Your facility does not have anyone that works here full time that it manager certified in food safety. You have 90 days to get someone certified. My 12/30/14 you need to get a certificate over to me, via e-mail, fax or delivery. Office conference required if i don't recieve a certificate by that date!
The door to the employee restroom is not self closing. Adjust/repair so that the door closes completely by itself to minimize attraction/access for insects and rodents.

The pop nozzles are in need of cleaning. Clean now and place on a more frequent cleaning schedule.
The cookie sheet liners were found with cookie food debris on them on storage shelf. Clean now and retrain in proper cleaning technique or in frequency of cleaning as needed.
Inadequate lighting in the walk in cooler. May be due to blocked light from boxes stacked so high. Reorganize to allow adequate light and or replace bulb with a larger watt bulb for adequate lighting
(Critical) At onset of inspection employee was observed to be eating in the rear prep area during prep and going back to food service at front counter without washing hands. Observed touching oven pan rack. Discussion with employee regarding correct procedures and to wash hands prior to going back to food service. Corrected after discussion regarding correct procedures and washing hands
5 gallon containers pickles on the floor in the walk in cooler. Keep all foods stored a minimum of 6" above the floor to allow access for routine cleaning.
(Critical) There wasn't a pic in the facility at time of inspection. There is only a pic in the facility monday, wednesday and friday day shift. There is no person in charge on tuesday, thursday and evening shift. There must be a pic in the acility during all hours of operation. Provide a manager immediately that can demonstrate knowledge.
