(Critical) Provide use-by dates for your containers of potentially hazardous, ready to eat foods used to top baked subs and preparing salads (deli turkey, chicken, ham, sliced tomatoes, feta cheese, etc. )

(Critical) Only one supply of food is provided to prepare pizzas, subs and salads. The supply of foods that are dispensed from the prep cooler with clean, bare hands for baked items (such as pizza and hot subs) may not also be used for topping subs (after they are baked) or for preparing cold salads. Provide a separate supply of small food containers with your ready to eat foods stored in the salad prep cooler. This way staff only need to use single use gloves with the separate supply of rte foods

Observed cracked door gaskets on the salad prep cooler. Please replace the gaskets.

Observed excessive dust debris build-up on the ceiling fan, and multiple shelving units. Please clean these areas now and more frequently in the future to prevent this build-up

Observed: 1) two of the four pizza oven exhaust hood filters to be over-due for cleaning, and 2) the pizza oven spill trays are also due for cleaning at this time. Please clean these filters and trays more frequently to prevent the build-up of grease/dust/food crumbs.

The pizza prep cooler is non-functional. The owner replaced the compressor last summer, and now the evaporator coil is damaged and needs to be replaced. The owner indicated the coil is on order and should arrive yet this week. Repair this unit asap so it can be used for holding your potentially hazardous foods. Once the unit is repaired to maintain 41f and below it may be utilized

Health Report - 08/04/2009 (2 critical incident(s))

(Critical) Observed the following food contact surfaces to be unclean: 1) the spindle, and cover of the electric food processor, 2) the dough mixer attachment for the food processor, 3) the can opener blade, and 4) two to three metal food pans (1/3 size). These items were sent to the dish washing sink to soak prior to cleaning.

(Critical) Many chemicals were stored on a tray located a shelf above foods/spices/clean equipment. Corrected by moving the chemicals off of the shelf away from food

Observed the front door propped open with out a screen door. Please either provide a screen door or keep the front door closed to keep out pests

Observed the thermometer in the bottom of the pizza prep cooler to be damaged. Corrected by installing a replacement one

Observed the following items to be in need of cleaning now and more frequently in the future: 1) the exhaust hood filters for the pizza oven, 2) the back area shelves above the three compartment sink, 3) the door gaskets on the prep coolers

Observed the bottom section only of the three door pizza prep cooler to be operating at a slightly elevated temperature. The owner indicated this cooler has had the compressor replaced since the last inspection. The temperature dial was at a setting of 5 on a scale of 9. The dial was moved to a setting of 6. Ensure the bottom of this unit maintains 41f and below

The bottom of the bathroom base cabinet and most of the wooden baseboard in the bathroom to still be rotting due to water leaks from the sink plumbing. All damaged wood needs to be eliminated and replaced with man-made materials that don't absorb moisture.

(Critical) Observed sliced deli turkey, feta cheese without use-by dates. Corrected by providing use-by dates based upon when the food was opened and sliced

Observed the wet mop head resting on the floor in the back room. Please utilize the mop rack to facilitate air drying of the mop head

A connection on the hot water line to the restroom hand sink is still leaking. Repair this connection asap to eliminate the dripping water.

Observed the low boy cooler for salads to be maintaining foods at 41. 5/42f and the temperature control dial was at its maximum setting of 9. Check the compressor coil for dust accumulation--and clean if needed, otherwise have the unit evaluated and repaired by a refrigeration contractor to maintain 41f and below

Observed the bottom of the bathroom base cabinet and most of the wooden baseboard in the bathroom to be rotting due to the water leak from the sink. Please remove all water damaged/rotting wood and replace with sealed (varnish/polyurethane) wood to prevent moisture absorption in the future

Observed several torn/cracked cooler door gaskets on coolers. Please replace these gaskets to provide a smooth and easily cleanable surface that doesn't trap food debris

Please either seal the wooden base board in the bathroom to prevent moisture absorption or replace it with a man-made material that won't absorb water
