(Critical) New fittings and a hose to a chemical dispenser were installed at the faucet under the three compartment sink drain board. This faucet has a built-in atmospheric vacuum breaker. Vacuum breakers are not rated for the installation of down stream shut-off valves. Either provide a wasting tee on the faucet (the water discharge must be routed to an air gapped disposal location) or install a pressure vacuum breaker (at the required height) to connect this hose for the chemical dispenser.
Observed an employee use a hard surface cleaner (diversey alpha hp multi-surface disinfectant cleaner) to spray down the blizzard mixing spindle and then apply a sanitizer. When formally cleaning this surface a separate rinse step with fresh water is required between the cleaning and sanitizing steps

Observed the drain line for the dipper well (next to the northern soft serve machine) to have slipped down into the floor sink. Please re-establish the drain line air gap. Corrected by elevating the drain line to provide 3" plus air gap
(Critical) Observed 1) the discharge line for the dipper well to be inserted into the floor sink drain. 2) the drain line for the southern soft serve machine to not have the required air gap above the floor sink drain. Please provide air gaps for these drain lines (vertical air gap = 2x the discharge pipe diameter above the flood level of the receiving drain)
(Critical) Observed no hose bib vacuum breakers (asse 1011 devices) installed on the four water cooling lines (non-food grade water hoses) for the two soft serve machines. Please either install vacuum breakers on all four connections or install food grade hoses to supply cooling water to these units (the discharge hoses must be air gapped)
Observed unprotected cardboard cake disks (two stacks) in the attic storage area. Please maintain these food contact surfaces safe from contamination by keeping them stored in a plastic bag or plastic storage tub.
Although your food service license is available at your facility it is posted on the side of the walk in cooler behind a storage shelf. Please relocate it to a location viewable by your customers
Provide a smooth, rigid and easily cleanable material to act as a splash guard on the storage rack/shelf used to store the bananas that is located adjacent to the hand sink
Provide sanitizer in all of your wiping cloth buckets. Correct w/in one day. This will prevent bacterial growth between uses
Observed hand washing happening at the three compartment sink. Please discontinue this practice (remove hand washing soap from this station) as of today to prevent contamination of the sink
Please re-establish the drain line air gap for the southern soft serve machine above the flood rim of the receiving basin
Observed uncovered red plastic spoons in the "attic" storage room. Please cover/protect these spoons
