(Critical) Observed facility made soup at 45f for an unknown amount of time. Potentially hazardous food that is to be cold shall be held at 41f or below. To correct discard the noted food into the garbage. Corrected: person in charge had the noted food discarded into the garbage.

Observed no hair nets or hats in use. Food employees shall wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food; clean equipment, utensils, and linens; or unwrapped single-service or single-use articles. To correct use a hair net or hat when working with food

Observed employee food improperly stored in two door cooler located in the kitchen. An employee shall eat, drink, or use any form of tobacco only in designated areas where the contamination of exposed food; clean equipment, utensils, and linens; unwrapped single-service and single-use articles; or other items needing protection can not result. To correct create an employee food storage space and keep the food sperate from consumer food.

Observed the ceiling, hood and vent to be very dusty. Physical facilities shall be cleaned as often as necessary to keep them clean. To correct clean the noted areas and clean at an increased frequency.

Observed ketchup being stored in an open metal can. Materials that are used in the construction of utensils and food-contact surfaces of equipment may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be: (p) (a) safe; (p) (b) durable, corrosion resistant, and nonabsorbent; (c) sufficient in weight and thickness to withstand repeated ware washing; (d) finished to have a smooth, easily cleanable surface; and (e) resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition. Observed single use plastic containers that used to contain other food being reused for food storage in the facility. Single-service and single-use articles may not be reused. To correct discontinue use of single service containers and purchase food grade containers for food storage.

Observed food being stored in grocery bags in multiple coolers. Equipment and utensils shall be designed and constructed to be durable and to retain their characteristic qualities under normal use conditions. To correct discontinue using grocery bags and provide food grade bags for food storage.

Observed employee working with food without a hair net or hat. Observed the shelving above the prep sink to have a soil build up and the hood above the stove in the back has a heavy dust build up nearest to the ceiling. Physical facilities shall be cleaned as often as necessary to keep them clean. To correct clean the noted areas.

(Critical) Found ready to eat (rte), time/temperature control for safety (tcs) food ham slices, turkey slices and cooked chicken breast without 7 day (day prepared + 6 days) datemark in the back kitchen 2 door reach in cooler. Found rte, tcs food, an opened carton of half and half, coleslaw and tuna salad without 7 day datemark in the 2 door prep top cooler in the cooking area. Pic corrected by providing 7 day datemarks for noted foods and has been instructed to monitor and ensure rte, tcs food is datemarked.

Observed employee working in cooking/prep area not wearing hat/hair restraint. To correct provide hat/hair restraint and remind employees to wear them.

Found the floor edges in the front counter and back kitchen area soiled with accumulation and build up. To correct thoroughly clean floor and clean on a more frequent schedule.

Found sauces stored in containers that previously held coffee in the basement storage area coolers. To correct store sauces in commercial, food grade storage containers.

Found the shelf for seasoning soiled with accumulation. Found the hood filter in the back cooking hood soiled with dusty build up. To correct clean shelf and hood filter and clean on a schedule to preclude accumulation.

Found employee personal food in various areas of coolers and freezers. To correct designate areas for personal food storage, and store personal food in designated areas only.

Pic temporarily corrected by re-arranging items in this cooler to increase airflow, as other items in cooler were at 41°F. Pic has been instructed to monitor that all tcs food is held at 41°F, and call for repair if necessary. ,

Found various single use food containers that previously held food being re-used and plastic grocery bags used to store food. To correct discontinue using single use food containers and grocery bags, and replace with commercial grade food containers.

Found paper towels lining stacked trays of inverted cups. To correct remove paper towels and replace with nonabsorbent liners if desired.
