(Critical) Observed a spatula and some other various utensils being taken from the wash basin in the 3 compartment sink - rinsed off and taken for immediate use. These utensils shall be sanitized after washing and rinsing prior to being placed into storage and completely air dried prior to placing into use. Corrected by placing items back into wash basin so they may undergo the proper washing/drying process. Discussed this issue with the employee handling these items and with the manager. Manager shall ensure that all employees who work in the warewashing section are retrained in proper washing/drying proceedures. Corrected

Observed the waste line from the backflow preventer on the carbonator unit for the drive thru to be weeping some liquid. This is an indication that the backflow preventer is starting to fail. To correct, please have this backflow preventer replaced as soon as possible.
Observed several wiping cloth buckets sanitizing solution to be very weak. Wiping cloth sanitizing buckets shall maintain the proper sanitizing solution concentration to preclude the growth of bacteria on the wiping cloths. Corrected by having the wiping solutions refreshed in all of the buckets with the proper quat sanitizing concentration. Corrected
At the walk in cooler condensate drain observed the drain line has fallen into the receiving drain eliminating the air gap. Correct by reestablishing the air gap
(Critical) An employee had placed a stack of serving trays in the back counter hand washing sink basin. Corrected by removing these trays. Corrected by removing these trays
Ensure staff are performing regular inspections for the presence of flies and fly breeding areas
An employee working at the drive through station was observed with jeweled/intricate rings on her hands. Employees who work this station also scoop ice for making beverages. Jewelry on the hands and wrists (bracelets, watches, rings (except a plain band)) must be removed prior to a food employee beginning their shift or they must properly wear a single use glove on that hand(s) to cover rings
Kitchen floors: many areas of the tile grout are deteriorated and/or have grout missing. In these damaged/deteriorated areas remove the remaining grout and install a new, full thickness layer of grout to eliminate the depressions that allow moisture and food debris to accumulate that can become fly breeding areas
1) the floor drain in the water heater closet is plugged and not draining. Clear the clog so the water heater condensate can drain away. 2) the "sidekick" installed on the water connection at the mop sink (the one with the black hose) is not flowing water properly from the 0. 5 gallon per minute outlet (similar to an aerator). Either clean/de-mineralize this removable device or provide a replacement outlet device. 3) re-establish the drain line air gap for the kitchen ice machine drain lines (gravity has slightly pulled down the copper drain pipes over time).
Provide an inexpensive metal stem thermometer at the dish washing station for staff to verify the minimum hot water temperature of the wash water (110f).

The floors in the following locations are in need of cleaning now and more frequently in the future to eliminate these breeding areas: the area under the drive through beverage station, under the back counter cabinets, the drain pipe under the dining room beverage station, and nearly all of the covered floor drains have debris build-up on the pipes below the floor level. Thoroughly clean and dry these areas to eliminate the possible fly breeding locations
(Critical) The upper portion of the lettuce chopper and the salad spinner parts were only rinsed off prior to placing in storage to drip dry. This equipment must be sanitized after washing with soap and rinsing. Corrected by discussing this issue with the manager and by employees sanitizing these items
(Critical) Observed an employee not use soap when cleaning the upper portion of the lettuce chopper and the parts to the salad spinner. Ensure staff are using soap during the cleaning process. Corrected by discussing this issue with the manager. Staff re-washed these items
(Critical) Observed a dial type back-up metal stem food thermometer to be reading approximately 12 degrees too cold. Corrected by discussing this issue with the manager and by removing the unit from use for calibration
The large amounts of silicone sealant that were applied to the corners of the mop sink are pulling up from the mop sink surfaces. Remove all existing sealant and after thoroughly cleaning and drying these surfaces re-apply silicone to these areas and smooth it out to provide a good bond to keep out moisture and debris
The wheeled trash cart in the back kitchen hallway is in need of cleaning of the interior surfaces to eliminate food debris build-up
Observed numerous small flies in the kitchen areas of the facility. Staff must be more diligent on maintaining the out of the way floor and drain areas clean and dry to eliminate these fly breeding areas
The cover for the front counter ice storage container for lemonade is in need of replacement due to it being cracked
Many of the out of the way floor areas (under the front and back counters, the bottom shelf under the lemonade dispenser, under the drive through beverage station, etc. ) are over-due for cleaning to remove debris (lids, straws, french fries, slime build-up) as well as all of the floor drains and floor sinks. These areas will need very regular attention (at least 3 x per week) to keep them clean and dry to eliminate the current fly breeding areas.
(Critical) Two chemical bottles were observed in the back kitchen stored with / above foods. Corrected by relocating these chemicals to a designated chemical bottle rack
(Critical) The white face-plate dispensing spouts for the two frosty machines: the outlet of the spouts are damaged (rough and have cracks, etc. ) and are no longer smooth or easily cleanable. Replace these face-plates/spouts
Observed an employee with a couple jeweled, intricate rings on her hands during food prep. Staff need to either remove their rings (except for plain bands) during their shift or utilize single use gloves (even when working with beverages/ice and non-ready to eat foods). Corrected by the employee putting on single use gloves
The floor sink drains in the following areas are unclean: under the dining room beverage station, under the drive through beverage station and at the dish washing station. Clean these areas at least 3 times per week to maintain them clean and prevent the attraction of pests
Dining room: the underside of the individual soda dispensing heads have excessive soda syrup build-up. Clean these areas daily when the spouts are cleaned

