(Critical) Observed the following food contact surfaces not clean to sight: 1. At the prep area the can opener blade observed black grime. 2. Inside the ice machine observed black and pink slim in several locations that touch the ice. Correct by thoroughly cleaning and sanitizing these surfaces now and on a more frequent basis. This violation was previously cited numerous times. On august 11, 2011, an informal hearing was held to discuss the numerous citations of this violation. During this current inspection rick morawa and i reviewed the informal hearing letter. If this violation is cited during any of the next four inspections damons will be subject to washtenaw county's progressive violation enforcement action. Action taken may include temporary closure and possibly license revocation. A followup will be conducted to confirm these surfaces have been properly cleaned. ,

(Critical) Observed delivery of sharpened knives by james ferrari company. Knives were put directly into use without proper wash, rinse, and sanitizing. Employee of delivery company states knives are not sanitized prior to delivery. Pic corrected during inspection by properly washing, rinsing, and sanitizing knives. In the future knives should be properly washed before being put into use

(Critical) Observed the following instances of improper bare hand contact of ready to eat food: observed a server prepare a salad for a customer by grabbing lettuce and several toppings with his bare hand. Observed another server fill a glass with ice and then use his bare hand to restrain the ice to drain water from the glass. The glass was then filled with soda. Each item was corrected by discarding the food item and counseling the server about no bare hand contact

(Critical) At the bar area observed a can of "raid commercial flying insect killer. " the label states, "do not use in food areas of food handling establishments, restaurants, or other areas where food is commercially prepared. " the pic indicates this product is used by employees of the establishment for the control of insects. Pic also indicates the establishment has a contract with orkin. Item corrected by removing the can from the premises. This type of pest control must be done by qualified individuals

(Critical) Observed the following improper cold holding food/temperature violations. 1. At the salad station prep top cooler on the top rail observed shredded lettuce, shredded cheese, and tomatoes at 53 degrees f. These items had been stored in unit for some time. The items had not been prepped recently and should have been 41 degrees f or lower. 2. At the onion ring make area observed onion ring batter (eggs and dairy potentially hazardous) at 50 degrees f. This batter was stored on ice. 3. At the victory upright cooler observed raw chicken breast at 51 degrees f marinated chicken breast at 58 degrees f precooked ribs at 50 degrees f. Each item was corrected by discarding the affected food item and replacing it with new proper temperature food. In the rear kitchen prep area on the shelves observed several no longer used items and broken items stored with other items. These intermixed items make it difficult to determine what utensils are clean and ready for use. In the former take out waiting area observed no longer used equipment. Correct by organizing the are and discarding unused and broken items

At the main cook line observed the outside of the toaster with an excessive amount of grime and grease. ' correct by cleaning now and more frequently to prevent the accumulation of grime

The following areas are in poor repair: 1. Back kitchen area dry goods storage area. Wall covering is loose, interior of wall is visible, tiling is loose and missing. 2. Drywall is broken at wall corner across from manager office. 3. Drywall is broken and missing between doors to dining area. 4. Drywall corner is missing and wall covering is missing behind ice machine. 5. Floor tiles at the wall floor juncture are missing or loose in the cook line, prep area, and next to wic door. Correct by repairing or replacing as necessary. All of these items were mentioned in a previous reports. Progress must be made toward repairing these items or enforcement action may be taken

Several pieces of equipment in the kitchen area are in need of repair: 1. Can opener cutting blade appears dull 2. Reach in cooler by deep fryer has broken and cracked seals 3. The front top cover for the ice maker has been removed. 4. The floor of the walk in cooler is separated and lifting upward 5. The door seals of the walk in cooler are cracked 6. The door handle of the top oven of the "alto-sham" hot cabinet is missing. 7. The door seals of the salad prep reach in cooler are cracked and hanging off of the door. 8. The door handle of the "halo-heat" hot holding unit is missing. 9. On the cook line the handle of the microwave is missing. Correct each item by repairing or replacing item noted. All of these items has been noted during previous inspections. Progress must be made on repairing these items or enforcement action may be taken

