooling to a temperature to maintain the potentially hazardous food items stored within at 41 degrees f or below. Correct by calling for repair on the unit

Observed the following floor areas with an accumulation of food debris: 1. Walk in cooler 2. Under the coke retail display cabinet 3. Under the three compartment sink correct by cleaning now and on a more frequent basis to prevent the accumulation of food debris. Also observed a black residue on the wall above the three compartment sink. Correct by cleaning or replacing the frp. Pic indicates she is aware of the situation and has tried different cleansers

On the main sandwich make line, observed no thermometers in the reach in portion of four of five prep top coolers. Correct by installing prominently displayed approved thermometers

Observed missing base cove tiles by the three compartment sink and by the hand sink adjacent to the managers office. Correct by replacing the missing tile

Observed several pans wet stacked at the three compartment sink. Correct by completely air drying utensils before stacking for storage
