There is a domestic freezer in use in the facility. It is in poor repair and has been excessively duct tape. The duct tape is fraying and is not cleanable attracting and holding a significant amount of food debris. Replace freezer with a commercial unit that is durable and approved for use in a food service establishment. (Critical) There is a y with flow control device attached to the faucet and a hose attached to that y on one side. Back flow preventor is not present. A hose bibb vacuum breaker must be installed on the faucet to protect the water supply if a hose is attached to the faucet. The y connector is prohibited as it has flow controls. Use a sidekick and hosebibb vacuum break. If the existing faucet does not accomodate a vacuum breaker then the entire assembly must be replaced with a built in vacuum breaker and attached approved sidekick. Bring into compliance as indicated above. ,

(Critical) Flies noted throughout the facility. Flies are to be eliminated. Do not keep rear door propped open to eliminate access for insects. Remove flies. ,

(Critical) Chemical test strips are not available in the facility. Provide test strips for chlorine in order to monitor the concentration of the chlorine. ,

The hood filters are in need of cleaning. Cextremely dusty and greasy. Clean now and place on a more frequent cleaning schedule. ,

The ledges and rollers on the pizza oven are in need of cleaning. Clean now and place on a more frequent cleaning schedule. ,

The rear door was propped open at time of inspection. Keep the rear door closed to minimize access for insects and rodents. Corrected by closing the door. ,

The thermometer for the small pepsi cooler where salads are and buffet foods to replenish salad bar are stored is missing. Provide a thermometer for this unit and for the pop cooler at the front counter which is also missing a thermoemter. Thermometers are required to properly monitor the temperatures of the foods.