At the cook line adjacent to the salad area observed the ice cream dipper well turned off with two in use ice cream scoops stored inside. At the server food prep line observed two in use condiment scoops stored in standing water. Correct each item by storing as outlined above

Observed fluorescent lightbulbs without protective shielding at the following locations: 1. Over the main food prep area in the rear kitchen area. 2. Over the storage shelves in the dry food storage area. 3. Over the main cook line correct by replacing protective shielding

At the main dishware storage area by the dishwashing area observed clean dishware stacked wet for storage. Correct by completely air drying before storing properly cleaned dishware

Observed the following floor areas with an accumulation of excessive grease, food debris, and grime. 1. Under salad make area 2. In the storage room 3. Under main cook line cooking equipment 4. Under the dishwashing area. Correct by cleaning now and on a more frequent basis to prevent the accumulation of grease, food debris, and grime

In the kitchen at the employee restroom sink observed no handwashing soap available. Pic corrected during the inspection by providing soap at the dispenser

In the kitchen at the wait station hand sink observed the faucet handle to be missing. The sink is not operable. Correct by repairing or replacing the sink faucet to allow for proper handwashing.

(Critical) Observed grated cheese in open bag at 48 degrees f. Located at the wait area three door reach-in refrigerator in the kitchen. Observed diced tomato and grated cheese at 48 to 52 degrees f. At raise rail refrigeration prep unit at the cook line and wait area sections of the kitchen. Observed hot vegetables peppers, onions at 130 degrees f. Placed in double raised pans at the cooks area hot food holding table. Found gallon of milk at 47 degrees f. Located at wait area pepsi glass door reach-in refrigerator. To correct monitor food cold holding temperatures frequently during each work shift to properly store cold food items at 41 degrees f. Or less as required. Foods not properly held at 41 degrees f. Or below should be reported to the pic and corrective action taken to ensure proper cold holding temperatures are provided or food should be discarded when temperatures are not able to be controled as required to prevent growth of pathogenic bacteria. ,

(Critical) Found the slicer machine located at the back kitchen area with food residue on surfaces of the slicer at shielding and guards. Found pans at the clean utensils storage shelf located beside the dishwashing area of the kitchen with food residue on food contact surfaces of the pans. To correct should retrain the dishwashing staff to inspect the pans after cleaning process to make certain the food contact surfaces of the utensils and equipment are cleaned as required prior to stacking at clean pan shelving. ,

Observed floor tile in poor repair at the cooks area of the kitchen. Found grout to be missing from floor tile at the dishwashing area three compartment sinks and at the cooks area also. Found the walk-in beer cooler floor to buckel up at entry found base coving missing from wall floor juncture at section of the kitchen. To correct schedule repairs to floor surfaces to provide an easily cleanable floor surface

Found the hand washing sink located at the wait area of the kitchen with low flow of water at the hand sink faucet. To correct increase flow of water at the faucet of the wait area hand sink to encourage proper employee hand washing

Observed the utensil shelf located at the back area of the kitchen with accumulation of debris on the shelf. To correct schedule routine cleaning of the shelving as needed to keep surfaces of the utensil storage shelf clean as required

Found the temperature dial for the dishwashing machine final rinse to not show the final rinse temperature on the dial as required. To correct service the temperature dial at the dishwashing machine to indicate final hot water rinse temperatures as required

Observed the light fixture located above the wait area of the kitchen and inside the beer walk-in cooler with light fixture without a protective cover to protect lighting against breakage as required over food service areas of the kitchen. To correct schedule repairs to provide shielded lighting to protect against breakage of light bulbs over areas needing protection

Observed items stored inside hand washing sink located at the bar hand washing sink preventing use of this hand sink as required. To corrrect remove items from the bar hand sink to encourage proper employee hand washing as required
