(Critical) Observed food such at the pot pie, shredded beef and facility made soup at 47-50f in an unplugged cooler for about 1 hour. Potentially hazardous food that is to be cold shall be held at 41f or below. To correct move the noted food to the walk-in cooler to rapidly cool it to 41f or below. Corrected: person in charge had the noted items put in an ice bath in smaller portions to rapidly cool it all to 41f or below

Food employees shall clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles (p) and: (a) after touching bare human body parts other than clean hands and clean, exposed portions of arms; (p) (b) after using the toilet room; (p) (c) after caring for or handling service animals or aquatic animals as specified in # 2-403. 11(b); (p). (d) except as specified in # 2-401. 11(b), after coughing, sneezing, using a handkerchief or disposable tissue, using tobacco, eating, or drinking; (p) (e) after handling soiled equipment or utensils; (p) (f) during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing tasks; (p) (g) when switching between working with raw food and working with ready-to-eat food; (p) (h) before donning gloves for working with food; (p) and (i) after engaging in other activities that contaminate the hands. Observed the ceiling in the kitchen in the prep and cook line area to be dusty. Physical facilities shall be cleaned as often as necessary to keep them clean. To correct clean the noted area ad clean at a greater frequency.
Observed the prep cooler below the slicer not working properly. Note: unit had become unplugged about an hour ago during cleaning. Equipment shall be maintained in a state of repair and good condition. To correct have the noted unit repaired to hold cold food at 41f or below. Corrected: person in charge had the unit plugged back in and was noted to be working properly
Observed the hand sink in the kitchen to have faucets to be in poor repair. A plumbing system shall be: (a) repaired according to law;(p) and (b) maintained in good repair. To correct have the noted faucets repaired.

Equipment food-contact surfaces and utensils shall be cleaned: (1) except as specified in # (b) of this section, before each use with a different type of raw animal food such as beef, fish, lamb, pork, or poultry; (p) (2) each time there is a change from working with raw foods to working with ready-to-eat foods; (p) (3) between uses with raw fruits and vegetables and with potentially hazardous food (time/temperature control for safety food); (p) (4) before using or storing a food temperature measuring device; (p) and (5) at any time during the operation when contamination may have occurred. Raw salmon stored above ready to eat foods in the small top loading reach-in cooler at the end of the cook line. Food shall be protected from cross contamination by: (1) except as specified in (1)(c) below, separating raw animal foods during storage, preparation, holding, and display from: (a) raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables,(p) and (b) cooked ready-to-eat food; (p) to correct move the noted raw food down and away from all ready to eat foods. Found first aid spray stored on prep table in the back kitchen area. Found various utensils, tongs and ladles on the clean utensil storage shelf in the dishwashing area soiled. (Critical) Found time/temperature control for safety (tcs) food, baked potatoe 44°F, stuffed mushrooms 44°F, and sauce 45 4°F in 2 door prep top cooler on the cooking line. Found tcs food, cheese 44°F, cheese cake 44°F and rhubarb and spinach pies 44°F in the 3 door worktop salad/dessert cooler. Found tcs food, several containers of tomatoes 49°F in the bottom shelf of the 1 door reach in cooler (heavily stocked with food). Pic corrected by discarding tomatoes, a refrigeration repair person arrived to service coolers immediatedly and stated cooler coils were plugged, cleaned, and coolers now have air temperatures of 39°F and lower. Asked employee to explain how the slicer is cleaned. Employee states slicer is broken down, parts are taken to the dishwasher to run through the dishwashing machine, and the base/remaining parts of the slicer is wiped with sanitizer. Corrected by educating employee that slicer parts are to manually washed, rinsed, and sanitized. Pic has been instructed to monitor and retrain employee on properly cleaning slicer.
