Corrected by discarding the ready to eat food into the garbage.

There is no expiration date on the certificate in the establishment. Found single service items and clean containers and lids in a storage tub stored directly on the floor. Corrected - single service items were relocated to an area at least 6" above the floor. The storage tub of containers and lids will be moved when the new racks are completed - within 30 days

Found pail of slush mix stored directly on the floor. Corrected - pail of mix was put on a crate that will be replaced by a permanent rack - within 30 days

No anti-choking poster found displayed in establishment. To correct - post provided anti-chiking sign in establishment

(Critical) Observed the following two foods in the walk in cooler to be expired: 1) re-fried beans (5-2 to 5-8), and 2) chili sauce (expired 5-15). Corrected by discarding these foods. Ensure staff are making daily checks of your stock and purging any required foods that are past their last date of use

(Critical) A small amount of raw produce that needs to be washed/rinsed off prior to preparation was observed in the facility today. No food prep sink is currently provided. To correct, provide an air gapped drain line for the sanitizing basin of the three compartment sink

Observed two wiping cloth pails that did not register any chlorine sanitizer with chlorine test papers. Corrected by providing fresh sanitizer for these pails

Observed a bulk container of syrup on the back kitchen floor. Provide a low rack/shelf to elevate this food container off of the floor at least 6 inches

1) styrofoam cup holder/dispenser: observed the top cups without a cover to prevent misc. Contamination. Corrected by placing lids on the top cups for each dispenser. 2) observed several red pails used for the shake & soft serve machines to be stored stacked in an upright position. Keep these pails stored inverted on a clean and sanitized surface

(Critical) Improper reheat of nacho cheese. Ncho cheese leftover is being removed from hot holding unit nitely and being placed into hot holding dispenser on the nest day to reheat. This unit will only rehat to max 145 degreesf. Anytime a potentially hazardous food is cooled and reheated fr hot holding it must be rapidly cooled properly and reheated to 165 degrees f within 2 hours. Keep product in this unit until consumed. It is designed for hot holding continuously. . Corrected by discussion and discarding product.

(Critical) The chocolate soft serve was found at 50 degrees f in the bin. This product is ph and must be maintained at 41 degrees f or below. Unit on this side is ordered out of service for storage of product. Call for repair of unit. Once repaired you may use it to store ph foods. And maintain 41 degrees f or below.

(Critical) Three cartons filled with homemade salsas found in dry storage (2) and 1 in the walk in cooler. Marked burning hands salsa dated from 2005-2008┬░Found in the facility. Owner indicates for personal use only. This is prohibited to be in the facility and are to be removed immediately. If these are found in the facility again, enforcement will be taken. Corrected by removing products and placing into owner's residence which is immediately adjacent to the facility

(Critical) Refried beans found on top of the prep unit at 46 degrees f. Bean are potentially hazardous and must be maintained at 41 degrees f. Or below. In order to maintain prper temperature recommend a deeper container and use less product. Corrected by discarding refried beans

Top of prep unit not holding refried beans at 41 degrees f. Use a deeper container for the beans and less product. Unable to maintain proper temperature in shallow container.

The soft serve machine is not holding product on the left side at 41 degrees f or below in the bin. This side of the unit is ordered out of service for the storage of ph foods. Once the unit is repaired ok to place back into service if capable of holding product at 41 degrees f or below.

The gaskets on the doors of the prep unit need cleaning. Clean now and place on amore frequent cleaning schedule.

The interior of the microwave needs cleaning. Clean now and place on a more frequent cleaning schedule.
