Found mouse droppings on the bottom of a pallet in the large storage warehouse. To correct keep areas clean and dry, in good repair, eliminate all cracks, gaps and openings, and continue working with pest control company.
Found the bottom shelf in the brewery walk in cooler not at least 6 inches above the floor. To correct raise bottom shelf to be at least 6 inches above the floor.

(Critical) Used a quat test kit and found the sanitizing solution flowing from the quat dispenser at least 500 ppm, above manufacturers specification of 200 to 400 ppm. To correct repair quat dispenser to dispense at manufacturers specificiation, and follow manufacturers directions to mix sanitizing solution until dispenser is repaired. This violation has repeated and a risk control plan has been issued.

Discussed the process of refilling returnable beer containers with pic, who states customers bringing bottles back to be re-filled are checked and must be clean, they attach a flexible tube to beer tap and tube is placed inside of refillable bottle and filled. To correct refilling process must be changed/modified that will prevent contamination, that is tube being placed in refillable bottle, stored in sanitizer and then re-used again.

Kitchen and dry storage room: canned goods (in the kitchen and dry storage room) and shaker containers of spices, a can of olive oil, etc. In the kitchen were contaminated with mouse urine and feces. Open containers of food were discarded and the canned goods are to be washed, rinsed and sanitized prior to use. Brewery: observed feces and urine stains on and in open bags of brewing grains. Corrected by discarding these products

(Critical) Bar: observed numerous small flies (fruit flies likely) at several bar areas (near the waste water drain for the beer tower spill trays and near the floor sink under the three compartment sink & glass washer). Trap flying adults with vinegar bait stations and/or fly paper (under the 3 comp. Sink & near the floor sink serving the beer tower spill trays--areas away from food, single service items and clean equipment--replace these traps as they become overloaded), keep the facility clean, dry and in good repair to eliminate wet debris and slime growth. Begin using a fly drain gel in your bar area drains to eliminate the slime growth in which the flies breed. Kitchen: observed mouse feces and urine on most items on the storage shelves (canned goods, shaker containers for spices, can of olive oil, single service/use items, etc. ). Mice spread disease and contaminate surfaces they urinate and defecate on while walking around in search of food and shelter. Numerous house flies were observed flying around the kitchen landing on clean equipment, etc. Utilize fly paper (in a safe location away from food, clean equipment and single service/use items) to catch these flies. Storage room: excessive mouse feces and urine stains were observed on every thing (packaged food, single service/use items, and towels) in the dry storage room. Brewery: excessive mouse feces and urine stains were observed in most areas of this space (on floor among spilled grain under the grain auger loading hopper, in the storage area on the east end of the building (among pallets, beer processing equipment, electrical items), on/in bags of opened beer making ingredients, and storage shelves, and observed a live mouse jump out of a curled up bag of beer malt. Examine the exterior of the building and seal any openings to prevent the entry of mice, and remove any weeds or plants growing adjacent to the building. In addition to having staff perform inspections of the facility and keeping the food production and storage areas clean and dry, a pest control contractor is needed asap to provide bait stations on the exterior and interior areas of the facility to eradicate the mice, and inspect the building for additional mouse entry points to be sealed
(Critical) Observed a raw egg batter for dipping raw fish in prior to cooking to be stored on the top shelf of the three door prep cooler reach in portion. Corrected by discarding this product

Observed the wall behind and the floor under the two sandwich grills in the kitchen. Observed spilled grain on the floor under the loading hopper for the grain auger, on the floor under the shelving unit for the opened bags of beer grain. Slime growth is located on the floor by the water hoses next to the walk in beer cooler. Yeast/slime growth was observed in the floor sink at the bar that receives waste water from the draft beer tower spill trays. The floor in most areas of the entire facility is in need of cleaning to remove mouse feces and urine
Observed several sections of the commercial vinyl floor tile to be peeling up and gaps under the base coving located at/near the floor sink under the bar three compartment sink. Repair this area to eliminate the gaps and to be water proof
Brewery: observed the overhead garage door to the brewing area to be left open longer than necessary, to be used for pedestrian egress, and to be damaged such that pests can enter the building. Management indicated a new, commercial coil door is to be installed soon. This work must be scheduled asap. Ensure the new door when installed is sealed on the top, sides and bottom to prevent pest entry. In addition to this new garage door, install an air curtain at this door to prevent pest entry when the door is open. Instruct employees to use a service door, not the overhead door for pedestrian traffic. Kitchen: provide a vinyl strip curtain or an air curtain at the door leading to the outside kitchen area to prevent the entry of flies.
Observed a large plastic container being used for the storage of recyclables (glass, plastic, soda cans, etc) to be stored uncovered in the brewing area. Provide a cover that fits this bulk storage tub to prevent access by pests and relocate it to a paved area outside. Doing so will reduce the attraction of pests with in the building and free up space in the brewing area
At the bar a wet wiping cloth was observed resting on the counter. Please keep wet wiping cloths stored in a solution of sanitizer at the proper strength to prevent bacterial growth between uses
Observed the two kitchen prep coolers to not be maintaining foods at required temperatures. Maintain/adjust/repair as needed to keep all foods at 41f or below
Observed food containers exposed to mice activity. Provide covered plastic storage tubs to store your food containers in until these pests are eliminated. The large bag for your frozen pretzels was observed to be wide open in the chest freezer. Please keep this bag closed to prevent contamination of the food. Observed bags of previously opened beer making ingredients on the brewery storage shelf that were not properly closed/sealed to prevent pest/misc. Contamination entry. Provide additional plastic storage containers for these brewing ingredients.
Observed a fruit fly trap (glass bottle with cider vinegar in it) located on the floor at the floor sink that receives waste water from the draft beer tower spill trays. This trap was observed to be overloaded (at least a 1/2" of dead flies accumulated in the bottom of it). Corrected by discarding this trap. Ensure your pest traps are dealt with routinely
Observed a wooden pallet being used in the dry goods storage room. Replace this pallet with a dunnage rack that has a minimum of 6" high legs to facilitate inspections for pest activity and cleaning of the floor
