Observed floor tiles in back kitchen area in poor repair (tiles are old and are not smooth and easily cleanable anymore). As a result, dirt and debris are embedded in tiles. To correct, please replace old floor tiles as needed so all floors are smooth and cleanable

(Critical) Repeat violation: observed interior faceplate and top condenser area of ice machine in back kitchen area with an accumulation of slime mold and debris. Interior food contact surfaces of all ice machines need to washed, rinsed and sanitized on a regular cleaning schedule and must be clean to sight and touch. To correct, please remove all ice from machine and wash, rinse and sanitize all interior surfaces as soon as possible. I will check this item at follow up inspection. Correct within 10 days. Please note: this violation has been cited at 2/3 routine inspections. If violation is cited at anytime within the next year, enforcement actions will be required. ,

Observed several boxes of deli paper stored under handsink sewer line in front prep area. All food and s/s items must be stored in a clean, dry area and not exposed to potential sources of contamination. Pic corrected by relocating deli paper to approved location at time of inspection. (Critical) Meatballs with sauce and chicken breasts in hot holding unit not maintaining at proper temperature. Potentially hazardous food shall be maintained at 41°F or below or 135°F or above. Corrected at time of inspection by reheating meatballs and sauce found at 129°F - 133°F and chicken breasts found at 114°F - 127°F in the microwave oven to >165°F. Employee used thermometer to verify proper reheat temperature before placing food back in hot holding well. After checking setting on hot holding cabinet appears was set too low per pic; adjusted to higher setting. Pic will monitor throughout the day to assure all food maintains at a minimum 135°F in the hot holding unit.

Water spray bottle found without label. Please label all working containers of food to avoid misuse

Some black insert pans stacked wet. Please use additional rack that has been provided to dry/drain dishes prior to stacking. Per employee the person closing last night stacked dishes prior to leaving even though they were still wet. Please leave the dishes until the next morning to stack if they are still wet

(Critical) Observed open cup rear kitchen. Food employees may drink from closed beverage containers. Critical corrected: at time of inspection lid and straw provided for open cup. Discussed use of closed beverage containers to prevent potential contamination

These areas in need of cleaning: handle area bottom microwave oven, floors under/near ice machine/mop sink, floors under shelving unit next to wif, floors rear storage room, interior bottom cabinet under pop dispensing machine and interior drain under pop dispensing machine. Please clean now and as often as necessary to maintain in clean condition

Some fruit flies observed near pop dispensing machine front counter. Please continue to work with your extermination company, orkin, to eliminate the presence of flies in your facility. This area needs to be kept clean and dry including area under cabinet and interior drain

Critical corrected: at time of inspection reheated meatballs and sauce to >165°F using microwave oven. Remember to check several meatballs to assure all parts of the food reach a temperature of at least 165°F. Meatballs found at 120°F - 122°F; other food items in hot holding well found at >145°F. Per pic back-up meatballs were just reheated.

(Critical) Potentially hazardous food items found overstacked on top of sandwich make line at improper temperature. Critical corrected: at time of inspection discarded all potentially hazardous food items found >41°F overstacked on top of sandwich make line. Some trayed meat items found as high as 50°F overfilled in pan. Please continue to monitor food levels to assure all potentially hazardous food items are maintaining at 41°F or below at all times. Please remember wrapped deli meats and trayed deli meats are insulated from the cold sides of the pan so they will need to be lower in the pans then the other phf items
