Food employees shall wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food; clean equipment, utensils, and linens; or unwrapped single-service or single-use articles. Observed no hand washing sign available in the men's bathroom which is used by employees. A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees. Observed the following non-food contact surfaces soiled with a debris build up: 1. Top of the fry warmer on the production line. 2. Pipe next to the cook top, located next to the wall that separates the cook line from the one door tall freezer. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. To correct clean the noted areas and maintain them clean of debris build up. ,
Observed the hand sink to have restricted water flow nearest to the 3 compartment sink. A plumbing system shall be: (a) repaired according to law;(p) and (b) maintained in good repair. To correct repair the noted sink to unrestricted the water flow. ,
(Critical) Front counter hand washing sink: when the hot water valve is opened, water only trickles out of the faucet. Trouble-shoot the problem and repair as needed
(Critical) The following food contact surfaces were observed to be unclean: the surfaces on both soda dispensing machines where the spouts/diffusers connect (contact coca-cola for a brush kit to clean these difficult surfaces); eight of the handled hot holding plastic food pans, several metal pans (1/3 and 1/9 pans); four or five of the plastic 1/6 pans for hot holding the cheese sauce. Corrected by re-cleaning these items.

The two dining room island trash cans had trash in them over night. Ensure staff are removing all trash at the end of each night (or tying off the bags) to prevent pest access
Observed two of the wheeled trash carts to have old liquid waste in the bottom of them that has become sour. Thoroughly clean these carts regularly
The floors and drains in the following areas were observed with excessive build-up: under the drive through beverage station, under the three compartment sink, under the two compartment food prep sink, under the ice machine, under the fry holding station and the deep fryers, and around the door frame of the door to the mop closet. Thoroughly clean and maintain these areas to prevent an attraction for pests
(Critical) 1) phfs (salad and sour cream) to be at 58 to 60f in the drive through area under counter one door refrigerator. Corrected by discarding the sour cream and four salads. 2) the top layer of tomatoes in an open top 1/6 pan on the top of the assembly line by the ice machine was at 46f. Corrected by discarding this top layer of tomatoes and providing a cover to keep the remaining tomatoes at 41f or below
The two rolling trash carts have moist debris that has accumulated in the bottom of them. Thoroughly clean these carts out at the mop sink at this time and as needed in the future
Observed an employee with a jeweled ring cooking and dispensing french fries. Employees preparing/cooking/dispensing foods (including fries, drinks, ice, etc. ) must either remove engraved/jeweled rings or they must wear a single use glove over that hand to prevent jewels from getting into food should they fall out and to prevent food debris from becoming lodged in the ring
Observed excessive moisture on the floor at the base of the hot water heater. It appears the condensate drain connection to the exhaust vent is leaking. Repair this fitting as needed to eliminate the dripping water
The following areas is in need of cleaning at this time to remove accumulated debris: 1) the floor sink basin and the floor under the drive through soda/ice station; 2) the floor to wall junction under the three compartment sink; and 3) the floor behind the soda syrup rack and the small co2 tank. Maintaining these areas clean on a regular basis will prevent odors and eliminate a food source/breeding area for pests
1) the drive through area silver king one door under counter refrigerator is in need of repair/replacement so it can maintain 41f or below. 2) observed one of the yellow plastic hot holding pans to be cracked in the center of the bottom. Corrected by discarding

The floor under the soda system station at the drive through area has a significant build-up of debris. Thoroughly clean this area now and more frequently in the future
The one door low-boy refrigerator at the drive through station used for salads wasn't operating at 41f or below. As soon as possible adjust and/or repair as needed to maintain phfs at 41f or below. The gm indicated the maintenance employee will be arriving later to repair an a/c unit and will also look at this refrigerator
The drain lines for the ice machine ice holding bin and the walk in cooler were out of position and not properly discharging into the floor drains. Corrected by staff re-positioning. Also, it appears one of the three soda system carbonator is weeping water from the back-flow preventer. Contact your soda system company to repair/replace this back-flow preventer as needed

Observed the two drain lines at the large ice machine (one from the ice maker and one from the ice bin) to be pushed out of place. Re-position and secure these drain lines so they route wastewater into the provided drain through the floor
Observed spilled garbage in the bottom of the wheeled waste hauling carts. Regularly clean these carts of spilled debris to prevent odors and the attraction of pests
Observed the sliding door on the side of the trash dumpster to be left open. Corrected by sliding the door closed to keep the birds and other pests out of the garbage
(Critical) Essentially all of the plastic hot holding pans for the chicken have old, stuck-on oil residue stuck to them (20 total pans). Either thoroughly soak and clean these pans or replace them so staff can work on maintaining new pans in a clean state. Observed a smaller, handle-less yellow plastic pan with old, stuck-on oil residue, and a metal 1/3 pan with some food build-up around some dents in the corners. These two pans were sent to soak and be cleaned. Also, observed the blender for the shake mixer to have shake residue on it from the previous day. Staff immediately cleaned and sanitized this blender
Observed a couple small flies located around the base of the hot water heater where the water has accumulated. Ensure someone is inspecting the facility frequently for the presence of pests
Observed the following areas/items to be unclean: 1) the floor and floor sink under the drive through beverage station (the frosty machine needs to be removed to gain better access to this area); 2) the floor and floor sink under the dining room beverage station; 3) the floor-wall juncture under the three compartment sink; 4) and the dust pan has excessive debris build-up in it; 5) the floor and floor drain at the hot water heater. Clean these areas now and more regularly in the future to maintain the facility clean and dry
1) a water leak was noted on the soda dispensing tower (water is running down the metal panel above the spill tray) at the drive through station. Locate and repair this leak as needed. 2) the condensate drain for the hot water heater vent pipe was detached allowing a moderate amount of water to drain onto the floor. Clean and glue this fitting so it is watertight. 3) observed a leak in the connection of the drain hose to the dining room beverage station spill tray. Provide a hose clamp, etc. As needed to eliminate this leak
Observed an employee with fingernail polish on her nails scooping ice without a glove on her scooping hand. Either remove fingernail polish or wear an intact single use glove on your scooping hand
Observed the new clear plastic cups & lids for iced tea, etc. To be stored adjacent to the hand washing sink at the drive-through window station. These items are being splashed when the sink is used. Find a way to relocate these cups/lids to protect them from splashing

The gasket on the prep unit door is torn. Once torn it is no longer cleanable. Replace split gaskets. The tub used for storage of equipment that is not in use (in the freezer) is cracked and broken). Replace with a storage tub that is intact so that it is smooth and cleanable.
