(Critical) The iceberg lettuce that was prepped this morning (+/- 7-9 am) was found in the walk in cooler to still be at 49 to 50f. When the lettuce is prepared it is placed into a large plastic pail of cold tap water. Corrected by discarding this lettuce. In the future, either use pre-chilled water or cold tap water with added ice to ensure the lettuce cools quickly

(Critical) Observed a "y" splitter with shut-off valves installed on the end of the mop sink faucet. This mop sink faucet contains an atmospheric vacuum breaker. Atmospheric vacuum breakers may not have any down-stream shut-off valves. As a correction install a wasting/bleed-off tee between the faucet and the "y" splitter

(Critical) The following food contact surfaces were observed to be unclean: two plastic hot holding 1/3 pans, three metal 1/3 pans, the auger for a frosty machine, the brush for the frosty machine freezing drum, two salt shakers for the fry station, the lettuce chopper, and one hot holding pan for chicken. These items were cleaned and sanitized--corrected
(Critical) Observed almost all of the yellow, temperature resistant plastic pans (handled and handle-less ones) to either have old stuck-on food debris or oily residue. Thoroughly clean these pans now and ensure staff keep up on the cleaning of these pans on a daily basis. Staff started to clean these pans and rotate them into use
(Critical) Observed numerous small flies (likely vinegar/fruit flies) in many areas of the facility. Asap schedule a joint appointment with myself, your pest control contractor and your maintenance employee to go over areas that need cleaning, treatment and repair to eliminate fly breeding areas.
The gaskets on the door of the reach in freezer have some debris build up. Clean now and place on a more frequent cleaning schedule.
