(Critical) Observed raw hamburger stored above ready to eat cooked rubsw. Food shall be protected from cross contamination by: (1) except as specified in (1)(c) below, separating raw animal foods during storage, preparation, holding, and display from: (a) raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables,(p) and (b) cooked ready-to-eat food; (p) to correct move raw hamburger down and away from all ready to eat foods. Corrected: person in charge moved all noted products down and away from all ready to eat foods.

(Critical) Observed the hand sink to have been blocked by soiled pans. A hand washing sink shall be maintained so that it is accessible at all times for employee use. (pf) to correct discontinue blocking hand sink and remove pans from sink. Corrected: person in charge unblocked noted sink

Observed drain flies in the basement in area of the facility. The premises shall be maintained free of insects, rodents, and other pests. The presence of insects, rodents, and other pests shall be controlled to eliminate their presence on the premises by: (a) routinely inspecting incoming shipments of food and supplies; (b) routinely inspecting the premises for evidence of pests; (c) using methods, if pests are found, such as trapping devices or other means of pest control as specified under ## 7-202. 12, 7-206. 12, and 7-206. 13;(pf) and (d) eliminating harborage conditions. To correct contact your pest control company and have them come out and treat and increase treatment until pests can be eliminated

Observed the upstairs bar coolers not holding food at 41f or below. Equipment shall be maintained in a state of repair and condition to correct repair all the noted coolers to hold cold food at 41f or below. ,

The pop carbonation area soiled with heavy grease build up, dripping down to the floor. Physical facilities shall be cleaned as often as necessary to keep them clean. To correct deep clean the noted area

(Critical) Observed the following soiled food contact surfaces: 1. Interior upper part of the lemon squeezer located nearest to the small one door top loading cooler. 2. Bottom of the shelf over the steam table and plate storage cart. Equipment food-contact surfaces and utensils shall be cleaned: (1) except as specified in # (b) of this section, before each use with a different type of raw animal food such as beef, fish, lamb, pork, or poultry; (p) (2) each time there is a change from working with raw foods to working with ready-to-eat foods; (p) (3) between uses with raw fruits and vegetables and with potentially hazardous food (time/temperature control for safety food); (p) (4) before using or storing a food temperature measuring device; (p) and (5) at any time during the operation when contamination may have occurred. (p) to correct wash, rinse and sanitize the noted items.

(Critical) Observed the following food holding above 41f for an unknown amount of time: 1. Half and half 45f located in the upstairs bar cooler nearest to the entrance. 2. Creamer 45f located in the upstairs bar cooler, nearest to the entrance. 3. Russian dressing 46f located in the one door small top loading cooler nearest to the basement stairs. Potentially hazardous food that is to be cold shall be held at 41f or below. To correct: discard the noted food into the garbage. Corrected: person in charge discarded the noted items into the garbage

(Critical) Observed the following in poor repair and needing to be replaced: 1. Both lemon squeezer's located at both bars. 2. Both lime squeezer's located at both bars. 3. Orange squeezer located at main level bar. Multi use food-contact surfaces shall be: (1) smooth; (pf) (2) free of breaks, open seams, cracks, chips, inclusions, pits, and similar imperfections; (pf) (3) free of sharp internal angles, corners, and crevices; (pf) (4) finished to have smooth welds and joints; (pf) to correct discard and replace all noted items

Observed the ceiling in the basement prep area and the dishwashing area in very poor repair and peeling paint and heavily damaged. Physical facilities shall be maintained in good repair. To correct repair and repaint the noted ceiling.

Observed both coolers not holding cold food at 41f or below in the upstairs bar. Equipment shall be maintained in a state of repair and good condition. To correct: repair both coolers to hold cold food at 41f or below

Observed many reused single service containers such as sour cream tubs and other items that used to contain food. Single-service and single-use articles may not be reused. To correct: discontinue reusing single service containers and purchase reusable/washable food grade containers.

(Critical) Found time/temperature control for safety (tcs) food, opened half and half 46°F and whipped cream 46°F in the 3 door bar cooler. Pic corrected by discarding half and half and whipped cream, and has been instructed not to use this cooler to hold tcs food, until repaired/replaced to hold tcs food at 41°F. ,

Found the tiled floor in the entire facility with cracked, chipped and damaged tile. To correct repair tile floor to be smooth, durable and cleanable.

Found the 3 door bar cooler in the upper level bar not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F.

Found the basement employee restroom door propped open. To correct close restroom door and keep closed.

Found an opened cup of beverage on the basement prep table. To correct either drink in designated areas or use a cup with lid and straw in prep areas.

Found the floor under equipment, edges and corners in the entire facility soiled with accumulation and build up. To correct throughly clean floor and keep clean.

Found the fan cover of the 1 door worktop cooler soiled with dusty build up. Found the interior of the microwave on the cooking line soiled. To correct clean fan cover and microwave and clean on a more frequent schedule.

(Critical) Used a chlorine test kit and found the dishwashing machine not sanitizing dishes at manufacturer's specification of 50 ppm. Pic states they do test dishwashing machine but will increase frequencty of testing. Pic corrected by calling for repair, service tech arrived and adjusted dishwashing machine to manufacturer's specification of 50 ppm. Violation corrected.

Found celery and cucumber stored on the bottom of the main level bar cooler. To correct store celery and cucumber in a container in cooler.

Found facility using a single use gallon container (filled with water and frozen) as an ice wand. To correct discontinue using single use gallon container as an ice wand as this container is not durable, cleanable and designed for continuous use.

Found a hole in a water stained panel attached to the basement ceiling above the prep table. To correct verify ceiling is not leaking and properly repair ceiling to be durable, smooth and cleanable. Found wall in main level kitchen and basement prep area with damaged walls. To correct repair walls to be durable, smooth, and cleanable.

Observed the presence of flies in the upper level bar corner pop gun holder. To correct keep facility in good repair, pop gun holder clean and continue fly control measures.

(Critical) Found time/temperature control for safety (tcs) food, hummus 48°F, mac and cheese 48°F, and rice pilaf 48°F in the bottom portion of the 2 door prep top cooler. Pic temporarily corrected by discarding tcs food in the bottom portion of cooler and has been instructed not to use this cooler to hold tcs food, until repaired/replaced to hold tcs food at 41°F. This is a repeated cold hold violation and a risk control plan has been issued. ,

Found the tiled floor in the upper level bar with numerous cracks. Found the tiled floor in the basement kitchen area with cracks and chips. To correct repair/replace tiled floor to be durable, smooth, and cleanable.

Found the bottom portion of the 2 door prep top cooler not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F or lower. Found the door gasket and interior panel of the chest freezer in the basement damaged and the spray rinse nozzle in the basement dishwashing area wrapped with tape and plastic. To correct repair/replace freezer and spray rinse nozzle.

Observed flies in the basement storage area. To correct keep areas clean and dry, in good repair, and continue fly treatments.

Found the sides of the grill on the cooking line soiled with build up and accumulation. Found the interior sides of the sanitizing buckets through out facility soiled with build up. Found the door gaskets of coolers soiled with accumulation. To correct clean noted areas and clean on a more frequent schedule.

Found the floor (under equipment, edges and corners) in the entire facility soiled with accumulation of debris and build up. Found the wall behind and around the steam table soiled. Found grease appear to be dripping from the main level floor and dripping on to the pop carbonator motor. To correct thoroughly clean floors and walls and clean on a more frequent schedule.
