(Critical) Observed a metal garbage can used to store sugar in the basement storage room adjacent to the office. Materials that are used in the construction of utensils and food-contact surfaces of equipment may not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions shall be: (p) (a) safe; (p) (b) durable, corrosion resistant, and nonabsorbent; (c) sufficient in weight and thickness to withstand repeated warewashing; (d) finished to have a smooth, easily cleanable surface; and (e) resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition. To correct replace garbage can with one approved for food storage. ,

(Critical) Observed the following food holding between 42-45f for an unknown amount of time in a non-working cooler: 1. Quiche 42-45f. 2. Yogurt 45f. 3. Cheese cake 42-45f. 4. Several different kinds of wraps 41-45f. Potentially hazardous food that is to be cold shall be held at 41f or below. To correct: move the noted food that's 42f or less to a working cooler and discard the food into the garbage at 45f. Corrected:person in charge moved the noted food at 42f to a working cooler and discarded the rest into the garbage

Chemical sanitizers and other chemical antimicrobials applied to food-contact surfaces shall meet the requirements specified in 40 cfr 180. 940 tolerance exemptions for active and inert ingredients for use in antimicrobial formulations (food-contact surface sanitizing solutions). (p) to correct have machine adjusted to sanitize at 50-100ppm chorine. ,

Observed the following: 1. All gaskets on the main level are torn. 2. The display cooler is not holding food at 41f or below. 3. Ice cream cooler has a heavy frost build up. Equipment shall be maintained in a state of repair and good condition. To correct: 1. Repair or replace the noted gaskets. 2. Repair the noted cooler to hold cold food at 41f or below. 3. Cloths in-use for wiping food spills from tableware and carry-out containers that occur as food is being served shall be: (1) maintained dry; and (2) used for no other purpose. (b) cloths in-use for wiping counters and other equipment surfaces shall be: (1) held between uses in a chemical sanitizer solution at a concentration specified under # 4-501. 114; and (2) laundered daily as specified under # 4-802. 11(d). (c) cloths in-use for wiping surfaces in contact with raw animal foods shall be kept separate from cloths used for other purposes. Observed the left side of the one compartment sink above the dish washing machine not sealed to the wall. Equipment that is fixed because it is not easily movable shall be installed so that it is: (1) spaced to allow access for cleaning along the sides, behind, and above the equipment; (2) spaced from adjoining equipment, walls, and ceilings a distance of not more than 1 millimeter or one thirty-second inch; or (3) sealed to adjoining equipment or walls, if the equipment is exposed to spillage or seepage. (Critical) Found the steaming pitchers for various coffee drinks not cleaned for over 5 hours. Pic corrected by washing steaming pitchers in the dishwashing machine and was instructed to wash the steaming pitchers at least every 4 hours.

Found an opened bag of sugar stored in a container without lid. To correct cover container and protect sugar from contamination.

Found the side cabinet besides the dishwashing machine in poor condition with damaged and uneven surfaces. To correct repair/replace side of cabinet.

Found single use utensils (forks, knives and spoons) stored mixed in the customer self service area. To correct store single use utensils with handles up to minimize contamination.

Found the exterior surfaces of coffee storage containers soiled. Found the bottom of the 2 door under the counter cooler soiled with spilled milk and food debris. Found the door gaskets and door tracks of coolers soiled with accumulation of debris. To correct clean noted areas and clean on a more frequent schedule.

Found the protective shipping lining still on the edges of the ice machine door sealing areas and starting to peel. To correct carefully remove protective shipping lining from door sealing areas of the ice machine.

Found the coving on the wall under the 3 compartment sink loose and laying on the floor. To correct resecure coving to wall.

Found mop left in mop bucket, broom and other cleaning utensils stored on the floor of the mop closet. To correct hang mop, and store all equipment and utensils in an orderly manner off the floor.

Found several spray nozzles of the spray rinse arm in the dishwashing machine clogged with debris. To correct follow manufacturer's directions and clean spray nozzles.

Found an opened case of bottles and lids used for cold brew in an opened bag/case not covered in the basement storage area. To correct store utensils in a protected manner that is cover utensils and protect from contamination.

Pic corrected by removing items for sale in cooler and labelled these items for staff. Pic was told these items are not recommended to be given to staff and was instructed not to store tcs food in this part of cooler until repaired/replaced to hold tcs food at 41°F. Observed the presence drain flies in the floor drain area of the 3 compartment sink. To correct keep areas clean and dry, in good repair, and continue fly control measures.

Asked employee how ice tea dispenser is cleaned, employee state ice tea dispenser is washed in the washing machine, and the beverage spout is flushed. To correct follow manufacturer directions and properly clean ice tea dispenser, that is, take apart spout and wash, rinse and sanitize all parts of the spout.

Pic corrected by discarding noted items, and has been instructed not to use this cooler, until repaired/replaced to hold tcs food at 41°F. Note: a long plate was left on top of the air vent which blocked air flow in this cooler. ,

Pic corrected by having employees remove carafes passed it's time control mark, display fresh cream and milk in new carafes in the self service area, and mark the time on the time control log sheet. Discussed with staff the process they use to cool soup at the end of the day, and found staff placing soup to be cooled in plastic storage containers. To correct use shallow metal pans to rapidly cool soups from 135°F to 70°F in 2 hours, and then from 70°F to 41°F in 4 hours.

Observed an opened bag of water softener salt and a case of food supplies on the floor of the basement storage area. Pic corrected by placing items on plastic pallets.

Found no chlorine test kit available for testing dishwashing machine and sanitizing buckets. To correct provide chlorine sanitizing kit.
