Found the floor under equipment, edges and corners in the pretzel prep area and dishwashing area soiled with accumulation. To correct thoroughly clean floor and keep clean.

Found a soiled pizza screen in the compact pizza oven in the back kitchen area. Pic corrected by taking pizza screen to the dishwashing area, and was reminded to clean pizza screens every day.

Found the wall corner and edge towards the entrance of the front counter area damaged with exposed surfaces. To correct repair damaged surface and finish area to be durable and cleanable. Found numerous ceiling tile through out facility with water stains. To correct verify facility is not leaking, repair if needed, and repair/replace ceiling tile.

(Critical) Asked employee how dishes are washed in the 3 compartment sink, employee states dishes are washed and scrubbed in soapy water and rinsed. Temporarily corrected by educating employee on proper steps in washing dishes and set up sanitizing compartment of sink with a chlorine sanitizing solution. To correct pic must monitor and ensure employees are washing, rinsing and sanitizing dishes. ,

(Critical) Found no sanitizing test kit available to test sanitizing solution. To correct determine what type of sanitzer facility will be using and provide appropriate test kit, that is if chlorine is the sanitizer, provide a chlorine test kit. ,

Found no manager certified in food safety employed at facility. To correct provide a certified manager.

Found build up in the spout of the sweet ice tea dispenser. To correct follow manufacturer's directions and properly clean ice tea dispenser and spout.

Found the cherry coke spout on the pop machine in the front counter leaking water. To correct repair pop machine spout.

Found the some of the door gaskets and door sealing areas of coolers soiled with accumulation and stains. To correct clean door gaskets and door sealing surfaces of coolers.

Found cookies and muffins for sale in the front counter , without ingredients and allergy information available. Employee was able to obtain ingredients and allergens for muffins, but not for cookies. To correct have ingredients and allergen information available upon customer request.

Temporarily corrected at time of inspection by discarding all potentially hazardous food - milk and yogurt found at 48°F - 49°F. This unit is ordered out of service for the storage of potentially hazardous food until it is repaired and capable of maintaining potentially hazardous food at 41°F or below. ,

Some unnecessary items found stored in rear kitchen. Please get rid of any items not used in the operation and maintenance of facility, including domestic dishmachine that is not in use. This will make it easier to clean and eliminate harborage conditions for insects and rodents

Observed folding table with contact paper, wooden cart to hold microwave oven and wooden tray holding single-service items on top of microwave oven in section of kitchen behind front counter and overturned crates in various areas. Please replace with smooth, non-absorbant, easily cleanable items

Please provide smooth, easily cleanable shelving units such as dunnage racks to store unwrapped beverages a minimum 6 inches above floor on shelving that is easily cleaned underneath. Facility currently using overturned crates

(Critical) Observed the mop sink faucet with an atmospheric vacuum breaker approved for use only without any terminal shut off valving located down stream of the mop sink faucet, however, during this routine food service inspection a "y" style connection was found with three shut off valves installed down stream of the hose bibb vacuum breaker. Pic person-in-charge corrected this item by having the "y" style connection device removed and reconnected the hose without a shut off valve to the faucet of the mop sink. Please note in the future if terminal shut off valving placed on the terminal end of the mop sink faucet then a pressure type vacuum breaker an a. S. S. E. 1020 or 1013 device would be required to be installed on both the hot and cold water supply lines to the mop sink to protect the water supply from a potential cross connection and back siphon of contaminants.

Observed shelving locatd under the front counter with edging loose. To correct should secure edging to shelf for a cleanable surface

Observed reach-in refrigerator with side vent cover removed, and the cooling fins of the unit cover with an accumulation of dust. To correct schedule cleaning of the cooling fins to remove dust build up and re-install vent cover to the refrigerator

Observed a domestic dishwashing machine located at the back dishwashing area of the kitchen with a proper three compartment sink. Please note the dishwashing machine does not meet commercial standards for use in a licensed food service establishment and currently is not connected to a water supply, the equipment is not approved for the intended use and should be scheduled for removal from this facilities kitchen area

Observed domestic designed reach-in upright freezer located in the kitchen. Observed the unit operating with acceptable temperature range 7 degrees f. For cold freezing temperature storage of foods, and in good condition. Please note this unit may be acceptable for use with the following conditions be followed: on future inspections if this unit does not hold foods frozen as required or is found to not provide cold hold temperatures to maintain frozen food frozen solid or becomes no longer cleanable due to interior damage to food contact surfaces then at such time the unit will be required to be replaced with a commercial grade unit properly designed and certified to meet standards for the intended use

Observed reach-in refrigerator with side vent cover removed, and the cooling fins of the unit covered with an accumulation of dust. To correct schedule cleaning of cooling fins to remove dust build up and re-install vent cover to the refrigerator

Observed raw wood surfaces on shelving located under the front counter. Observed missing base coving under front counter area. To correct schedule repairs to seal raw wood surfaces to be easily cleanable, recommend two coats of a light color of paint to provide smooth easily cleanable surfaces.