(Critical) Observed three bottles of an unapproved insecticide (raid flying insect killer) located in the office behind a filing cabinet). Corrected by removing these insecticides from the facility. Ensure only approved insecticides are used according to label directions to protect foods and clean equipment
Observed a small number of small flies at the front counter lemonade station area and two locations in the back kitchen with one fly each. Ensure staff are performing frequent inspections for both the presence of flies and possible breeding/feeding areas.
1) observed no tile grout between the floor tile located under the drive through beverage station--provide tile grout in this area after cleaning. 2) mop sink area--observed the sealant to be in need of replacing at the junction of the tile base tile and the frp wall material. Remove old sealant, clean and dry the surfaces and reapply a silicone sealant to prevent the mold/moisture issues at this location
Observed the interior bottom of a trash container in the back hallway to be in need of cleaning to eliminate the food debris and moisture accumulation
Observed the following areas to be in need of cleaning at this time and more frequently in the future: 1) the floor and the floor to wall junction under the front and rear counter cabinets; 2) drive through beverage station--the floor drain and flooring under this station; 3) dining room beverage station--the floor and floor drain in the base cabinet of this unit; 4) the utility closet with the water heater--the floor in general and around the base of the water heater; 5) the mop sink basin area; 6) around the base of the waste oil holding tank; and 7) the floor sink for the three compartment dish washing sink
Observed a small number of little flies in the facility: at the dining room beverage station, the area of the hand washing sink at the front service area, and the back kitchen area near the bag in a box soda system storage rack. Ensure staff perform frequent inspections of the facility to identify problem areas where flies can breed and to monitor the adult fly population
A few areas of the kitchen tile flooring are in need of repair due to loose and/or cracked tile, as well as, well as worn or missing tile grout. Repair as needed to eliminate these depressions that trap food debris and moisture
Observed the interior surfaces of the clear flexible drain lines for the two dining room iced tea stations to have a lot of accumulated residue in them. Replace these drain lines to eliminate a possible fly breeding area
The plumbing at the mop sink was changed again such that the "side-kick" bleed off tee fitting was installed in the wrong location with respect to a "y" splitter. Corrected by removing the "y" splitter and re-installing the "side-kick" to the mop sink faucet
Several out of the way flooring and drain areas are over-due for cleaning to remove debris build-up, such as: the floor sink for the dish washing station, around the bulk frying oil storage tank, the floor and drain under drive-thru window beverage station, the floor in the employee bathroom, the floor under the front counter and the floor sink for the dining room beverage station. Clean these areas now and on a more frequent basis to eliminate debris accumulation
(Critical) 1) the water line serving a chemical dispenser at the three compartment sink does not indicate it is an approved potable water hose and there is no back flow preventer just upstream from this hose. This situation can be corrected by providing a food grade hose to connect the potable water supply to the chemical dispenser (the dispenser itself has a back flow preventer to prevent chemical back-siphoning). 2) mop sink: the faucet does not contain a back flow preventer. Attached to the end of the spout is a "y" splitter. One branch is connected to a chemical dispenser via a non-potable water hose. The other branch has an atmospheric vacuum breaker and a short length of open ended hose to fill a mop bucket (no violation for this branch). As a correction install an ecolab "sidekick" flow-through back flow preventer (or equivalent) on the branch of the "y" splitter leading to the chemical dispenser
(Critical) Observed the guacamole and pico de gallo in the front counter and drive through window stations to be at 44 to 57f. Corrected by discarding.
Observed the drive through sliding window (for food payment) to not be closing properly. Repair this window as needed so it functions as intended
Re-establish the air gap (vertical height = 2x the discharge pipe diameter) for the two compartment food prep sink basins above the flood rim of the drain under the three compartment sink or alternatively, provide air gaps under each individual basin.
The following areas are in need of cleaning now and more frequently in the future: the floor and drain under the drive through beverage dispensing station, the drain in the lobby base cabinet for the beverage dispensing station, and the floor drain at the front of the ice machine
Observed the bottom of the wheeled trash hauling container to have a lot of spilled food in the bottom of it. Please increase the cleaning frequency of this container. Ensure the wastewater from cleaning this containers is properly routed to the sanitary sewer
Adjust/repair as needed the two prep top stations (front counter and drive through) to hold phfs at 41f or below
