Observed a wet wiping cloth stored on the counter under the espresso machine. A) cloths in-use for wiping food spills from tableware and carry-out containers that occur as food is being served shall be: (1) maintained dry; and (2) used for no other purpose. (b) cloths in-use for wiping counters and other equipment surfaces shall be: (1) held between uses in a chemical sanitizer solution at a concentration specified under # 4-501. 114; and (2) laundered daily as specified under # 4-802. 11(d). To correct get a sperate container and store the noted item in the sanitizer between uses. Corrected: person in charge placed the noted item in a container of sanitizer.

(Critical) Observed facility receiving a shipment of cold time/temperature control for safety (tcs) food above 41°F, salads 60°F, goat cheese dip 47°F, and carrot chia 50°F, and quiche 60°F. Inspector informed pic that tcs food above 41°F must be rejected and returned to supplier. Pic corrected by rejectiong tcs food above 41°F and having company take back tcs foods above 41°F. Pic has been instructed to monitor and ensure cold tcs food is received at 41°F and to reject cold tcs food above 41°F.

(Critical) Found ready to eat (rte), time/temperature control for safety (tcs) food, opened cartons of half and half, milk and soup with 7 (day opened + 6 days) in the 2 door prep top cooler. Pic corrected by discarding rte, tcs food, and has been instructed to monitor and ensure appropriate foods are datemarked.

Found the spray nozzles of spray rinse arm of the dishwashing machine clogged with debris. To correct follow manufacturer directions and clean spray nozzles.

Found water not very hot flowing from the hot water valve of the right faucet in the 4 compartment sink. Pic states this faucet is rarely used as they wash dishes in the dishwashing machine. To correct provide adequate hot water to all faucets.

Found single use utensils (forks) stored in a container with handles down. To correct store forks with handles up to minimize contamination.

Found a rubber band wrapped around bend of the steam wand. To correct replace rubber band with protective sleeve that is smooth and cleanable.

Found several spray nozzles in the dishwashing machine clogged with debris. To correct follow manufacturers directions and clean spray nozzles.

Found some cookies in the front counter display without ingredient information. To correct have ingredient (allergens) information available upon customer request.

Found the edges around the ice machine door soiled with accumulation. Found the bottom of some storage drawers and shelves soiled with accumulation of debris. To correct clean the exterior of the ice machine, drawers and shelves.

Found the display cooler not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F or lower.

Found the floor, under equipment, edges and corners in the front counter prep area soiled with accumulation of debris. To correct thoroughly clean floor and keep clean.

Found a heavily loaded dishwashing rack with numerous dishes stacked and not loaded properly to allow unobstructed spray from all cycles. Corrected by educating staff to place soiled dishes in dishwashing rack in a manner that allows all items to be sprayed from all cycles.

Found the hot water rinse pressure guage on the dishwashing machine not moving while dishwashing machine in operation. To correct repair the water rinse pressure guage to function and operate at manufacturer specification of 20 psi.

Found the spray nozzles of the dishwashing machine clogged with debris. To correct clean spray nozzles and clean when clogged.

Found tcs food, goat cheese 47°F, cream cheese packets 51°F, hummus 49°F in the top portion, and all milks 49°F, cream 49°F, mac and cheese 48°F, lasagna 49°F, in the bottom portion of the 2 door prep top cooler. Pic temporarily corrected by discarding tcs food in both coolers, and has been instructed not to use these coolers to hold tcs food, until repaired/replaced to hold tcs food at 41°F or lower.

Found the ice scoop laying on top of ice in the ice machine. To correct store ice scoop in a holder or clean container.

Found the sliding glass display cooler and the 2 door prep top cooler not holding tcs food at 41°F. To correct repair/replace coolers to hold tcs food at 41°F or lower.

Found the interior bottom and sides of the 1 door reach in cooler in the basement soiled with debris and splash. To correct clean interior of cooler.

Pic temporarily corrected by having employee discard tcs food in the 2 door prep top cooler. Pic has been instructed not to use this cooler to hold tcs food, until repaired/replaced to hold tcs food at 41°F. , found time/temperature control for safety (tcs) food, half and half 48°F, various milks 48°F, soy milk 48°F, and cooked pasta 46°F in the bottom of the 2 door prep top cooler. On follow up inspection found tcs foods, artichoke dip 41°F, and guacomole 41°F, on the top portion of the 2 door prep top cooler, and found tcs foods, half and half 41°F, skim milk 41°F, and cooked pastas 41°F in the bottom of the 2 door prep top cooler. Pic states cooler has been adjusted to hold food at 41°F. Continue to monitor cold hold temperatures. Violation corrected.

Found the 2 door prep top cooler not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F. Found the edges of counter tops, shelves, and drawers with chipped and missing trim surfaces and paneling. To correct repair all countertops, shelves and drawers.

Found containers of frozen soup thawing on the counter top of the prep area. Employee corrected by placing frozen soup in the cooler. Pic has been instructed to monitor that employees are following proper thawing procedures, such as to thaw items in cooler.

Found a stack of coffee filters store in an uncovered bowl. To correct store coffee fitlers in a more protected manner, such as in a covered storage box.

Found the edges around the spout of the hot water urn soiled with lime build up. Found the bottom of the reach in cooler in the basement soiled with accumulation of debris. To correct clean as soon as possible, and clean on a more frequent schedule.

Found cups stored inverted on top shelves lined with an absorbent type lining. To correct remove absorbent type lining and replace with a nonabsorbing type lining.

Found no handwashing sign in the employee basement restroom. To correct provide sign reminding employees to wash their hands.
